Sophie Schrader (Dietitian) E: sophie.schrader@minda.asn.au Minda ASSIST Therapy Dietetics Matthew’s Nutrition and Hydration Plan High Energy, High Protein GOALS:  Support wellbeing through achieving a comfortable, healthy weight and optimising nutrient & fluid intake. OBJECTIVES:  Meet nutrition and hydration requirements optimally  Prevent weight loss  Support weight maintenance at approximately 47kg. Goal weight range = 46-53kg ☐ Other: STRATEGIES:  Follow Speech Pathology directives for all food / fluid textures. Duty of Care requires that these guidelines be followed by all paid carers and staff. Matthews current diet is MINCED MOIST + SLIGHTLY THICK FLUIDS (April 2021)  Encourage food first, then drinks. Please provide full feed assist and gentle encouragement to try all foods.  High Energy High Protein Diet. Fortify foods with additional energy and protein where possible, for example: Breakfast Lunch Dinner ▪ Cereal: add yoghurt / pureed fruit / mashed banana / finely ground nuts (like flaxseed or LSA mix) ▪ Spaghetti/baked beans: add parmesan cheese / ricotta cheese ▪ Eggs: cook with margarine / extra virgin olive oil. Add cheese. Serve with avocado. ▪ Pancakes: add yoghurt / custard / pureed fruit / mashed banana / honey / maple syrup / finely ground nuts (like flaxseed or LSA mix) ▪ Eggs: mash boiled eggs with mayonnaise. Add avocado. ▪ Soups: add cream / finely ground nuts (like flaxseed or LSA mix) ▪ Mixed dishes: mash with extra virgin olive oil / margarine / sour cream / white sauce. Add cheese / parmesan ▪ Potatoes/starch: mash with margarine / ricotta / milk / white sauces Cold potatoes Add mayonnaise / avocado ▪ Veggies: mash with margarine / ricotta / white sauces / sour cream ▪ Fish/meat: mash with ricotta / white sauces / sour cream  Include high energy/high protein drinks & snacks between meals. ▪ Yoghurt (Gippsland brand is a good choice) ▪ Smoothies with blended fruit, yoghurt and milk ▪ Pureed fruit & custard ▪ Desserts mixed with custard or yoghurt  Encourage fluid intake - Encourage increased cordials and juices Now that Matthew has achieved a higher weight and has good oral intake, he is less reliant on iced coffee to meet nutritional needs.  Provide Fortisip Compact Protein (1x 125ml bottle) at 1000am (Provides extra 1,100kJ & 18g protein/day) ▪ This can be taken by itself, can be mixed with iced coffees or milkshakes Name: Matthew Ogden Date of birth: 14/03/1990 Date of Visit: 20/04/22 Date of Report: 20/04/22
