Go to KnowTheCosts.com and submit the contact form. To arrange imaging, we need the physician's order along with your City and State. Once we locate a facility for your exam, you will be notified by text or email.Call your physician's office and give them our contact information. They can fax the order or use the easy upload feature on our website under request appointment.When your doctor orders a radiology exam, present your KTC card and ask them to upload your order via our website. Details are listed on the back of your KTC card.) If your doctor's office has additional questions, have them reach out to us.Going to a KTC preferred facility and using your KTC benefit is totally voluntary. It's always your choice. If you choose a non-KTC facility, you will have to file with your health plan. Remember, you may have to pay co-pays, deductibles and co-insurance.

At your appointment, tell the technologist that your doctor is requesting a CD of your exam and you would like to take it with you.Yes. A member card will be sent via mail and can also be downloaded from our website. Use this card instead of your health plan card for savings on radiology imaging.The imaging facility will send the results to your physician in the same manner as if you were using traditional health insurance.
