Our team, at Hope Human Services, brings many years of experience providing residential services to individuals with Developmental and Intellectual Disabilities who are experiencing instability. We have developed a variety of successful specialized programs with clinical supports. We have a trained an experienced Diversion Team that include a Clinical Supports Director, Diversion Director, Program Manager and Direct Support Professionals. We use a clinical model and curriculum called Green Zone that focuses on Skill Acquisition for Individuals and extensive training for staff on Groundworks for Support, Coaching, and Self Care. All plans and supports focus on developing skills that contribute to long-term stabilization. Diversion Breakdown This program was designed as a short-term stabilization service that provides individualized support as necessary in a safe welcoming environment. The focus of treatment is stabilization and transition. The current location of our Diversion Beds are in 2 duplexes in Puyallup so we can offer services to 4 individuals at a given moment, depending on need. All units have private bedrooms, bathrooms, kitchens, living rooms, as well as a sensory room. These are some of the services that we offer: • 24-hour supervision from Diversion staff trained in the Green Zone clinical support model • Providing basic needs, including 3 healthy meals a day plus snacks, and personal hygiene supplies • Access to a vehicle on-site for appointments and community outings along with funds for doing community outings • Clinical and Behavioral Supports to assist in stabilization and transition to a long-term placement • Wrap around clinical meetings to coordinate services, resources, and communication among the individual’s support team. Program Overview Once an individual is referred to the program, an assessment process begins that will include document review and in-person observations. The Diversion Program Manager and Clinical Supports Director will coordinate with DDA to determine if Diversion program services are appropriate for the referral. Service targets and discharge criteria are established, and the admission process is completed once the individual agrees to receive services from the Diversion program. Service plans, Functional Assessments, and Positive Behavior Support Plans are developed based upon data collected. Weekly wrap around clinical meetings are facilitated to coordinate all care, services, therapies, and resources need for the individual’s path towards stabilization. The primary focus of the program is specialized service, stabilization, and transition. Diversion staff will work to facilitate the referral and placement process including transition supports.
