When undertaking any written assessment tasks, you are to ensure that you focus on the set tasks and that all written work is legible. Prior to commencing the assessments, the assessor will explain each assessment task and the terms and conditions relating to the submission of the assessment task. It is important that you understand and adhere to the terms and conditions and address each assessment task fully. If any assessment task is not fully addressed, then the assessment task will be returned to you for re-submission. Assessment Components The assessor will examine all of the required assessment evidence submitted for this unit of competency and decide whether: a. Satisfactory performance has been demonstrated for this assessment, or b. Satisfactory performance has not yet been demonstrated. If you are deemed as unsatisfactory, you will need to discuss with the assessor any further evidence required. You may only need to be reassessed on the area that was not assessed as satisfactory. Please note that you must satisfactorily complete all assessment tasks to be deemed competent for this unit. You cannot be signed off as competent for this unit until any/all workplace and/or practical experience requirements have been met. CHCCCS040 Support Independence and Wellbeing Student Assessment Kit © 2023 Coast Wide Training Solutions | CHCCCS040 Student Assessment Kit V1.0-23 56 Assessment 3 – Practical Skills Demonstration Instructions Due to the nature of vocational learning, the assessment of the practical skills of this unit of competency cannot be entirely conducted by the assessor. To ensure the integrity of the assessment and that you are authentically acquiring the skills required by the unit, the assessor requires a workplace supervisor to observe your performance and verify the skills acquired. Involving a workplace supervisor as a third party in the collection of evidence allows assessors to gather authentic and valid evidence of your real work performance under real working conditions. The purpose of the Workplace Verification Report is for the assessor to verify and record your progress towards achievement of competencies. It contains a record of all the competencies required for this assessment and must be acknowledged by the third-party, yourself and the assessor. Details of competencies in which you must perform, and the assessor must acknowledge are listed on the following pages. The Workplace Verification Report… • Identifies tasks that you will need to undertake. • Details the instructions that the supervisor, will need to follow. • Allows the supervisor to record their observations of you and additional comments. • Allows the assessor to validate supervisor feedback and add their own comments. • Is an important record of your achievements and must be completed in full and returned on completion of the assessment. • Provides additional evidence to support the competencies required for this unit of competency to be achieved. The supervisor must complete all the sections in the report after observing you throughout the task requirements. The supervisor will complete each section and your opinion of if you demonstrated the skills described by adding comments and feedback. Responsibilities You are responsible for: • Providing the Verification Report to the supervisor. • Ensuring the Verification Report is completed by the supervisor before submitting it to the assessor. The workplace supervisor is responsible for: • Facilitating the practical activities. • Observing the performance of the student • Reviewing and monitoring your performance by indicating observations and providing feedback to the assessor. • Discussing observations and feedback with the assessor.
