2 St. John’s Medical DialogueMSQC Updates for the Medical StaffI would like to provide you with some important pearls of wisdom from recent meetings among the Medical Sta Quality Committee MSQC. This is helpful information that is applicable to the entire Medical Sta.Pre-eclampsia DangersDid you know pre-eclampsia continues for many weeks after delivery? If you see a recent post-partum woman with hyper-tension, consult an obstetrician right away. Unfortunately, maternal mortality due to pre-eclampsia after delivery is not insignicant. According to the National Institutes of Health, to of direct maternal deaths are associated with preeclampsia and eclampsia.Review Your X-RaysOur Radiology sta is excellent. However, it is good medicine to always review your own lms. The ordering physician likely has a specic question and may identify a nding not appreciated by Radiology. Something I was told as an intern by my Program Director at Cedars: “An abnormal chest x-ray is one you didn’t look at.”As always, remember to place orders. We all get busy and can forget to place orders. Yet, it’s important to continue providing optimum care. Please don’t forget.Documentation Requirements and Rule BreaksThe Medical Executive Committee MEC has updated the Medical Record Completion policy to address the lack of timely and complete documentation of key elements required by Medical Sta Bylaws and regulatory requirements from the Centers of Medicare and Medicaid Services, The Joint Commission and Title California regulations. Key documents include the History and Physical, Operative Record, Anesthesia Pre- and Post-Notes and Discharge Summary. The lack of compliance to any of these for any patient constitutes a “Rule Break” and all practitioners are being notied of these missing or incomplete documents. The MEC wants to provide practitioners with early notication of these upcoming changes in hopes that they will nd a way to comply with these well-known regulations before this policy is enforced in the upcoming months. All practitioners are encouraged to use Dragon, the voice recognition system, or get additional help from Clinical Informatics to support timely notes in the patient health record. The Health Information Management HIM Department team is reviewing each patient admission daily to identify opportunities to capture these key documents with the intent that all patients and all caregivers have the benet of the most current information regarding each patient. Thank you for ensuring appropriate and timely documentation in all patient records.Kevin Major, MD MSQC Committee ChairJoseph LoPresti, MD Chief of StaffIntegrityExcellenceCompassionInclusionCollaboration
