This enrollment guide is designed to help you understand your benet options so you can make the choices that meet your needs. Please review your options, their costs, and make sure to ask questions to help you make your benet choices. The benets you elect will be e󰮑ective through August 31, 2025. Current Employees: Open Enrollment will occur each year during the month of August. Benets elected during open enrollment will be e󰮑ective from September 1, 2024 through August 31, 2025. Per IRS guidelines you are not eligible to make changes to the benets you elect until Open Enrollment unless you have a qualifying life event. Health Plans | Charter Frequently asked questions (FAQs): UnitedHealthcare Charter Plan Q A How can I verify that my primary care physician (PCP) has submitted a referral to UnitedHealthcare so I can receive care from a network specialist? Your PCP must submit referrals electronically to UnitedHealthcare. This enables the referral to be effective immediately and allows it to be reviewed faster by your referred specialist. Before seeing a network doctor or specialist, confirm that your referral has been entered by visiting myuhc.com® or calling the member phone number on your health plan ID card. How long is a referral from my PCP to a network specialist valid? A referral is good for 6 months after the date of submission. The maximum number of visits per referral is 6. The referral will remain valid until the visits are used up or the 6-month time period has passed (whichever comes first).If I travel out of state for a period of time, can I choose a PCP in a different state? No, you will not be able to change your PCP to one outside your state of residence. For example, if you live in Indiana and travel elsewhere for a period of time, you cannot choose a new PCP in the state you’ve traveled. How far from my address can my PCP be located and still be valid for me to select? You should select a PCP within the state of the permanent address submitted to UnitedHealthcare with your enrollment. If you need to update that information or have questions, please call the member phone number on your ID card. Do I need a referral to see a network chiropractor? Yes, referrals from your PCP are required to see a network chiropractic specialist. If I use a UnitedHealthcare 24/7 Virtual Visit option, will my visit details be transmitted to my PCP for their files? No; however, depending on the 24/7 Virtual Visit vendor, they may provide a visit summary for you to share with your PCP. Can I see a virtual care provider without a referral? Are these virtual care providers allowed to make referrals for me if I need to see a network specialist? If the telehealth service is delivered by a UnitedHealthcare “designated” national vendor, a referral is not needed (please check the provider directory on myuhc.com for a current list of national vendors). If the telehealth service is delivered by an individual physician (i.e., not a UnitedHealthcare national telemedicine vendor), then a referral is required. continued 3.2

Q A What happens if I have a procedure done at an out-of-network facility but the procedure is done by a network provider? It is your responsibility to check before any scheduled procedure that the hospital or facility is in your medical plan’s network. If your medical plan does not have out-of-network coverage benefits, non-network hospital/facility claims will not be covered for services, even if the surgeon/admitting provider is in the Charter network and had a referral. Non-network hospital/facility services will only be covered for true medical emergency situations or if approved in advance by UnitedHealthcare via prior authorization. Can my OB/GYN or network specialist refer me to other network providers for treatment? Can they order tests or send me to a freestanding lab without a referral from my PCP? No, your OB/GYN or network specialist cannot submit referrals to UnitedHealthcare on your behalf to see other network providers. However, they can contact your PCP with recommendations for additional care that may be needed outside of their respective practices. Only your PCP responsible for directing your care can submit referrals to UnitedHealthcare for you to receive care from other network providers. Note: Referrals are not required for ordering tests from a freestanding radiology or lab facility. Do I need a referral to a network specialist who is in the same group practice/location as my PCP? 17Qualifying Life Events and Special Enrollments If you have a qualifying life event (e.g., marriage, divorce, birth or adoption), you have 31 days to notify Northwest Interiors, Inc. of the event, change your benet elections and have your premiums adjusted accordingly. The change in benet elections must be due to, and consistent with, the qualifying life event. If the event is marriage, birth or adoption, you also may be able to enroll yourself as well as your dependent(s). In addition, if you decline enrollment for yourself and/or your dependents (including your spouse) because of other health insurance coverage or group health plan coverage, you may have a special enrollment opportunity that would allow you to enroll yourself and/or dependents if you or your dependents lose eligibility for that other coverage or if the employer stops contributing toward your or your dependent’s coverage. If you fail to notify Northwest Interiors, Inc. within 31 days of the qualifying life event or loss of coverage, your coverage and premiums will remain the same until the next plan year. To ensure you have the right coverage and are paying the appropriate premiums for your needs, be sure to notify Northwest Interiors, Inc. of any qualifying life event within 31 days. Please note that timing for notifying Northwest Interiors, Inc. is extended to 60 days if your child loses coverage under Medicaid or the Children’s Health Insurance Program (CHIP), or becomes eligible for Medicaid or CHIP coverage. Women’s Health and Cancer Rights Act of 1998 Notice The Women’s Health Act requires the publication of the following notice annually: The plan provides mastectomy coverage and also provides for reconstructive surgery in a manner determined in a consultation with the attending physician and the patient. Coverage includes reconstruction of the breast on which the mastectomy was performed, surgery and reconstruction of the other breast to produce a symmetrical appearance, and prostheses and treatment of physical complications at all stages of the mastectomy, including lymphedemas. This notice is made solely to satisfy the Act’s requirements. The Health Plan has always covered such procedures and in no way does this reect a change in plan provisions. It’s your right to request a paper copy .of detailed plan summaries. As a participant in the plan, you are entitled to certain rights and protections under ERISA. ERISA provides that all plan participants shall be permitted to receive information such as Plan benets, obtain hard copies of all Plan documents and other plan information. Employees will receive an Open Enrollment email from Employee Navigator with a hyperlink to the portal. Step7:UpdatePersonal&DependentInformaƟonYou will be prompted to verify your personal informaƟon, add your address and dependents. Please keep in mind you will not be able to enroll Dependents in coverage if they are not entered into the Dependent screen. Step8:EnrollinyourBeneĮts!You will be prompted to elect or waive each of the BeneĮts oīered. On the right hand side of the screen you will be able to track your progress through the enrollment process. Select the “View Steps” arrow to see a drop down of all steps remaining before the enrollment is completed. Step9:CompleteyourEnrollmentOnce you complete the enrollment process you will see an Enrollment Summary with all of your elecƟons.
