Welcome to Open Enrollment for Lifetech Resources 3 BENEFITS ELIGIBILITY WHO IS ELIGIBLE If you’re a full-me employee at Lifetech Resources, you’re eligible to enroll in all benefits outlined in this guide. Full-me employees are those who work 30 or more hours per week. In addion, the following family members, including a legal spouse, domesc partner, and / or children are eligible for medical, dental and vision coverage:  You, your spouse’s or domesc partner’s biological children, stepchildren, adopted children or foster children up to age 26.  You, your spouse’s, or domesc partner’s children of any age if they are incapable of self-support due to a physical or mental disability. HOW TO ENROLL Are you ready to enroll? The first step is to review your current benefits. You will receive an email from your broker shortly aer the open enrollment meeng which will include a link to the benefits portal and instrucons on how to login. Once you successfully log in, please verify all of your personal informaon, and make any necessary changes. Once all your informaon is up to date, it’s me to make your benefit elecons. The decisions you make during open enrollment can have a significant impact on your life and finances, so it is important to weigh your opons carefully. The portal will list your current plan and rate along with the opons and rates for the new plan year. WHEN TO ENROLL Open enrollment begins on November 14th and runs through December 4th. The benefits you choose during open enrollment will become effecve on January 1st, 2024. WAIVER PROVISION Employees may elect to “Waive” the benefit coverage offered by their employer if they have access to coverage through a spouse / domesc partner or through another plan. Therefore, any employee who declines parcipaon in the benefit plan offered by Lifetech Resources MUST complete a waiver elecng not to enroll in the plan(s) offered by Lifetech Resources. Anyone who is not elecng coverage MUST complete and submit the waiver form online through the group’s online benefits portal by the end of the open enrollment process.

Welcome to Open Enrollment for Lifetech Resources 4 MAKING CHANGES OUTSIDE OF OPEN ENROLLMENT If you decide to change your elecons as the result of one of the events listed below, you must do so within 30 days aer the qualifying event. If you do NOT nofy HR within 30 days aer the event, you cannot change your elecons unl the next annual Open Enrollment. You must provide HR with verificaon of all changes of life events. Examples of QE’s include (according to the IRS):  Loss of other coverage, including change in Medi-Cal eligibility.  Marriage, divorce, or legal separaon.  Birth or adopon of a child.  Change in child’s dependent status.  Death of a spouse, child, or other qualified dependent.  Change in residence.  Change in employment status or a change in coverage under another employer-sponsored plan.

What is an HMO? HMO stands for Health Maintenance Organizaon and an HMO plan covers services performed solely by in-network providers. Tends to be a lower cost system but is more restricve than a PPO plan. What is a PPO? PPO stands for preferred provider organizaon and a PPO plan has a network of preferred providers, but also allows for the use of providers outside the plan’s network. It is more flexible than an HMO but is usually more expensive. The following charts compare each of the health plans that will be renewed on Jan. 1, 2024. From there you must select your plan by following the steps: Click ‘Select a Plan,’ enter the plan year for 2024, the plan type is ‘2024 Small Business HMO Access +,’ then the subplan is ‘Gold Access+ HMO 500/35 OffEx’ then click connue with this plan to pull your list of doctors. If you want to search the PPO network, then you can choose the plan type as ‘2024 Small Business PPO Full (Incl Covered CA)’ then select the subplan you are enrolled (or enrolling) on.

If you take a brand-name medicaon, ask your doctor if you are a good candidate for a generic substute. Also, take advantage of the mail order program for maintenance medicaons. 4. Be sure to ulize the opportunies available to you through the Kaiser Permanente and Blue Shield wellness programs. Virtually all workshops and events are a free benefit to you and, in most cases, your dependents. 5. Lastly, don’t hesitate to ask quesons. We encourage you to be your own advocate. Write down your quesons before your doctor’s appointment, take a family member or friend with you as a second set of ears to hear what the doctor is saying, and be sure to get answers to all of your quesons before the appointment ends. 1. Take full advantage of the resources your medical, pharmacy and dental plans have to offer. Review their websites for detailed informaon. Use their nurse line services and take advantage of free prevenve services and wellness visits. 2. If you need to see a doctor and it is aer hours or you cannot get an appointment right away, you have a few opons: Coverage Benefit Virtual Visits See a board-cerfied doctor in a few minutes with no appointment. Doctors and Nurses are available 24/7 to assess your condion. Urgent Care For urgent, but not severe, problems that would otherwise warrant a trip to the hospital Emergency Room For situaons that involve serious trauma or other life-threatening condions

Welcome to Open Enrollment for Lifetech Resources 9 DENTAL INSURANCE In addion to protecng your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings, and X-rays. Several studies suggest that oral diseases, such as periodons (gum disease), can affect other areas of your body—including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery. We’re happy to say that there are no-cost changes to your dental benefits for Jan. 1, 2024. The following chart outlines the dental benefits we offer. If you enroll in the HMO dental plan you will need to select a primary dental office. you can do this by following the steps below. 1) Click on the link below to search for your primary dental office: hps://www.humana.com/dentalinsurance/find-a-denst 2) Click on "Find a denst" 3) It will take you to a new screen where you will provide your zip code and the lookup method to determine what network we are searching for. Aer entering your zip code, click on the coverage type, being DHMO, then the network is Liberty Dental, then hit select. 4) From there you can choose the category on how you want to search for providers. For example, you can type in general, for general densts. 5) From there select your denst and input the dental ID number and name of the office/denst below your selected plan. Your ability to do all of these acvies, though, depends on your vision and eye health. Vision insurance can help you maintain your vision as well as to detect various health problems. Lifetech Resources’ vision insurance entles you to specific eye care benefits. Our policy covers roune eye exams and other procedures and provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses. If you seek the services of a provider listed in our Preferred Provider directory, your benefits include the following: Product underwritten by Humana. Welcome to Open Enrollment for Lifetech Resources 11 FLEXIBLE SPENDING ACCOUNTS Paying for health care can be stressful. That’s why Lifetech Resources offers a flexible spending account (FSA). WHAT IS AN FSA HEALTHCARE PLAN? WHAT ARE THE BENEFITS? A Flexible Spending Account (FSA, also called a “flexible spending arrangement”) is a special account you put money into that you can use to pay for certain out-of-pocket health care costs. This plan lets you use pre-tax dollars to pay for eligible health care expenses for you, your spouse, and your eligible dependents. You don’t pay taxes on this money. This means you’ll save an amount equal to the taxes you would have paid on the money you spend on these items. With an FSA, you submit a claim to the FSA (through TASC) with proof of the medical expense and a statement that your plan hasn't covered it. Then, you’ll get reimbursed for your costs. You cannot stockpile money in your FSA. If you do not use it, you lose it. You should only contribute the amount of money you expect to pay out of pocket that year up to the limit of $3,200. WHAT IS AN FSA HEALTHCARE PLAN? WHAT ARE THE BENEFITS? Dependent Care FSAs allow you to contribute pre-tax dollars to qualified childcare & dependent care expenses, for children age 12 and younger. The maximum amount you may contribute each year is $5,000 (or $2,500 if married and filing separately).

They're meant to complement, not replace, your exisng coverage. Plus, they’re portable – if you switch jobs or rere, your coverage follows you. It’s about being prepared for the unexpected, so make sure to check out the plan summaries for all the specifics. ACCIDENT PLAN Accident insurance provides financial assistance in the event of an unexpected injury, helping to cover costs that may not be fully paid by your medical insurance. This could include emergency room visits, hospital stays, medical exams, and other expenses associated with an injury. The plan typically pays out a lump sum benefit based on the type of injury and the treatment received, offering addional financial support during a potenally challenging me. HOSPITAL INDEMNITY PLAN Hospital Indemnity Insurance offers financial protecon by providing a daily, weekly, or monthly benefit if you are hospitalized due to an illness or injury. This plan can help cover the extra expenses that may not be covered by your medical insurance, such as hospital stays, outpaent surgery, and other related costs. The payouts are made directly to you, giving you the flexibility to use the funds where they are needed most. SPECIFIED DISEASE PLAN Specified Disease Insurance provides a lump-sum benefit upon the diagnosis of a specified crical illness, such as a heart aack, stroke, or cancer. This financial support can help cover the high costs associated with treang these serious illnesses, as well as any addional expenses that may arise, such as travel for treatment, home modificaons, or lost income. Changing from a PPO to an HMO, or waiving)  Enrollment or terminaon of individual and/or dependent coverage in a plan REQUIREMENTS FOR ALL EMPLOYEES All employees must provide their electronic signature on the enrollment/waiver forms by the deadline below. Whether an employee is making a change or not and enrolling or waiving coverage, all employees must go to the online benefits portal to provide their electronic signature. HOW TO ACCESS THE ONLINE BENEFITS PORTAL  Go to hps://lifetech.ease.com and sign in with the below login credenals or use the QR code below. o Username: First inial and last name (no caps, no spaces) o Password: First two leers of your first name, first two leers of your last name, and last four digits of your SSN (The first leer must be capitalized) o Example: Jane Smith with SSN 441-53-5614.
