EXECUTIVE SUMMARY Sacred Heart Rehabilitation Center Inc was founded as a charitable service organization in June of 1967 to serve indigent males afflicted with alcoholism Sacred Heart has changed greatly over our long history to meet the changing needs of our clients This Strategic Plan reflects that tradition of progressive change to assure the continued viability of Sacred Heart and our services The FY 2023 2027 Strategic Plan builds on the previous strategic plan and takes into consideration the ongoing issues that we anticipate facing due to the pandemic This plan continues the course of forward movement for Sacred Heart while adapting to the challenging workforce development environment that Michigan is facing The plan reaffirms the commitments of the Board of Directors regarding general corporate direction Maintenance of mission Preserve independence of operation Pursuit of principled growth and commitment to fiscal integrity Explore opportunities for integration affiliation and acquisition and Continue development of services including Prevention Treatment and Recovery Supports The plan reaffirms our rich legacy of charitable service as reflected in our mission statement The plan also remains rooted in a far reaching vision that calls upon the organization to reach for a higher standard of service excellence in pursuit of that mission As we pursue that vision we remain committed to our core values The FY 2023 2027 Strategic Plan has been developed to guide the organization through the new normal that behavioral health organizations are managing due to the pandemic We do this while continuing to focus on comprehensive and compassionate services doing things the Sacred Heart Way Sacred Heart along with other behavioral health organizations will experience ongoing workforce recruitment and retention issues service costs that are accelerating at a faster pace than our current reimbursement structure an increased demand for services all while we continue in the transitional phase of the pandemic Many things have been put on hold during the pandemic and Sacred Heart still remains uncertain for the

foreseeable future on the continued impact of COVID 19 on the organization and services due to COVID 19 Despite upcoming challenges Sacred Heart continues to be flexible in adapting to changing needs without losing focus on our mission of helping individuals with limited means We work closely with the consumers communities throughout the state and with our funding sources to align our services to meet overall needs to do our part in making Michigan a healthier state The Strategic Plan describes the makeup of corporation today and lays out the plan for the coming five years outlining how we intend to continue to grow and develop as a corporation of service with an increased emphasis on utilizing technology Sacred Heart has a long legacy of service to the less fortunate in our communities and this Strategic Plan continues to affirm that charitable service commitment Actualizing this commitment to provide charitable care requires sound fiscal discipline a commitment to improving the cost effectiveness of service delivery and support and intelligent investment to enable growth and development It also requires a skilled staff which given the tools resources and support will enable them to maximize their ability to succeed at serving their customers Finally we continue to see a role for Sacred Heart in advocating for the rights of our clients to live lives of maximum achievement and fulfillment To guide our efforts we have established three strategic initiatives with task oriented objectives beneath each initiative reaching across a five year period The initiatives are 1 Continue the growth and development of the corporation 2 Maximize the use of information technology in providing services and to support operations 3 Maximize opportunities for our staff to succeed in serving clients To be effective the implementation of the strategic plan must be the responsibility of the President CEO The Leadership Team will actively monitor the strategic initiatives and the objectives ascribed to each with updates given to the Board of Directors through our routine quarterly reports 5

OUR LOCATIONS SERVICES Sacred Heart offers an extensive network of specialized services ranging from withdrawal management detoxification to peer support residential to outpatient including medication assisted and drug free therapies and HIV AIDS care management services housing and other recovery support services Services are provided at 12 locations throughout Michigan 217 3 9 Residential Beds Varies on of children 45 Detoxification Beds Residential Facilities Outpatient Locations 50 Supportive Housing Capacity 11

OUR HISTORY In 1967 Sacred Heart started off in the City of Detroit in a donated building Many things have changed over the last 55 years but our commitment to our mission remains the same to help individuals with limited means receive the treatment they deserve PAULA NELSON Sacred Heart President CEO Sacred Heart collaborated with a health system in Southwest Michigan to develop a medication assisted treatment program in the health system s walk in clinic Opened St Clair Shores Outpatient a comprehensive health and wellness center offering drug free and medication assisted outpatient services The Macomb County Health Department started providing HIV testing at the Richmond Township Residential program Opened a new recovery home in Madison Heights with an emphasis on individuals engaged in medication assisted treatment services Sacred Heart partnered with Oakland Community Health Network and the Oakland County Sheriff s Office to develop a medication assisted treatment program inside of the jail in Pontiac Partnered with Region 10 Prepaid Inpatient Health Plan and the St Clair County Sheriff s Office to develop a medication assisted treatment program utilizing all three FDA approved medications with all services provided at the jail We also started providing medication assisted treatment services at the jail in Macomb County Opened two recovery homes in Eastpointe for individuals engaged in medication assisted treatment services

Sacred Heart received certification at our recovery homes from the Michigan Association of Recovery Residences Implemented an Opioid Health Home at our St Clair Shores location Outpatient services began at our Serenity Hills Residential location Purchased a building in St Ignace to begin renovations for a medication assisted outpatient location The remainder of the year was spent trying to safely manage the Coronavirus Global Pandemic which included an overhaul of our programs to comply with the federal and state guidance Much focus was spent on shifting in person services to teletherapy Opioid Health Home services were implemented at our Madison Heights Recovery and Wellness Center Implemented a new electronic health record EHR Additional technology was implemented to enhance virtual services provided at all locations Our St Ignace location received extensive renovations This location also went through the required federal and state regulatory inspections for certification as an Opioid Treatment Program Converted our recovery home in Berrien County to a home for women Started Opioid Health Home services at our Flint and Richmond locations Celebrated our 55th anniversary Early in 2022 we received approval from the Centers for Substance Abuse Treatment certifying the St Ignace location as an Opioid Treatment Program In February 2022 the Saint Ignace office began providing comprehensive outpatient services focusing on opioid treatment utilizing all three FDA approved medications Buprenorphine Methadone and Naltrexone This location is the only certified opioid treatment program to ever exist in the Upper Peninsula 15

THE NEXT FIVE YEARS Beyond doubt this is one of the most rewarding positions I have ever worked At my age and driving over two hours a day to get here let s me know that I believe in the mission and the philosophy of Sacred Heart TEAM MEMBER Sacred Heart This is why we are here to serve others The Sacred Heart Way means we work to ensure that people are first in everything we do PHILOSOPHY OF CARE Sacred Heart
