SWHP has expanded their service area to include the following counties in the DFW metroplex: Denton, Dallas, Collin, Tarrant and Rockwall Counties. You are not required to select a Primary Care Physician, and you may see any network physician without a referral. 5/23/2023 Payroll and Benefits 17Disability InsuranceShort -Term Disability (STD) and Long-Term Disability (LTD) ➢ Disability coverage provides a source of income by replacing 40% to 60% of your earnings if you are unable to work due to an injury or illness. ➢ As a New Hire, EOI (Evidence of Insurability) is not required to enroll in Short Term and/or Long-Term Disability.Please Note: The monthly rates for STD and LTD are based on the employee’s salary and age. Supplemental Term Life Insurance will pay a benefit to your designated beneficiary upon death. More about your options: • You may choose additional coverage for yourself, up to five times your annual base salary. • You may choose Term Life Insurance in $10,000 increments, up to $50,000 for your spouse. (Dependent life may not exceed 50% of employee coverage amount. • You may also elect $5,000 or $10,000 for your dependent child(ren). AD&D Insurance provides additional benefits for an accidental death, and for an accidental dismemberment, as defined in the schedule of benefits.
