Know Before You GoMedical – United Healthcare Choice Plus network plan administered by Assured Benefits Administrators• Medical coverage will utilize the UHC Choice Plus network and benefits will remain the same.• To create your user account, visit https://portal.abadmin.com/Logon.• Telehealth visits are available with Lyric Health providing 24/7 access to board-certified doctors at no cost.Pharmacy Benefits - VerusRx• The pharmacy program with VerusRx will remain the same. Dental – Sun Life• The dental plan uses one of the nation’s largest PPO networks – Sun Life Dental Network.• The program offers the ability to rollover unused annual maximum dollars to the next year.• Special Note for 10/1/2024-12/31/2024: The 2024 calendar year deductible will reset on 10/1/2024. Anew deductible will apply with Sun Life for you and your covered dependents. However, the 2024calendar year maximum benefit will also reset on 10/1/2024. This means you and your covereddependents each have the full $2,000 benefit from 10/1/2024-12/31/2024. The deductible and calendaryear benefit resets each year on 1/1. The lifetime orthodontics maximum does not reset. Vision – VSP through Sun Life• A traditional vision program utilizing the VSP network.• Benefits include exam services and materials for a low-cost copay; safety frames are included. Group Life/AD&D Insurance and EAP – Sun Life• Employees receive $25,000; Spouses receive $10,000; Dependent Children receive $5,000 – all at no cost.• You may purchase additional amounts of Voluntary Life/AD&D based on your needs.• An Employee Assistance Program provides no cost, confidential resources and someone to talk to 24/7.Voluntary Benefits - Colonial • A variety of voluntary benefits are offered through Colonial Life allowing you to choose as desired.Freshbenies - A Health Advocate is also available to help!The team of Health Pros at Alight through freshbenies can help with locating network providers, reviewing bills and EOBs to ensure accuracy, and can even negotiate with providers, when needed. Who is Eligible?Full-time employees who work 30 hours or more per week are eligible to enroll in the benefits outlined in this Guide. In addition, the following family members are eligible to participate:• Your legal spouse• Your eligible child(ren) up to age 26 for medical, dental or visioncoverages (natural children, stepchildren, legally-adoptedchildren, and child(ren) for whom you are the court-appointedguardian); other coverages may have different age limits.• Physically or mentally disabled children of any age who areincapable of self-support. Proof of disability may be requested.New Hires You will become eligible for medical benefits on the 91st day of employment. For all other plans, you will be eligible on the 1st day of the month following 6 months of employment. The benefits elected will be effective through September 30, 2025.Current EmployeesOpen Enrollment occurs each year; this is the time for you to make any changes. Benefits elected during Open Enrollment will be effective October 1, 2024 – September 30, 2025.Qualified Life Events• Marriage, Divorce, Legal Separation• Birth or adoption of a child• Change in child’s dependent status• Death of a spouse, child or other qualified dependent• Change in employment status or a change in coverage underanother employer-sponsored plan• Dependent loses eligibility due to ageHow to Make ChangesYour elections are intended to remain in place until the next open enrollment. If you experience a qualified life event, you must contact Human Resources within 30 days of the event.Enrollment ProcessJefferson County Drainage District No. 7 will be utilizing enrollment forms to enroll in benefits.Before beginning enrollment, please have all pertinent information available for both yourself and any dependent(s) you will enroll. Information such as dates of birth and Social Security numbers will be required, along with beneficiary designations. During the enrollment process, you will have the opportunity to add the dependents you want to be covered. You may purchase Voluntary Life and AD&D for yourself, your spouse and/or your dependent child(ren). You must elect coverage for yourself in order to elect for any dependents. Voluntary coverage is paid for via payroll deductions. The cost is based on your age and the amount you elect.
