Some examples include getting regular physical examinations, mammograms and immunizations. Through the plan offered by Curi, all covered individuals and family members are eligible to receive routine wellness services like these, at no cost; all copays, coinsurance, and deductibles are waived.Which Preventive Care Services Are Covered?The US Preventive Services Task Force maintains a regular list of recommended services that all Affordable Care Act (i.e., Health Care Reform) compliant insurance plans should cover at 100% for in-network providers. Telehealth provides acute and behavioral care 24 hours a day via phone or video by board-certified doctors and behavioral health specialist. Telehealth is a good option for minor health problems when you can’t see your regular doctor. It is also a convenient choice when you want to speak to a counselor or therapist. Telehealth through Teledoc has a $10 copay.Some providers will also offer telehealth appointments. All services described within are subject to the definitions, limitations and exclusions set forth in each insurance carrier's or provider's contract.Employee Benefits Guide2023-2024CuriThis Is How An FSA Works:• You set aside money for your FSA from your paycheck before taxes are taken out.• You then use your pre-tax FSA funds throughout the plan year to pay for eligible health care or dependent care expenses.• You save money on expenses you're already paying for.Health Care FSAOur plan through Flores & Associates has a 2023-24 plan year maximum of$3,050. The full amount you elect is available at the start of the plan year on June 1. Our plan offers a carry over up to $610 of unused funds to the following year. If you don’t use it, you lose it. Any funds over the carry over limit will be forfeited. Eligible Expenses• Medical expenses: copays, coinsurance and deductibles• Dental expenses: exams, cleanings, X-rays and braces• Vision expenses: exams, contact lenses, eyeglasses and laser eye surgery• Professional services: physical therapy, chiropractic and acupuncture• Prescription drugs and insulin• Over-the-counter health care items such as bandages, pregnancy tests and blood pressure monitorsDependent Care FSAThe maximum you can contribute to a dependent care FSA is $5,000 annually. If you are married and filling separately then the maximum is $2,500. These programs are designed to fill the gaps in coverage such as your deductibles and co-insurance under your major medical. Curi will contribute up to $50 per month to use towards the purchase of the following benefits: hospital indemnity, critical illness or accident insurance.ADDITIONAL BENEFITSHospital Indemnity Provides a benefit for hospital admission and confinement for an illness or injury. The benefit is paid directly to you and can be used however you need. Coverage can be purchased for employee and dependents. This plan includes a $50 annual wellness incentive per covered member. Critical Illness*Pays a lump sum benefit should a covered family member experience a heart attack, stroke or other covered serious illnesses. You can choose between a $10,000 and $20,000 benefit, with the option to cover your spouse and children. Evidence of Insurability is required if enrolling after your first eligible. This plan includes a $50 annual wellness incentive per covered member. *Premium varies by age and benefit amount and dependents covered; see rates in the plan summary or on MyCuri.AccidentPays a lump sum benefit if you or a covered family member experience an accident. Free and discounted legal care for divorce, traffic ticket defense, document review, etc. This benefit includes free tax return preparation and identity theft solutions. The cost of this plan is $10.05 bi-weekly.401(k) Retirement PlanCuri’s 401(k) retirement plan is designed to help employees save for a financially secure future. You are eligible to participate in the plan on the first day of the month following your date of hire. You may contribute up to 50% of your salary, not to exceed the IRS limits. The contribution is made pre-tax. Curi will match 100% of the first 3% of the contribution. You may choose to direct your contributions to a traditional 401(k) or a Roth 401(k) or both. You can make or change your elections online at www.netbenefits.fidelity.com.Employee Well-Being Curi is committed to being your partner in your overall well-being. That is why we offer many programs geared toward improving your physical, mental, and financial well-being. Computer Purchase/Loan ProgramCuri offers a one-time reimbursement of up to $250.00 along with an interest-free salary advance of up to $2,250.00 with a maximum 24-month payback schedule for the purchase or upgrade of personal computer equipment and accessories. An employee may receive only one computer purchase program/loan in a 24-month period. You must be employed for at least 90 days to be eligible for this program. Please see the Employee Handbook for more information.Employee Referral Bonus ProgramFull and part time employees are eligible for a one-time referral bonus should you refer qualified candidates who are subsequently hired to fulfill staffing requirements. The parental leave runs concurrent with the Short-Term Disability leave administered by The Hartford, if applicable, and provides 11 weeks of paid time off (not to exceed 100% of pay from all combined sources) following the birth of an employee's child, or the qualifying placement of a child in connection with adoption or foster care. Curi maintains all benefits for the employee while on Parental Leave. Paid leave under the parental leave benefit runs concurrently, not in addition to, Family and Medical Leave Act (FMLA) leave. Parental Leave benefits can be accessed once per rolling 12-month period. For additional information about Parental Leave, contact Human Resources. Employees become eligible for parental leave benefits once they have completed 12-months of cumulative service with the company.Family Medical Leave Act (FMLA) FMLA is administered by The Hartford. FMLA entitles you to take unpaid, job-protected leave for specified family and medical reasons with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave. Those who qualify are entitled to:• Twelve work weeks of leave in a 12-month period for:• the birth of a child and to care for the newborn child within one year of birth;• the placement with the employee of a child for adoption or foster care and to care for the newly placed child within one year of placement;• to care for the employee’s spouse, child, or parent who has a serious health condition;• a serious health condition that makes the employee unable to perform the essential functions of his or her job;• any qualifying exigency or urgent event arising out of the fact that the employee’s spouse, son, daughter, or parent is a covered military member on “covered active duty;” or• Twenty-six work weeks of leave during a single 12-month period to care for a covered service member with a serious injury or illness if the eligible employee is the service member’s spouse, son, daughter, parent, or next of kin (military caregiver leave)ADDITIONAL BENEFITS
