Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Introduction3Policy Statement from Andy Healey, CEO of VerTek LLCThe purpose of this Policy is to ensure the safety of those individuals who operate motor vehicles for VerTek, LLC.Vehicle accidents are costly to our company, but more importantly, they may result in injury to you or others. It is the driver's responsibility to operate the vehicle in a safe manner and to drive defensively to prevent injuries and property damage.The attitude you take when behind the wheel is the single most important factor in driving safely.VerTek, LLC endorses all applicable state motor vehicle regulations relating to driver responsibility. and expects each driver to ooperate the vehicle in a safe and courteous manner pursuant to the following safety rules.

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Who is Eligible to Operate a Motor Vehicle4Driver Eligibility• VerTek, LLC drivers must have a valid driver's license for the type of vehicle to be operated and keep the license(s) with them at all times while driving.• All CDL drivers must comply with all applicable FMCSA (DOT) regulations, including successful completion on medical, drug, and alcohol evaluations.• VerTek, LLC vehicles are to be driven by authorized employees ONLY, except in emergencies, or in case of repair testing by a mechanic.• Other employees and family members are not authorized to drive the Company vehicle.• VerTek, LLC vehicles are to be driven for Company Business ONLY. Personal use of company vehicles is prohibited. No unauthorized persons are allowed to ride in company vehicles.

Failure to do so may result in disciplinary action, including possible termination.• All accidents while driving for VerTek, LLC, regardless of severity, must be reported to the police and to VerTek, LLC.• Failing to stop after an accident and/or failure to report an accident will result in disciplinary action, including termination of employment.• While driving for VerTek, LLC, the use of intoxicants and other drugs is forbidden and will result in immediate termination of employment.

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Who is Eligible to Operate a Motor Vehicle6Driver Eligibility cont'd• All drivers and passengers operating a vehicle for VerTek, LLC must wear seat belts, even if air bags are available.• Authorized Personal Vehicle Use - In the event that VerTek, LLC employee is involved in an accident while driving his/her own vehicle on company business VerTek, LLC may be liable if you do not have insurance or if the loss exceeds your personal policy limits.• Employees who are authorized to use their personal vehicles for company business are required to carry adequate limits of liability, with a required minimum of $100,000 for property damage and $300,000 for bodily injury.• A copy of the declaration page of your personal automobile insurance policy must be provided to VerTek, LLC annually at your renewal date.

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Mobile Device Use & Distracted-Driving7In order to increase employee safety and eliminate unnecessary risks behind the wheel, VerTek, LLC has enacted a Distracted Driving Policy. We are committed to stopping distracted driving and have created the following rules.This policy applies to all employees when any of the following five conditions apply:• Operating a company vehicle• Operating a personal vehicle on company business• Driving on company property• Using a cell phone supplied by the company.• Using a personal phone for company business

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Mobile Device Use & Distracted-Driving8• VerTek, LLC employees may not drive any vehicle on company business using a laptop computer or any two-way communications device including mobile phones, laptops, tablets, etc. while driving, unless the device is “hands-free”. Otherwise, these devices should only be used if the employee has pulled off the road in a safe area, or by passing the device to a passenger for use. In the case of vehicles occupied by only the driver, in accordance with DOT regulations.• Direct Connect devices may be used to answer calls provided the driver is able to operate the device with a single touch to the button needed to operate the push-to-talk feature and do so from the normal seated position with the safety belt fastened.• Navigation or directional equipment may not be used unless it has oral communication capability, and the screen is placed in a safe location so it can be seen without looking away from the road.• If company employees need to use their devices not hands-free, they must pull over safely to the side of the road or another safe location.

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Automobile Accident ReportingThe purpose of this policy is to ensure the timely reporting, proper handling, and investigation of all automobile accidents involving company owned vehicles or vehicles operated by contractors while on company business for VerTek, LLC.Procedures [Complete procedure is part of every reporting form]1. Driver’s Responsibilities at the Scene of the Accident2. Reporting Automobile Accidents3. Drug Screening (Urine Analysis)4. Management Reporting to the Safety and Risk DepartmentsThe Manager shall forward the required documentation associated with the incident to the Human Resources Department within 24 hours of the occurrence or first notification. The documents required are determined by the Safety and Risk Departments and are subject to change or modification.In the Event of an Accident13

An unfavorable record will result in the loss of vehicle driving privileges or employment. This link will download the MVR Acknowledgement FormThe following criteria and method of evaluation for all prospective and current VerTek, LLC drivers’ MVRs will be used.All company drivers will be required to sign an authorization for VerTek, LLC to periodically review your Motor Vehicle Record to determine continued eligibility to drive for company business.Unacceptable driving records:• One (1) or more type ‘A’ Violations (as defined below) in the past 5 years• Two (2) or more at-fault accidents in the last 3 years.• Three (3) or more ‘B’ violations (as defined below) in the past 3 years.

Module 10 - Driving Safety and Accident Reporting Proprietary & Confidential Copyright VerTek, LLC 2022Records Review Policy16Driving records needing additional monitoring:• Any driver with two moving Type B violations or one (1) accident in the three-year period will to be put on warning and per the discretion of VerTek, LLC Management, MVRs may be ordered more frequently on these drivers.Type ‘A’ Violations:• Driving While Under the Influence of Alcohol or Drugs• Refusing to take a substance test• Driving with an open container (alcohol)• Reckless / Careless driving• Speeding: More than 14 mph over posted limit• Driving while texting• Hit and run• Fleeing or evading police or roadblock• Resisting arrest• Racing/Speed contest• Driving with license suspended or revoked• Vehicular assault• Homicide or manslaughter or using vehicle in connection with a felonyType ‘B’ Violations:• All Moving Violations not listed as type ‘A’ Violations.
