Out-of-network benetsOut-of-network providers haven’t agreed to discounted rates, and their fees may vary signicantly. Your policy’s co-insurance is applied to the lesser of the provider’s charge or a percentage of the customary charge in your area (90th percentile), and may not cover the total costs of dental care.
