III.MedicalA. Medical History: It is important that asyou fill out the online registration foryour camper you provide a completeand up to date medical history. TheNDA and its Medical Staff will thencreate a medical plan that addressesevery camper’s needs individually. Ourgoal is to provide every camper with asafe experience; medically, physicallyand emotionallyTo achieve that goal:1. Each camper must have been examined by a medical practitioner within the SIXmonths prior to attending camp.2. If the camper uses any medication other than insulin, the parent/guardian MUSTprovide enough of that medication for the duration of camp. The medication must bein the original container, labeled with the camper’s name and the instructions fordispensing. The Medical Staff will give the medication as stated on the container andlegally cannot adjust the dosage amount. Statements from parents/guardians such as“this is what the camper takes now” will not be taken into consideration. If there havebeen ANY changes in the medication please make sure the dose and times areCORRECT on the medication bottle/container. If you have any questions or concernsdo not hesitate to contact the Camps Manager, Emily Hixson. (Emily@diabetesnv.org)3. During the camp check-in please bring every medication you want given during camp.During check-in the Medical Staff will review the medication, and aer approval, willthen document those medications in our Electronic Medical Record for each camper. At this time, we would greatly appreciate you leaving the medication with the NDAMedical Staff. If you need additional labeled containers for camp all you need to do isask the pharmacy to give you additional EMPTY labeled bottles/containers. It is StateLaw that the pharmacy must give you extra containers, at no cost, wheneverrequested. Thank you, in advance, for helping the Medical Staff properly administerany necessary prescribed medication. The NDA and its Medical Staff may choose not toadminister medications deemed unsafe to use in a camp setting

III.Medical Cont.Cont.4. The NDA camps have a stock of over-the-counter medications, overseen by theMedical Staff, for use during camps. This includes oral Tylenol, ibuprofen, allergymedication, Zofran, cough drops, sore throat lozenges, topical anti-itch medication, and topical antibiotic ointments. If your camper needs a specific over-the-countermedication, please bring it to the camp check-in labeled with the camper’s fullname. Once it is reviewed and approved by the Medical Staff it will be administeredusing the package directions either by age or weight. Any over the counter medications/vitamins sent with the campers must remain in the original container or else they will not be distributed to the camper throughout the duration of camp5. Continuous Glucose Monitors (CGM’s) are allowed and encouraged for use at camp.CGM use is covered in the same policy as personal cell phones used as medicaldevices at camp. (NDA/CDA Cell Phone Policy) Any active share data on a CGM devicewill be turned off once the camper arrives at camp. If your camper is using an FDAapproved continuous glucose monitor (CGM) they will only be required to do bloodglucose testing at bedtime and as needed when their symptoms do not match theCGM reading. Campers without FDA approved CGM’s will have blood glucose testingat least four times a day unless the camper’s specific insulin regimen indicates and/ormedical staff decides otherwise.6. If your camper is on an insulin pump and/or a CGM be sure you pack enough suppliesto last at least a week with additional supplies for accidental incidents.

III.Medical Cont.Cont.7. All Insulin, syringes, insulin pens, blood glucose monitors/strips and ketone strips areprovided by the NDA at camp. If your camper has a unique non-traditional insulin orother diabetic supply requirement please let the Camps Manager, Emily Hixson, knowat least two weeks before camp begins.A. INSULINS PROVIDED : Humalog, Novolog, Fiasp, Apidra, Lantus, Levemir (ifavailable), Basaglar, Toujeo, Tresiba8. Due to the rigorous nature of the camp program, the camp medical staff, supervisedby the Camp Physician, may alter the camper’s insulin dose, carb correction ratios, basal rates and dietary plan.9. At the conclusion of camp during check-out you will be asked if you would like anemailed copy of your camper’s blood glucose readings, insulin given and othermedical charting. You can also send an email request to the Camps Manager, EmilyHixson, if you would like a copy.B. Dietary Information: The food service during camp is supervised by a RegisteredDietician (RD). There will be three meals a day with an aernoon and evening snack.Meals and snacks are set according to daily camp activity. The camp menu is childoriented and is designed to meet the needs of young people with diabetes. DO NOTSEND ANY FOOD with your child to camp. When food is sent from home it is difficultfor the camp medical team to maintain acceptable blood glucose levels. Food in thecampers’ cabins can also attract unwanted animals and insects, which lessens thecamp experience. If your child has any special dietary needs, please make sure theyhave been noted in the camp registration form. We need a minimum of two weeksnotice to prepare meal plans for any special dietary needs
