Dr Neils Cover Letter Dear Professor X We are pleased to submit a manuscript by Rentschler et al entitled Notch Signaling Regulates Murine Atrioventricular Conduction and the Formation of Accessory Pathways for consideration by the Journal of Clinical Investigation The authors declare no conflict of interests COI We present the generation of a striking new model of human cardiac ventricular pre excitation commonly referred to as Wolff Parkinson White syndrome WPW and present evidence to support a developmental model to explain the etiology of this disease We believe that this paper will be paradigm shifting 40

Although this relatively common disorder has been appreciated nearly 100 years little is known about etiology One genetic cause has been described for a relatively rare variant of WPW associated a glycogen storage disorder and mouse models of this variant involving PRKAG2 seem to develop pre excitation because of a breakdown of the fibrotic layer of insulation between atria and ventricles This does not mimic the pathology of more common forms of WAW in humans Our work provides a better animal model and a different developmental explanation invoking different genetic pathways While various commentaries over the years have suggested that persistence of embryonic structures could explain features of WPW we know of no prior experimental evidence to support this hypothesis Appropriate referees include Person 1 Institution email Person 2 Institution email Person 3 Institution email We request to exclude person 4 Institution email person 5 Institution email as referees due to COI With regards Jonathan Epstein MD 41
