Community access (catchingthe bus, attendingappointments, visiting theplayground & much more)Attending community events(music concerts, markets,sporting events)Participation in recreationalactivities (sports competitions,art classes)Complete practical householdtasks (tidy the kitchen, feed thepets, take out the bins)Filling out that never endingpile of paper-work!Administer medications andother personal care (showering,getting dressed)Getting ready for school, workect.Grocery shoppingVisiting friends and familyAnd much, much more!WHAT IS ACOMMUNITYMENTORThe Community Mentor program is aservice that offers participants one-to-one support in a variety of areas withintheir daily life.A Community Mentor (otherwise knownas a Support Worker) is a professionallytrained person who provides social,emotional and practical support tosomeone living with a disability. They work with you as an understandingand compassionate mentor who offersyou guidance in navigating both socialand community settings- or simply in thecomfort of your own homeWHAT CAN THEY HELPME WITH?FREQUENTLY ASKEDQUESTIONSIf Core supports are funded in your NDISPlan, its as simple as giving us a call oremail! If you aren't funded, you can stillgive us a call and we can work out how tosupport you to get it!HOW DO I GET A COMMUNITYMENTOR?WHAT HAPPENS IF MYCOMMUNITY MENTOR LEAVES?DO I STILL HAVE CHOICE &CONTROL?Unfortunately sometimes people changejobs, whilst we wish we could keep our staffforever, we always support them to continueto grow. If your Community Mentor leaves,we have a process in place to ensure you arewell supported by a friendly and familiar faceyou met at the beginning! We will alwayscommunicate and work quickly to ensure youare never without support.Always! This is your NDIS journey, our roleis to support you to work towards yourgoals!A community mentor is there to help youbuild your capacity and confidence so thatyou can live as indepently as possible.
