EON PARKSPRESIDENTA WORDFROM THEEXECUTIVE DIRECTORAs we conclude our forty-ninth year of service, I amtremendously proud of the mission-directed work ofMDPYFS, which has positively impacted the outcomesof countless young people and families in New YorkCity and beyond. Our work has never been morechallenging, yet it is done at a high level by atremendous team of talented individuals who aregenuinely dedicated to our Lasallian Culture of Care®model, which emphasizes “touching hearts, instillinghope, and transforming lives.”In FY 22, we served approximately 1,000 individuals inour continuum of care. Our community of servicerecipients includes young people and families in ourfoster care and juvenile justice residences, childrenand family treatment support services, fair futures,and care coordination through health homes services.Our programs make a difference and have positiveoutcomes when we examine their impact on theindividual, family, and community. These programsallow MDPYFS to build stronger communities, and weknow that to help our young people and familiessustain the success they have achieved while in carewith us, they will need stronger communities to drawfrom. OUR MISSIONAt Martin de Porres Youth & Family Serviceswe offer the opportunities necessary tomaximize the potential of the youth andfamilies entrusted to our care, and we believe,that when given the means and support,everyone can become more self-sustaining.Our Trauma Informed Lasallian Culture ofCare® model embraces all without distinctionand our programs and interventions aredeveloped to “Touch Hearts, Instill Hope andTransform Lives.”OUR VISIONWe envision a city in which no childor family is left marginalized orvulnerable to the intractable ills ofsociety. The model of care, which is the hallmark ofMartin de Porres Youth and Family Services(MDPYFS), is the trademarked LasallianCulture of Care, a trauma-informed practice.The Lasallian Culture of Care (LCC) has itshistorical roots in 17th Century France, withthe founding of the De La Salle ChristianBrothers serving the “children of theartisans and the poor, abandoned, and leftto themselves – far from the wholeness oflife.” This model of care was adopted andenhanced by MDPYFS upon our founding (in1974) and is used today to providecare for all youth who have been and areentrusted to our care. The foundationalelements of the Lasallian Culture of Carefocus on “touching hearts, instilling hope,and transforming lives.”LASALLIAN CULTURE OFCAREHighlight the young person’s Resiliency.Exploring the young person’s PotentialEstablishing Relational Connections for theyoung person to Caring, PositiveCollaborators.Guiding the young person to Empowermentand SustainabilityWe do this by placing the highest emphasis on thebuilding and nurturing of supportive, positiverelationships with our youth and families. The LCCis a relational model built on developing trust withour young people. It is a model that encouragesand practices positive encouragement and thebuilding of self-esteem, seeking to knowindividual rather than group needs. The LaSallianmodel develops a sense of family and community, emphasizing correction rather than punishment.We require staff to embrace specific values andvirtues to be mentors and role models, working"together and by association." Our model requiresstaff to view their work not as a job but as avocational journey and commitment. | 7 Helps us to put into practice our values and training for the benefit of those entrusted to us.P R U D E N C ECHILD & FAMILY TREATMENT & SUPPORT SERVICES(CFTSS)DEPARTMENT HIGHLIGHTIn FY 22, MDPYFS made great strides in expanding our vision of building stronger communitieswith our programs that provided services to young people and families in their homes andcommunities. Our community-based programs, Care Coordination through Health Homes, andour Children and Family Treatment Support Services (CFTSS)/Home and Community-BasedServices (HCBS) programs, collectively began serving approximately 60% more clients. Theseprograms allow MDPYFS to reach into homes and communities where crucial support isnecessary to meet real-time needs. These programs also ease the stress, inconvenience, andpossible financial burden placed on young people and families of having to travel to meet theirservice provider at an inconvenient location outside of their community.Ms. Najimah McKenzieNajimah McKenzie's journey at Martin de Porres Youth andFamily Services began in 2015 as an intern at the QueensVillage close to home (CTH) facility. She then transitionedto roles such as HR/Finance Associate, Full-Time ServiceProvider, and Billing Coordinator. In January 2021, sheassumed the role of Director of Children & FamiliesTreatment Special Services (CFTSS) & Healthcare Billing &Collection Services (HBCS) . Her diverse experiences acrossdepartments have contributed to the success of the CFTSS& HCBS Departments. Najimah leverages her insights toguide Service Providers in building client relationships &delivering quality services, while also understandingparental expectations. She continually embraces growthand remains open to innovative ideas in her journey.Ms. Emelda AgardMs. Emelda Agard's journey with Martin de Porres Youthand Family Service began in March 2013 as an internfrom York College. Her roles evolved from Intern toYouth Specialist, Coordinator, and Director. Currentlyserving as the Director of the Medicaid Managed CareDepartment, she supervises nine Care Managers andmanages a caseload of 212 members. Ms. Agardpassionately believes in the rewarding impact of herwork with youth and families. Her unique positionrequires her to provide guidance and support,reinforcing her sense of purpose and the difference shemakes. She aspires to grow professionally, advocate forothers, and promote efficacy and equity in all endeavors.

Everytime that I’ve asked for help with mydaughter, Martin de Porres has beenthere even though she is not in theprogram.” “I didn’t have much time with my kids,but Martin de Porres made it happenwith their activities. I always lookforward to the family activities andhaving my family all together. Everyoneis so wonderful there.”-Mrs. Shabana AliPart of our mission at Martin de Porres Youth &Family Services is to foster a loving-familyenvironment for children and parents to thrive.That mission is made so much more difficultwhen the children have been displaced.MDPYFS watched the Ali family go throughtheir separation and then helped them in theirreunification.Though saddened by the separation of threechildren at three different agencies, this familyhas found a way to remain united. It was theoutstretched hands of the mother and fatherthat kept their children close. With the supportof MDPYFS, this family has persevered throughtheir darkest days and has stepped into abrighter future.Mrs. Shabana (Mother to Ismaael Ali) says, “Ihave three kids at different agencies, and I tellthe two other agencies that Martin de Porres isbest. | 11 Gives us the ability to discern what is true, right and important in our service to youth and families.W I S D O MDYA l e x n d r oSI Alexandro Ramirez was challenged withbeing discharged from Martin De Porresinto the Marine Corps. I was young and thesole provider for my immediate family. Iwas only able to keep my faith and keepgoing due to the continuous support ofMDP. I didn’t have money to feed myself attimes and I would reach out to my favoritestaff Domo. I would ask Ms. T occasionallyfor money due to it coming with a speech. Ioften thought to myself how I wouldsurvive without the support from my staff.I look at them as family, not staff. I havelearned a lot about myself and how toapproach the real world. I didn’t likefollowing rules or hearing the staffencourage me when I was a resident. I amnow a leader who continues to encouragemy colleagues and wife. I owe my successto Martin De Porres. I will forever bethankful and grateful for Martin De Porres,an agency who became my family. I wouldlike to thank everyone but a special shoutout to Ms. T , Sean, Domo and Charles.When I make it big I won’t forget about youguys. P.S I am now on my third promotionin the Marines.
