But there are ways around this horrible surprise.I recently discovered a way to save a lot of money ($80 on a 90-day prescription) by doing two things: I changed pharmacies, and I started using GoodRx. Here’s how:Go to the GoodRx website (goodrx.com) and put in your ZIP code so that all the local pharmacies show up. Enter the name of the drug you take, as well as the dose and number of pills. See what each pharmacy charges using GoodRx. They will vary widely.Boy, did I get a shock. It took me all of two min-utes to call a new pharmacy and ask them to have the prescription transferred.Then I printed out the GoodRx coupon showing the pharmacy name, the drug and dose, the price and so on. I took that to the pharmacy and voila! We ate cupcakes, white bread and real butter, and drank soda pop with sugar in it – We weren’t overweight because we were always outside playing! No one was able to reach us all day. And we were all OK. We would spend hours building go-carts out of scraps and then ride them down the hill; only to nd out we forgot the brakes. We did not have Playstations, Nintendo’s, X-boxes, no video games at all, no 99 channels on cable, no videotaped movies, no surround sound, no cell phones, no personal computers, no Internet or Internet chat rooms. What we had were friends with whom we met outside and played.We fell out of trees, got cut, broke bones and teeth and there were never any lawsuits from any of these accidents.We were given a BB gun for our tenth birthday, made up games with sticks and tennis balls and although we were told it would happen, we did not put out “very many eyes”.Little League had tryouts and not everyone made the team. Those who didn’t had to learn to deal with disappointment. Imagine that!!!! The idea of a parent bailing us out of jail if we broke the law was unheard of. They actually sided with the law. Our generations have produced some of the best risk-takers, problem solvers and inventors ever! The past 50 years have seen an explosion of innovation and new ideas. We had freedom, failure, successes and responsibilities, and we learned how to deal with them all. If you are one of that generation, congratulations!!! You might want to share this with others who have had the luck to grow up as kids before “outside inuences” regulated our lives “for our own good”. Juvenile idiopathic arthritis (JIA) is the term used to describe arthritis in children. JIA is an autoimmune disorder, which means that the body’s immune system mistakenly attacks some of its own healthy cells and tissues, causing joint pain, swelling, warmth, stiness and loss of motion. JIA may last a limited time, such as a few months or years, but in some cases, it is a lifelong disease that requires treatment into adulthood. With treatment, most children achieve periods of wellness (remission), and sometimes the disease goes away permanently with no further need for medications. It is important to see a doctor early if your child has swollen or sti joints because delaying therapy can lead to joint damage, a lesser response to treatment, and other problems.JIA begins in children and adolescents before the 16th birthday. Most types of the disease are more frequent in girls, but a form of the disease that involves inammation of the places where ligaments and tendons (exible bands of tissue) attach to bones, is more common in boys. Systemic JIA, a rare type of JIA that features fever and rash, aects boys and girls equally. Children of all races and ethnic backgrounds can get the disease. It is very rare for more than one member of a family to have JIA, but children with a family member with chronic arthritis, including JIA, are at a slightly increased risk of developing it. Stand Downs are a good place to start.Stand Downs were started in California in 1988 by two veterans who saw the great need for services to homeless veterans. Over the years, the number of events and the veterans served has grown to include help from government agencies, veteran service organizations, groups and individual volunteers all Step Up to Help Your Fellow Vetsacross the county. The need is so great that many locations see hundreds of veterans lined up outside the door before they even open on the rst day.The list of help provided at each Stand Down can depend on the location and the length of the event (usually one to three days), but typically includes health screenings, clothing, showers and haircuts, food, legal advice, dental and eye-care services, mental health counsel-ing, substance treatment referral, employment help, benets counseling (both Department of Veterans Aairs and others, such as Social Security), housing referral and more. The schedule of Stand Downs in 2022, whether you need services or want to help, can be seen at www.va.gov/homeless/events.asp. Keep an eye on that page because new events can be added at any time. Another page to watch is the National Coalition for Homeless Veterans (nchv.org/current-stand-downs).Or you can call your local VA medical facility and ask what’s scheduled for your area. Best bet is to call them anyway and ask if the event is still planned due to COVID playing havoc with scheduling.Ask about transportation, if you need it or can oer it. That’s kind of ironic, because the truth is that uncharred fresh fruit tamps down res related to inammation in your body. Inammation can be a good response if short-lived; it helps your immune system to heal a wound or ght an infection. When it becomes chronic because of the presence of visceral belly fat and obesity or chronic sedentary behavior, it increases your risk for cardiovascular disease, cancer, arthritis, diabetes, chronic kidney disease, nonalcoholic fatty liver disease and autoimmune and neurodegenerative disorders. Has the standard number increased? -- D.M.I.ANSWER: The “pressure” in “blood pressure” is given in millimeters of mercury. It is a measurement of the pressure inside large blood vessels, rst at its peak during the left ventricle squeez-ing (the systolic, or top number) and then at its lowest, right before the ventricle starts squeezing again (diastolic, or bottom number). Both of these numbers are important, and they provide important information about the physiology of the heart and blood vessels, as well as provide prognostic information about the risk of heart attack and stroke.In general, for healthy people, the higher the numbers, the greater the risk. The optimum blood pressure for health is about 110/70, but there are some people with lower values who are also very healthy and who have very low risk for vascular disease.Risk for heart disease starts to go up more signicantly at about 140/90, but above a systolic number of 160, the risk goes up even more dramatically. The trend in blood pressure control over recent years has been toward making the blood pressure closer to the optimum, as long as treatment is well-tolerated. Having a slightly lower risk of heart attack may not be worth it to a person who has signicant symptoms from blood pressure medicines.Fortunately, blood pressure is much easier to control than it used to be with better medications and with non-drug treatments, such as stress reduction, healthy diet, regular moderate exercise and, for some people, salt restriction.* * *DEAR DR. ROACH: I am an 89-year-old man who has been taking tamsulosin (Flomax), 0.4 mg, for many years. I take it after supper in the evening. I get up at night three or four times to urinate. Would it be better to take it after lunch? I would be happy if I could reduce the number of times I get up to maybe only once or twice. -- P.J.S.ANSWER: Tamsulosin is in the class of alpha blockers, and they relax smooth muscle, a special type of muscle found in the prostate and in blood vessels (among other places). Relaxing the smooth muscle in the prostate makes the urethra, the tube that carries the urine from the bladder and through the prostate, larger. As a result, men can empty their bladders more easily. However, even though tamsulosin is better at relaxing smooth muscle in the prostate than in blood vessels, some men will get lightheaded upon standing when using it. This occurs especially on the rst dose, and over time most men no longer have trouble. It is usually dosed at night-time so that the lightheadedness on stand-ing is minimized. You can take it at any time of the day (a half-hour after eating is recommended, at the same meal of the day), but I doubt it will work much better.Many experts will use 0.8 mg in men who haven’t had an ideal response and who do not have lightheadedness. I’d ask your doctor whether an increased dose might be better. Before considering another drug, be sure you avoid too many liquids at night, especially alcohol, and try voiding your bladder twice before bed to make sure it is as empty as possible. * * *Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible.
