www.afpm.org.my | 5 ACADEMY OF FAMILY PHYSICIANS OF MALAYSIA The President, Dr Norsiah Ali called the house to order at 10.00 am sharp with a quorum of 55 members present for the AGM. The President then welcomed all to the 51st Annual General Meeting of the Academy of Family Physicians of Malaysia and thanked all for giving her the honour in heading this organisation. During her tenure for the past one (1) year, she informed that she went through an interesting journey which was memorable for her. It has polished and sharpened her skill to lead an organization. There were many things that she learned and ventured together with all the members. Her initial task in the Academy of Family Physicians of Malaysia (AFPM) was to represent AFPM in the Medical Safety Council. She was also representing AFPM in the MSQH committee where they were reviewing the Medical Clinics Standard for 2nd Edition and was appointed as the Chairperson. They had completed reviewing the first guideline and had tested it at the Annual Scientific Conference in the past two days where the MSQH took part in the exhibiting the Medical Clinics Standard 2nd Edition guideline to check whether it was user-friendly. Thirdly, she had spearheaded the strategic planning workshop where discussion on how to make AFPM important, relevant and continuously be among important primary care providers in term of training for both the public and private sector in this country. In the workshop, discussion and planning on Key Performance Indicators (KPI) for all the units under the AFPM was done to fine tune the job scopes of the secretariat and each unit in AFPM. Planning of proper asset registers was also implemented. In the year end of her term as the President, she managed to complete both the KPI and the Asset register for AFPM. Apart from that, AFPM faced quite huge challenges in terms of AFPM conducting training as the parallel pathway for Family Medicine Specialists in Malaysia. Madam President informed that there were several meetings with Ministry of Health and the Minister of Health himself, discussing about the training with the Royal Australian College of General Practitioners. Meetings with Malaysian Medical Council and Director General of Health were also attended. Explanation on the AFPM parallel pathways especially on the International Conjoint Fellowship of the Royal Australian College of General Practitioners (icFRACGP) was given. It was a name change on the award only as it was the requirement in the Australian Medical Council since 2018. There are no changes in the training and curriculum that AFPM conducts. Pertaining to this, the EXCO also met the AFPM Royal Patron, DYMM Tuanku Muhriz Ibni Almarhum Tuanku Munawir, Yang di-Pertuan Besar Negeri Sembilan to explain on the situation and have written a joint statement together with Malaysian Family Medicine Specialists’ Association (FMSA) to the Minister of Health on how the change can be made under the Medical Act no. 50 and revised Medical Act 1443 of 2012 which was implemented from 1st July 2017. They had also written in the media on this matter so that the general public will know how we feel about this matter and what we wish it to be. Fortunately, last week, 23rd April 2024, Madam President informed that she was called to represent AFPM under a special committee appointed by Minister of Health to look at the list of qualification that supposed to be listed in the Malaysian Medical Council (MMC). They had already come up with list of the qualifications that are supposed to be there including the icFRACGP, so the list will be submitted to the MMC.They will have their meeting on the 3rd week of May 2024 and will be directed by the Minister of Health to adopt the list submitted.The AG Chamber had agreed to change the law so that similar situation will not occur again in the future. The law will be brought to Parliament meeting in June 2024 and will be passed accordingly.

6 | www.afpm.org.my 52ND ANNUAL GENERAL MEETING Madam President also reported on the Golden Jubilee celebration of AFPM where a community programme was held at the Kampung Sendayan, Negeri Sembilan and took the opportunity to celebrate the Golden Jubilee celebration year 2023. About three hundred (300) members of the community attended the health programme. This year the annual scientific meeting was rebranded as the Annual Scientific Conference and managed to attract six hundred and twenty-seven (627) participants/delegates to attend this conference. It was very interesting observation, almost 90% of participants stayed until the end of the conference of day one and they were very attentive and took part in the Q&A session. It shows that we attracted the right audience for the conference. A Gala Dinner was also held after the conference, and it was assisted by the Chapter of Young Family Doctors (The Rajakumar Movement Malaysia) in arranging the Gala Dinner where there were three (3) celebration conducted concurrently as listed. i) Celebration of 50th Year Golden Jubilee ii) Celebration of World Family Doctors Day: jointly celebrated together with the Family Medicine Specialists Association of Malaysia, FMSA (officially celebrated worldwide on the 19th of May) iii) Introduction of our new Medical Academies Building and beginning of the donation drive for the building. (Flyer on the process of donation drive will be shared). Madam President informed that Dato’ Dr JP Kamalanathan, who is the permanent member of the Board of Task Force will report on the progress on the building. She also informed that the certificate of completion of the medical academies was released the day before on the 27th April 2024. She hoped that the secretariat will be able to move all the moveable furniture to the new building by end of May 2024 and start operation over there. She also shared with the general body that despite certain challenges on the ICFRACGP, we still managed to overcome it and will be conducting our Convocation Ceremony on the 5th May 2024, Sunday at the Sheraton PJ Hotel. She also managed to get the blessings of the DYMM Tuanku to proceed with the convocation and pass the scrolls to the candidates except for the ICFRACGP which will be handled by the Censor in Chief of the RACGP at a second session on the same day. She then invited all present to go to the agenda for the meeting on the Annual Report as follows. 1. To confirm the Minutes of the previous 50th Annual General Meeting. The President invited the general body to go through the minutes of the last Annual General Meeting held on the 29th of April 2023, Saturday which has been circulated to all members. She ran through minutes from pages 3 to 18 of the annual report. She then enquired if anyone had any questions on the minutes and if none, requested the minutes be adopted. The minutes of the last 50th Annual General Meeting were then adopted. Proposed by Dr Kaviyarasan Sailin and seconded by Dr Shamsath Bte Abdul Gafoor. 2. To consider and adopt the Annual Report of Council for the period of April 2023 to April 2024. The President, Dr Norsiah Ali led the house through the report of Council starting from pages 19 to 108 as below. ➢ Individual photos of Council members

Amendments to Constitution The President informed the house that the Honorary Secretary received one (1) amendment to the Constitution, six weeks prior to the AGM and which has been circulated to the General body two (2) weeks before the meeting as below. The Annual Report was then adopted, proposed by Dr Kaviyarasan Sailin and seconded by Dr Sasikala Devi. Madam President then informed that Dato’ Dr JP Kamalanathan had requested to allow him to report on the Academies Building to the general body.

www.afpm.org.my | 9 ACADEMY OF FAMILY PHYSICIANS OF MALAYSIA Report on the Medical Academies Building Dato’ Dr JP Kamalanathan presented the photos of the new building and reported on it as follows. MEDICAL ACADEMIES MALAYSIA (AKADEMI PERUBATAN MALAYSIA) will be an iconic building befitting its status as the home for the Medical Academies and Medical Fraternities of Malaysia. Our previous building in Jalan Tun Razak was compulsorily acquired by the government in 2017 after paying compensation. The land was owned by the two academies at 2/3:1/3 ratio (AMM: AFPM). A joint venture agreement (JVA) signed on 27th August 2006. A Joint Academies Development Committee (JADC) was formed to find a suitable land and build an iconic building with the necessary facilities. Academies Management Committee (AMC) formed a dedicated Building Task Force (BTF) on 22nd September 2019 for the purpose of construction equipping and commissioning of the new Medical Academies Building. Prof. Dato‘ Sri Dr Abu Hassan Asaari Abdullah Academy of Family Physicians of Malaysia 1. Dato’ Dr JP Kamalanathan 2. Assoc. Prof. Dato Dr Mohamad Husni Jamal The Academy of Medicine of Malaysia and the Academy of Family Physicians of Malaysia have forged ahead with getting a new home in Plot 8C8 Precinct 8, Putrajaya. The sales and purchase agreement signing ceremony for buying the land was held on 20th July 2020 between both the Academies and the Putrajaya Holdings Sdn Bhd on a boat cruise. This auspicious occasion would also go down in the history of both the Academies as a major step in our vision of getting an iconic building in Putrajaya. It was our fervent hope at that time that this building would be completed within 24 to 26 months. The Academies were greatly honoured that YAB Tun Dr Mahathir bin Mohamed laid the foundation stone at a ceremony on 10th October 2022 at the construction site itself. YAB Tun Dr Mahathir bin Mohamed is an Honorary Fellow of both Academies. He was accompanied by YAB Tun Dr Siti Hasmah binti Mohd Ali, who is also an Honorary Fellow.

10 | www.afpm.org.my 52ND ANNUAL GENERAL MEETING DETAILS AND LOCATIONS Madam President thanked Dato’ Dr JP Kamalanathan on his comprehensive report. Remarks a Location It is located at Plot 8C8, Precinct 8 Putrajaya on a piece of 2.5 acres of land (108,900 sq ft) b Type of land Freehold & Commercial. c Medical Corridor The Academies had initially wanted to apply for institutional land but were advised against it by the then Prime Minister, Tun Dr Mahathir. He then helped to secure this piece of commercial land in the year 2018 at a concessionary rate of RM215.00 per sq ft. d Connectivity It is 1 km from the Putrajaya Sentral. The Putrajaya MRT 2 line started operation in March 2023. From Putrajaya Sentral, there is a bus shuttle as well as a monorail service which stops at the road in front of the Academies building. e Views & Other Developments The said Building also fronts the mosque and the lake with some new developments (shopping malls, hotels, conference facility (1,000 pax), and residents’ condominiums are ongoing. f Auditorium The auditorium of 6,500 sq ft can accommodate 350 seats with connectivity to the Flexi-hall (380 pax banquet) and the specialist training rooms (5,730 sq ft). The stage would have a drop-down projector screen with LED panels at the back wall. It is accessible from the Ground Floor & and the 1st Floor. g Ground Floor 1) Lobby – 2,890 sq ft 2) Main reception – to usher the participants to the lift lobby or to take the stairs to the upper floor. 3) Café – 2,000 sq ft 4) Multi-Purpose Hall for events/exhibitions -6,500 sq ft 5) Participants can enjoy some refreshments in the relaxing area before proceeding to the event hall 6) The focal view is the feature staircase with floating metal works with a series of steel rods enclosing it which leads to the upper floors h 1st Floor The Flex-Hall is 5,347 sq ft and can be permutated into: 1) Banquet – total capacity 380 2) Seminar Room – total capacity 180 3) Exam Hall – total capacity 242 4) The whole area can be opened for exhibition i 2nd Floor 1. The specialist training areas are the core areas that could attract some potential donors as well as generate some income. 2. There are 2 types of training rooms: a) 7 simulation rooms - Room 1 to Room 7 = 195 square feet (Size 4.3m x 4.2m) b) 13 rooms – Room 8 to 20 = 130 square feet (Size 2.9m x 4m) c) All the training rooms are fitted with: i) Collapsible doors which can be opened for multi-function usage (Except for Room 1 to be a fixed wall) ii) Plaster ceiling with strip lighting including the corridors. iii) Vinyl flooring 3. 11,000 sq ft empty space located on this floor is planned for rental. j 3rd Floor 1) 3rd Floor are the office suites. 2) At this level, the office suites are mostly fully furnished. 3) Most of the offices are open-planned concepts with flexible space functions and agile environments. 4) There is one vacant office space of 715 sq ft that is still available for rent or sale. k 4th Floor 1) MSQH will occupy the biggest space of 6,000 sq ft. 2) There is a vacant space of 1,317 sq ft and the Malaysian Endocrine & Metabolic Society (MEMS) is interested in this. l 5th Floor This empty space is planned for a gym or eating area with about 8,500 sq ft

The President requested the Honorary Treasurer, Dr Aishah Mohd Zain to share and report on the audited statement of accounts. The Honorary Treasurer, Dr Aishah Mohd Zain thanked the President and presented the financial statements of the Audited Accounts for year ending 31st December 2023 as follows: i) Academy of Family Physicians of Malaysia ii) Medibase Sdn Bhd iii) Joint Academies Development Fund Dr Aishah informed that the accounts were audited by the external auditors, En Ali Shah and company. She summarised the important pages starting off with the accounts of the AFPM from page 113 onwards of the Audited Accounts of the AFPM for year ending 31st December 2023 consisting of: ➢ Statement by Council members ➢ Consolidated Statement of Financial Position Dr Aishah, Honorary Treasurer then continued with the report as below: ➢ Consolidated Statement of Comprehensive Income ➢ Consolidated Statement of changes in the Members’ Fund ➢ Consolidated Statement of Cash Flow ➢ Notes to the Financial statements for year ended 31st December 2023 ➢ Independent Auditor’s Report The accounts of AFPM were adopted, proposed by Dr Ng Shiau Pei and seconded by Dr Samantha Veronica Teh Poh Suan. Honorary Treasurer, Dr Aishah Mohd Zain proceeded with the Medibase Sdn Bhd accounts from pages 146 to177 of the Annual Report. The treasurer informed that the reporting on the Medibase accounts have been completed and enquired if there are any other queries and if none, she requested for the accounts of Medibase Sdn Bhd to be adopted. The Medibase Sdn Bhd accounts were then adopted, proposed by Dr Ng Shiau Pei and seconded by Dr Samantha Veronica Teh Poh Suan. Honorary Treasurer, Dr Aishah Mohd Zain continued with the accounts of Joint Academies Development Fund from pages 179 to 198. The JADF accounts was then adopted, proposed by Dr Ng Shiau Pei and seconded by Dr Samantha Veronica Teh PS and Dr Kaviyarasan Sailin. Madam President then presented En Ali Shah with a token of appreciation.

To elect the Council consisting of President, President Elect, Chairman of Council, Honorary Secretary, Honorary Treasurer, and Nine (9) Council Members. The President led the house to item no. 4 in the agenda of the AGM as above. She passed the floor to the Election Committee Chair, Dato’ Dr Jaspal Singh Manocha to proceed with the election process. Dato’ Dr Jaspal thanked all present and thanked fellow colleagues for coming to the elections for year 2024. He referred the house to page 105 and 106 of the Annual Report which has been circulated online as well as hard copies at the venue. He reported on the total nominations received for year 2024. The nominations were closed on 15th March 2024, Friday by Dato’ Dr Jaspal Singh Manocha, Dr Leong Kok Weng and Dr Gurunathan. The total nomination was 20, and out of which, only 10 are valid nominations. They were for the posts as listed below. i) President Elect – Assoc. Prof. Dr Hazian Hamzah ii) Chairman of Council – Dr Kaviyarasan Sailin @ Stalin iii) Honorary Secretary – Dr Wong Ping Foo iv) Honorary Treasurer – Dr Aishah Mohd Zain v) Six (6) Council members - Dr Samantha Veronica Teh Poh Suan - Dr Sasikala Devi A/P Amirthalingam - Dr Rajvinder Singh A/L Mahinder Singh - Dr Ng Shiau Pei - Prof. Dr Khoo Ee Ming - Prof. Dr Kwa Siew Kim There were no nominations for the following posts: i) Three (3) Council members Dato’ Dr Jaspal Singh Manocha informed that it was brought to the election committee’s attention that several members requested additional ballot papers because they did not receive it or was misplaced. It was decided by the election committee that in future the request for another ballot paper will be gladly given to the members concerned on condition they officially write in or email to the secretariat addressed to the Secretary and election committee requesting for an extra ballot paper with their current updated contact details viz email, correspondence address and mobile number. On receiving the request, the ballot paper will be given to the member personally and acknowledged by the member at the AFPM secretariat, but it will be stamped duplicate with the same serial number that is allocated to all original ballot papers, this is to prevent abuse in the nomination process.

The IT staff went through the process of the voting system as below: - Members will be receiving nominations for the polling system. - Followed by an email or SMS notification. - Need to put the membership and the password will be the phone number. - Each member will be given about 20 minutes to complete the voting. The Election Committee Chairman then open for nomination for the post of three (3) Council members. All nominations should have proposers and seconders of good standing. The nominations should also have the acceptance of the nominee. The nomination for the three (3) Council members were opened as below. 1) Dr Kaviyarasan Sailin proposed Dr Shamsath Abdul Gafoor for the post of Council member. Seconded by Dr Samantha Veronica Teh Poh Suan. 2) Dr Sasikala Devi proposed Dr Nagammai Thiagarajan for the post of Council member. Seconded by Gnanasegaran Xavier. 3) Dr Shamsath Abdul Gafoor proposed Dr Husna Hanim Abdul Rahim for the post of Council member. Seconded by Dr Ng Shiau Pei. 4) Dr Wong Ping Foo proposed Dr Loke Yean Yee for the post of Council member. Seconded by Dr Emma Fazilah Zulkifli. Dr Elvind Yip proposed that the nominations for four Council member to be closed. Seconded by Dr Kwa Siew Kim. All the nominations were accepted by the nominees. Prof Datuk Dr Sheikh Mohd Amin made a request for the members nominated to open the video and briefly introduced themselves for the benefit of the members who may not know who they were. Valid nominations for Council members stand at four (4) and out of this, only three (3) Council members would be elected. Dato’ Dr Jaspal Singh Manocha requested the house to give some time for the names to be listed down for the voting process. Madam President enquired if there were any objection to the amendment to the Constitution. There was no objection, and the amendments were unanimously accepted to be changed accordingly.

Installation of the In-coming President The President, Dr Norsiah Ali requested the Incoming President for the term 2024 -2015, Dr Isriyanti Mohd Rafae to come to the floor. The President then adorned the Chain of Office on Dr Isriyanti Mohd Rafae in keeping with the tradition of the AFPM and requested her to give a brief introduction. Dr Isriyanti Mohd Rafae gave a brief introduction on herself. She thanked Dr Norsiah Ali for her exemplary leadership and for all the successful projects for the last one (1) year. She stated that she is not here to compete with her as she had set such a high benchmark but will try to continue her legacy dengan ‘tatangan ilmi’ our AFPM aspiration. She then congratulated all the new EXCO and Council members and then introduced the President Elect, A/Prof Dr Hazian Hamzah who gave a brief introduction of herself. Dr Isriyanti Mohd Rafae thanked A/Prof Dr Hazian and stated that at the 51st birthday of the AFPM, we are looking forward to moving into our new building. As her aspiration of ‘Dengan Tatangan Ilmi’, she believes that as doctors, they are lifelong learners, and she also believes that they have the power of the people and not just about the building. The building looks very awesome, very beautiful and promising but it also the individual that makes the institution. She hoped that all the members will continue to strive with good communication and collaboration with our vision of primary care as our top priority. She thanked all her mentors, exco and council members for giving her the trust to lead the Academy. With all their assistance, she believes that the journey will be pleasant and successful. 11. To Declare the Meeting Adjourned Dr Norsiah Ali gave a speech and called the AFPM AGM of 2023- 2024 to a close with a vote of thanks to the 2023- 2024 EXCO and Council of AFPM, the AFPM’s Trustees, and the AFPM Secretariat for commitment and support. She wished the In-coming President Dr Isriyanti Mohd Rafae every success and passed the KPI file to her. There being no other matter to discuss, the meeting was adjourned at 12.40 pm. Our Conjoint MAFP-FRACGP/ICFRACGP program is scheduled for reaccreditation in 2025 and we are confident that our program will be reaccredited by RACGP. Most meetings are still conducted via zoom platform while workshops are conducted face to face as during the pre-Covid era. With the new Academies Building being completed, future face to face meetings and workshops will be held in the new building whenever feasible. We also vet the applications to sit for the Final GCFM examinations, Part I and Part II Conjoint MAFP/FRACGP/icFRACGP examinations. We strictly follow the criteria stated in the AFPM Postgraduate Courses Guideline and Policy Handbook. Candidates who did not meet the programme criteria or examinations could not proceed. Those who enrolled on the programme with falsified documents or information were thoroughly investigated and, if found guilty, removed from the programme. Board members conducted a few practice visits to applicants' practices to assess practice suitability for the GCFM or ATFM programme. We also conducted practice visits before Part II Conjoint MAFP/FRACGP/icFRACGP examinations as required by the Royal Australian College of General Practitioners (RACGP). As part of our effort to continuously improve the training programmes, we routinely review the rules and regulations for the programmes, examinations, admission criteria, and examination eligibility criteria. The Board of Censors is responsible for quality assurance of the Part I and Part II Conjoint MAFP/FRACGP/icFRACGP examinations. Upon completion of the Part I and II examinations, the Censor-in-Chief conducted QC of the examination mark for all candidates to ensure no errors with the result. Following the release of the part I result, we release the Part II exam application and practice diary instruction to all eligible candidates. We organised a special briefing for Part II candidates on Zoom on 3rd May 2024 (8.30 pm) to provide support and guidance on practice diary submission. Exam applicants submitted their applications in softcopy this year. IT team provided them with a personalised link to upload their documents. Delegated censors will review candidates' softcopy submission and their hardcopy files. Upon closing the Part II application, BOC vetted the applications and practice diaries. We had a meeting to finalise the list of eligible candidates and identified clinics for routine practice visits. Censors conducted visits to most clinics physically. We sent two Censors to tour Johor and visited all the candidates there. Upon completing all BOC tasks, CIC sent a report on candidates' profiles with their practice diaries to the RACGP Censor as part of the accreditation requirement. Upon completion of the Part II examination, again, the Censor-in-Chief conducted QC on candidates' marks. RACGP ratified the result during the post-examination meeting conducted via Zoom on 18th October 2024. Upon the release of the Part II result, the BOC assisted our successful Part II candidates with their FRACGP/icFRACGP application documents. The applications must be vetted and endorsed by the Censor-in-Chief before they are sent to RACGP. We close 2024 activities with our yearly BOC workshop on 29th December 2024 at AFPM's new office. We managed to complete a few essential tasks during the workshop, such as updating the AFPM handbook 2025, preparing a guide for monitoring ATFM trainees' practice throughout their training, finalised the Part 2 exam new application form, updating the list of Part 2 eligible repeaters for 2025, identify a list of prospective FAFP recipients for 2025 convocation and nomination of new BOC members. The year 2025 activity started with the opening of the exam application for Part I Conjoint MAFP/FRACGP/icFRACGP Year 2025 on 1st November 2024. The exam application has now changed to softcopy submissions, and the vetting process was the same as for the Part II exam. We aim to start the online submission of GCFM exam applications in 2025.

44 | www.afpm.org.my 52ND ANNUAL GENERAL MEETING As part of the ongoing RACGP Training Provider Accreditation requirement, AFPM is required to submit six-monthly reports to the RACGP Education Accreditation Committee. This was done successfully as scheduled. We are due to submit the next report in March 2025. Our accreditation certificate will expire at the end of October 2025. Therefore, we will have another accreditation visit by RACGP in 2025. Lastly, I would like to take this opportunity to thank EXCO and council members past and present, and especially the ATFM Faculty members and their various boards, for their encouragement and support, dedication, and great teamwork, which made all the work easier. A total of 271 candidates were eligible, however, only 270 sat for the examination. The examination was conducted at five UniKL IT centres namely RCMP Ipoh (53), BMI Gombak (48), MIDI Cheras (102), MESTECH Kajang (45) and MITEC Johor Bharu (23). Total number of candidates = 138 Total number of examiners = 44 examiners + 15 trainee examiners Total number of coordinators = 16 Total number of AFPM Staff = 16 staff There were 7 QA Examiners; 5 Internal QA examiners (Prof. Dr Chan Sook Ching, Dr Tan Chow-Wei, Prof. Datuk Dr Sheikh Mohd Amin, Dr Lee Tong Weng, and Dr Ng Aik Chiew) and 2 External QA examiners (Dr Tess van Duuren and Dr Kenan Kamau Wanguhu). There were 12 examination stations with 6 stations for each day, over 5 circuits (circuits A–E) and 4 sessions (0815-1000H, 1040-1225H, 1325-1510H, 1550-1735H). There were a few changes made to the exam format for 2024, for example: i Reading time was increased to 5 minutes from 3 minutes ii Duration of each station increased to 10 minutes from 8 minutes iii QA examiners sat with the examiners during standardization sessions for each station Cases for the exams were: (CD= Case Discussion, CE= Clinical Encounter) • Day 1: Peer Level Discourse (CD), Elderly Health (CE), Short Term Care (CD), Emotional Health (CE), Long Term Care (CD) and Practical Procedure (CE) • Day 2: Practice Management (CD), Men Health (CE), Child Health (CE), Women Health (CE), Aboriginal Health (CD) and Emergency Health (CE) Total number of candidates who passed Part 2 = 92 (66.67%) Total number of candidates who failed = 46 (33.33%)

Chapter of Teachers (CoT) 2024 Report The committee members elected in 2022/2023 continued to serve the Chapter of Teachers (CoT) in 2024 with exception of 2 members who had to withdraw due to family matters and acceptance of a working post in Australia. In 2024, the CoT continued to carry out the core activities namely: 1. Mentor Training and Workshop 2. Mentoring Sessions 3. Supervisor Training & Sessions 4. Centralized Workshops - Clinical teaching sessions 5. MAFP/icFRACGP Examination - Involvement of mentors as invigilators and examiners Mentor Training / Workshops We successfully conducted one CoT Teaching and Learning Workshop, in collaboration with the QIP on 3rd August 2024. A total of 88 mentors attended the workshop. Five mentors who have not received their National Postgraduate Medical Curriculum (NPMC) Certificates were also given the opportunity to join the hybrid 3rd NPMC (Family Medicine) Workshop that was conducted in the UiTM Sg Buloh on 24 & 25 September 2024. As for an update on the number of mentors and supervisors, below is the breakdown on the numbers. The CoT continues to encourage all graduates of the MAFP/icFRACGP and Family Medicine Specialists to become mentors and contribute towards the advancement of the Primary Care fraternity in Malaysia.

50 | www.afpm.org.my 52ND ANNUAL GENERAL MEETING Supervisors Briefing Sessions The new structured supervision method that was introduced for trainees who are private General Practitioners (GP) in 2023 was continued. This includes scheduled monthly chart audit sessions with selected GP Supervisors. The briefing sessions for the intake 14 KK supervisors were done online on 28 October 2024. The briefing session for the GP Supervisors was done online on 27 November 2024. WhatsApp discussion groups for the Klinik Kesihatan Supervisor and the GP Supervisors continued to ease communication and information dissemination. The information for supervisors was also uploaded into the e-LMS or the online learning platform under the ATFM Supervisor Module. Centralized Practice Workshop (CPW) CoT had contributed actively in the CPW. A total of five CPWs were successfully conducted in 2024. The following CPWs were held: ATFM INTAKE 13 CPW1: 24 February 2024 (Module CVS, RESPI, PSY, MSK) ATFM INTAKE 13 CPW2: 21 July 2024 (Module GIT, ENDO, GERI, HOP) ATFM INTAKE 13 CPW3: 13 October 2024 (Module GUSS, WOM, MEN, EEN, PAEDS) The trainees were given opportunities to practice in the stations based on the Clinical Competency Examination format, followed by constructive feedback from the facilitators. They also had the chance to practice clinical examinations. In February 2025 we had the opportunity to do the CPW at the new home for the AFPM, namely the Medical Academies Building. It is hoped that this will be the first to the many more training sessions to be done on the premise. Eligibility Examination & MAFP/icFRACGP Examination CoT works closely with the Board of Examiners to identify trainee mentors and mentors to be involved as invigilators as well as examiners for the examinations. Publication of the Articles and Supplement Issues in 2024 In 2024, MFP published 72 articles, which is almost similar with 2023 (73 articles). In addition, there is an increase in original research and review articles, which is 41 articles in 2024 as compared to only 34 articles in 2023. Publication of more original research and review articles is crucial as part of the requirements to be indexed in the Clarivate Journal Citation Report (JCR) under the ISI-Web of Science. Number of articles and the types of articles submitted to MFP In 2024, the Malaysian Family Physician (MFP) received a total of 197 articles (Table 1). www.afpm.org.my | 57 ACADEMY OF FAMILY PHYSICIANS OF MALAYSIA *About the JCR indexing criteria The Journal Citation Report (JCR)/ISI-Web of Science has a four-stage evaluation process for journals to enter the Emerging Sources Citation Index (ESCI) and potentially the Science Citation Index Expanded (SCIE). The first three stages, initial triage, editorial triage, and editorial evaluation, cover 24 criteria and the MFP editorial board has been working to fulfill these criteria over the past 12 months. Upon passing these 24 criteria, MFP will enter the ESCI in JCR. However, to enter the SCIE, a more rigorous evaluation on Impact Analysis consisting of 4 criteria will be conducted by the JCR team. F. DOI of each article registered in Crossref MFP has managed to register an account under Crossref to create DOI for every published article since 2021. All of the 72 articles published in 2024 were registered for a DOI.

The two main reasons for this are: • to cover the increasing operating costs of MFP • to reduce the submission of low quality articles to MFP AFPM Exco approved the proposal and called for a meeting on 2 Dec 2024 to brainstorm the implementation flow, which was attended by the AFPM Exco (Dr Wong Ping Foo), MFP (Prof. Dr Lee Ping Yein, Dr Hani Syahida Salim and Dr Teo Chin Hai) and AFPM staff (Mr Saufi and Ms Atiah). The announcement of the Submission and Publication Fees were posted on MFP website and MFP’s submission portal (OJS). In brief, all authors are needed to submit a RM50 Submission Fee electronically when submitting original research, case report or review articles. An additional publication fee will be payable once the article is accepted for publication (see the details below). Editors of MFP who submit an article to MFP will not be charged. Authors who have reviewed manuscripts for MFP within the past one year may enjoy a waiver of the Submission and Publication Fees. Only one article is waived for one article reviewed. Aims of MFP for 2024 There were five main aims of MFP for 2024 as shown below: No Aims Status 1 Increase the number of associate editors to cope with the increasing workload in MFP 2 new associate editors and 1 local editorial advisor were appointed. 2 Continue to promote MFP and increase the number of research articles submitted to MFP Number of research articles received by MFP has been consistent since 2022. It will further increase once MFP is indexed in WOS. 3 Improve processing time to further shorten publication time Editorial office processing time improved (<1 month for submission to first decision & <1month for acceptance to publication) but peer review time still can be improved. The editorial board decided to increase the number of reviewers invited for each article from two to four to improve peer review time. 4 Conduct another training for reviewers to improve the quality of peer review Will be conducted in May25 with AFPM Annual Scientific Conference 5 Improve the article submission portal (OJS) to further increase the efficiency of manuscript processing The OJS has been improved and consistently being updated. 6 MFP to pass stage 3 evaluation in Clarivate JCR/WOS. Clarivate responded with 3 minor revisions. Revision resubmitted in Jul 2024. I. Plans for 2025 The MFP aims to achieve the following in 2025: 1. Continue to promote MFP and increase the number of research articles submitted to MFP. 2. Improve peer review time. 3. Conduct training for reviewers to improve the quality of peer review (with AFPM Annual Scientific Conference). 4. MFP to be indexed in Clarivate JCR/WOS 5. Implement the submission fee protocol. Malaysian Primary Care Research Group (MPCRG) Annual Report 2024 Malaysian Primary Care Research Group (MPCRG) is a research interest group within the Academy of Family Physicians of Malaysia aims to foster and enhance research initiatives among primary care practitioners, positioning itself as the national hub for primary care research. Over the next few years, the group will focus on promoting the importance of research in primary care, providing training and resources for practitioners to conduct research, facilitating collaboration among stakeholders, disseminating research findings to inform practice and policy, and advocating for funding and support for primary care research initiatives. To achieve this, we have lined up five objectives for next few years. Objectives 1. Build research capacity by training primary care researchers. 2. Facilitate primary care research networking. 3. Develop and share research resources (bibliography and links to research resources). 4. Provide a platform for dissemination and collaboration of primary care research. 5. MOH Supervised Post Qualification Work Experience Posting Committee Report 2024 Previously known as credentialing process, a candidate who has successfully passed his/her Professional Examination and Training in the field of Family Medicine under the Parallel Pathways (non-MOH candidates) need to undergo a period of supervised work attachment under supervision by a Family Medicine Specialist at a government health clinic i.e., Klinik Kesihatan before applying for registration as a Specialist in Family Medicine under the National Specialist Registry (NSR). In 2024, the committee continued with online meetings and had 4 meetings (every three months) to vet new applications, to review logbook of candidates who had completed their posting and to discuss any issues pertaining to the MOH posting. This year, a new improvised logbook has come out which allows trainee specialist to submit only softcopy logbook. We have also standardized 12 months posting for all trainee specialists including those with MRCGP and FRACGP (Australian) qualification. This year we received and approved 13 new applications to start their postings in 2024. 11 specialists had successfully completed their credentialing process and were issued the certificate of completion. The Chapter of Young Family Doctors (CoYFD) Report 2024 The Chapter of Young Family Doctors (The Rajakumar Movement Malaysia) unites Family Medicine trainees and early-career Family Medicine Specialists within their first 10 years of practice. Our mission is to build a supportive and collaborative community among young family doctors, working closely with other chapters in the Academy of Family Physicians of Malaysia (AFPM) to develop meaningful programs and initiatives. This landmark event was a resounding success, uniting esteemed professionals, stakeholders, and community members to celebrate the contributions of family physicians worldwide and emphasizing the critical link between planetary and human health. The evening featured a memorable opening speech by AFPM’s royal patron, DYMM Yang di-Pertuan Besar Negeri Sembilan, Tuanku Muhriz ibni Almarhum Tuanku Munawir, a cake-cutting ceremony marking five decades of dedication to family medicine, a video montage showcasing AFPM’s legacy, and vibrant performances by Monocort and family physicians. Organized by PERDIM, the event saw the participation of AFPM mentors who facilitated small group discussions on diabetes case management. Members of the chapter contributed by assisting in organizing sessions, supporting discussions, and sharing insights on practical diabetes care. This collaboration with our GP colleagues was highly enriching, and the chapter is eager to continue engaging in such valuable partnerships to enhance clinical knowledge and skills. During 21-24 August, Chapter of Young Family Doctors were proud to be part of the WONCA Asia Pacific Region (APR) Conference that was conducted in Singapore. This international event gathered family physicians from across the Asia-Pacific region, providing a platform for cross-border collaboration and the sharing of best practices. Postgraduate Training in Family Medicine under the Academy of Family Medicine of Malaysia (AFPM) Report for Quality Improvement Programme 2024 Background The Quality Improvement Programme (QIP) for Training was first set up in 2018 to ensure quality in the training of doctors undergoing the 4-year Family Medicine (FM) postgraduate training course; the first two years of which, is an on-line distance-learning culminating in the award of the Graduate Certificate in Family Medicine (GCFM). This is followed by another two years of Advanced Training in Family Medicine (ATFM) in which mentors are assigned to meet trainees monthly for teaching. QIP is a formative work-based assessment to monitor and improve the trainees’ clinical competencies. The first QIP rolled out in August 2021 with Intake 10 Year 1 on the virtual platform due to the pandemic. Currently we have progressed to face-to-face visits to all GP clinics and those government health clinics within 35 km to the Practice Assessor. Training for the QIP Practice Assessors is held during the half day QIP Workshop which is held on the second day of the Mentor Training workshop. A briefing on the QIP process and standard operating procedures for internet technology requirements for the online sessions is always given to Practice Assessors and Trainees for each intake before the start of the first QIP session. QIP Activities QIP activities are held during the 3rd and 4th year of training during the Advanced Training in Family Medicine (ATFM). The QIP activities are case-based discussions, the QIP Practice Visit which is a precepting session and the GP clinic Practice Assessment. The objectives of the QIP case-based discussion and precepting sessions are mainly to teach, assess and provide feedback on candidates’ workplace-based performance to improve clinical competencies. The objective of the GP clinic Practice Assessment is to assess that the GP clinic is suitable for Family Medicine Specialist training. The case-based discussions sessions are conducted during the monthly mentoring sessions using the cases from the trainee’s logbook for discussion. The Practice Visit, which is the precepting session is a maximum 4-hour direct observation by the Practice Assessor, either online or face-to-face, of the trainees’ consultation of four cases at their workplace. All GP trainees from the year 2024 will have face-to-face precepting sessions. QIP Practice visits are conducted once each during ATFM Years 1 and 2. Practice Assessors should preferably be the trainees’ own Mentor so that remediation can be continued, and trainees’ progress tracked during the monthly mentoring sessions. The GP clinic Practice Assessment is done during the first year Practice Visit, and the report submitted to the Board of Censors. The GP trainee will be advised by the Board of Censors on any requirements that need to be improved to meet the requirements of a Family Medicine Specialist training facility. The feedback from trainees and Practice Assessors has so far been positive as there is one-to-one observation and feedback on the trainee’s consultation and clinical performance. Practice Assessors can identify their trainees’ needs and support them during the following mentoring sessions to improve their clinical competencies.

84 | www.afpm.org.my 52ND ANNUAL GENERAL MEETING QIP activities in 2024: 29 trainee mentors were trained as QIP Practice Assessors during the QIP workshop on 4th August 2024, making a total of 134 trained Practice Assessors. Intake 13 Year 1 and 2 completed their Practice Visits in 2024. All GP trainees had a face-to-face Practice Visit and had their GP clinic Practice Assessment done to ensure the suitability of their clinic for Family Medicine Specialist training. Collaboration with Pertubuhan Doktor-Doktor Islam Malaysia (PERDIM) Pertubuhan Doktor-Doktor Islam Malaysia (PERDIM) collaborated with QIP, AFPM to run a two-day workshop on Diabetes Mellitus on 17th and 18th August 2024. It was an interactive workshop with 10 scenarios provided and facilitated by the QIP Practice Assessors on common cases seen in practice, based on diagnosis of diabetes mellitus, management and management of diabetic complications. The feedback from the participants was very positive and the interaction during the workshop in the various small groups was fruitful and educational for both facilitators and participants. Diagnosis and Management of Fever in Primary Care • Date: October 13, 2024 • Feedback Highlights: o The webinar was well-received for its inclusivity and engaging Q&A session. o Suggestions for future topics included dengue, STI management, ECG interpretation, and women's health. o Recommendations to invite speakers from tertiary centers for additional perspectives.

Introduction I am pleased to present the Annual Report for the Johor Section of the Academy of Family Physicians of Malaysia (AFPM) for the year 2024. It has been a year of continuous efforts to encourage our General Practitioners (GPs) in Johor to expand their professional competencies and maintain the highest standards of clinical practice. Despite not organizing our own seminars this year, we have actively collaborated with various organizations and utilized the Johor GP united WhatsApp group to disseminate information and opportunities for professional development. 2. Promotion of the GCFM Program A key highlight of 2024 was our ongoing push to increase enrollment in the Graduate Certificate in Family Medicine (GCFM) program. This program remains a cornerstone in strengthening family medicine practices, offering structured training, mentorship, and an evidence-based curriculum. Throughout the year, we: ▪ Encouraged GPs to consider the GCFM program as a means to advance their clinical expertise and understanding of comprehensive patient care. ▪ Shared success stories and testimonials of GPs who have benefited from the GCFM program. 3. Encouraging Continuous Professional Development (CPD) Recognizing the importance of ongoing learning to keep pace with rapidly evolving medical knowledge, we urged GPs to participate in various workshops, courses, and seminars organized by reputable institutions. These included: • AFPM Workshops and Training Sessions: • Whenever AFPM organized workshops, we circulated details and encouraged Johor GPs to register early for slots. • Universities and Hospitals: • We promoted CPD events hosted by local universities, teaching hospitals, and specialized centers to ensure GPs had ample opportunities to upgrade their clinical skills and stay updated with best practices. • Inter-Professional Collaboration: • We encouraged GPs to collaborate with specialists through case discussions and shared learning sessions, fostering a more holistic approach to patient care. 4. Webinars and Online Learning With the growing popularity and convenience of digital platforms, webinars have become a mainstay for professional development. Throughout 2024, the AFPM offered several webinars on topics ranging from chronic disease management to the latest advancements in family medicine. As the Section Head: • I promoted these AFPM Webinars vigorously on the Johor GP united WhatsApp group. • Shared relevant links, schedules, and summaries to make it easier for GPs to join and benefit from the sessions.

Communication via Johor GP United WhatsApp Group Our dedicated WhatsApp group has been an invaluable tool in engaging with GPs across Johor. It has allowed us to: • Disseminate timely announcements on upcoming events, webinars, and workshops. • Foster discussion on clinical updates, case studies, and emerging trends in family medicine. • Provide a platform for GPs to network, ask questions, and share resources. 6. Collaboration with Other Organizations While the Johor Section did not organize our own seminars this year, we successfully leveraged the plethora of medical seminars already available to Johor GPs through: • MMA Johor: Many GPs actively participated in the regular seminars and workshops organized by MMA Johor. • Private Hospitals: Private hospitals in the region hosted multiple CPD programs, and we worked closely with them to encourage attendance and collaboration. By promoting these established events, we ensured minimal duplication of effort and maximized attendance and resources for all involved. 7. Plans for 2025 Moving forward, the Johor Section will: • Continue to Encourage GCFM Enrollments: We aim to see a significant increase in the number of GPs enrolled in the GCFM program. • Expand Online Learning Platforms: Work with AFPM to offer more engaging and topic-focused webinars, ensuring diverse clinical topics are covered. • Strengthen Collaborations: Explore potential partnerships with other professional bodies and healthcare institutions to provide quality training programs tailored to the needs of Johor GPs. • Revisit In-Person Seminars: Depending on demand and feedback, we may consider organizing specific in-person seminars next year to address targeted topics not fully covered by other providers. 8. Conclusion In conclusion, 2024 has been a productive year in terms of advocating for continuous professional development and promoting the GCFM program among Johor GPs. Our strategic decision not to host seminars was compensated by close collaboration with existing events hosted by MMA Johor and private hospitals, allowing us to direct our efforts towards effective promotion and engagement. I extend my sincere appreciation to all Johor GPs for their commitment to advancing family medicine and to the AFPM for its unwavering support. One forum on Insightful talk on Diabetic Foot Ulcer: Time is Tissue was successfully carried out on 28th February 2025 by Associate Professor Dr Edward Choke Tieng Chek Consultant Vascular & Endovascular Surgeon based at the Northern Heart Hospital Penang held at Mantra Indian Cuisine Bayview Hotel Georgetown. About 30 doctors attended the above talk where Asso Prof Dr Edward highlighted their causes, complications and the latest advancements in treatment and prevention. He also discussed the pathophysiology diabetic foot ulcers, focusing on neuropathy, ischemia and infection as key contributing factors. He also underscored the significance of patient education, proper footwear and regular foot care in preventing ulcers and amputations. Recent advancements in wound care including negative pressure therapy, skin grafting and revascularization techniques were also explained. The session concluded with an engaging Q&A where Asso Prof Edward addressed practical challenges in managing diabetic foot ulcers. The talk provided valuable insights reinforcing the need for early intervention and comprehensive care in reducing the burden of diabetic foot complications.

Building – Medical Academies A) Introduction With the disbanding of the Joint Academies Development Committee (JADF) after its last meeting on 26th July 2019, the Academies Management Committee (AMC) decided to form a new Building Committee, the Academies Building Task Force (BTF) to manage and oversee the development of a 5-storey new Medical Academies Building in Putrajaya. The membership comprises of five (5) members representing from the Academy of Medicine of Malaysia (AMM) and Academy of Family Physicians of Malaysia (AFPM): a) Tan Sri Datuk Dr Ridzwan Abu Bakar - Chairman b) Datuk Dr Yeoh Poh Hong - AMM c) Prof. Dato’ Sri Dr Abu Hassan Asaari Abdullah - AMM d) Dato’ Paduka Dr JP Kamalanathan - AFPM e) Assoc. www.afpm.org.my | 97 ACADEMY OF FAMILY PHYSICIANS OF MALAYSIA B) Meetings The BTF has regular monthly meetings with the building consultants which include the project managers and the architects as well as the regulatory authorities as and when required. Both the AMM Master and the AFPM President were also invited to these monthly meetings. It is of interest to note that the BTF has seen through the tenure of 6 AFPM Presidents since its formation. BTF also monitors the work progress of the contractors via the BTF-appointed project managers who in turn would provide a monthly report to BTF. Most of the meetings were held at the Pantai Hospital KL in the earlier days and virtually during the Covid-19 Movement Control Order (MCO) days as well at GDP Office at Plaza Damansara KL. Apart from these interactions, there were decisions also made through emails and WhatsApp messages. C) Updates On Putrajaya Land and Building The BTF had an arduous task and faced many obstacles along the way due to the ongoing coronavirus epidemic and the unstable geopolitics which began in March 2020. This has significantly impacted all the economic sectors globally, including the building industry which disrupted the supply chain of building materials and labour shortage, both of which resulted in the escalation cost of the building materials and the budget overrun including the delay in the completion of the building project which has been post-phoned several times. D) Awarding Of CPC And CCC Even though BTF had faced many unforeseen obstacles and challenges along the way, the BTF together with the supporting staff and building consultants weathered through the challenges and were able to carry on with their task to complete their targets. The BTF finally obtained the Certificate of Practical Completion (CPC) from the main contractor on 27th April 2024 followed by the Certificate of Completion and Compliance (CCC) on 18th June 2024, which was then ratified by the Architects to 15th May 2024. This means that the consultants are satisfied that the building has been completed according to the building specs that were agreed by BTF at the beginning. It also means the defect liability period starts on this day, 15th May 2024 and ends on 14th May 2025. After much pressure from BTF, using the whip and the carrot, the Certificate of Completion and Compliance (CCC) was issued by the Architects GDP with the blessings of the local authority PJC on 18th June 2024, one day after Hari Raya Haji. This certificate is an official recognition that the building is fit for the purpose it was built for, meaning that it has been well-constructed and suitable for occupants to move/live in. This was the biggest challenge and arduous task that the BTF must face since the building project started. With the above awards, the BTF is proud that it has successfully accomplished its main duties and obligations in overseeing the construction, equipping and commissioning of the New Medical Academies Building as one of the terms of reference. The Academies then started the relocation to the Putrajaya office on 27th and 28th June 2024 and was fully operational from 1st July 2024 onwards.

Today, the Medical Academies Building stands as a proud icon within the medical corridor near Putrajaya at Precinct 8. With this building, both the Academies would be able to achieve their vision to continue expanding their roles and advocates in promoting post-graduate education and the training of medical specialists. As a vital part of the global WONCA network, the Asia Pacific Region focuses on enhancing family medicine through education, policy advocacy, and collaboration among healthcare professionals in this region. The Role of WONCA Asia Pacific The Asia Pacific Region encompasses 23 countries of different healthcare systems, cultural backgrounds, and socio-economic conditions. WONCA works to bridge gaps in healthcare delivery, ensuring that family medicine remains a cornerstone of primary care. Message Form Honorary Secretary The Academy is proud to reflect on a successful 2024, a year filled with enriching events, new collaborations, and achievements in the local and international family medicine scene. The Academy strengthened its commitment in advancing primary healthcare and supporting family doctors nationwide through a lineup of conferences, workshops, courses, professional exams, and continuous professional developments delivered both physically and online. The Academy successfully held the Annual Scientific Conference 2024, held from 26th -27th April at Everly Putrajaya, centered on the theme "Primary Care - For Each and Everyone." This conference highlighted the essential role of primary care providers from both the government and private sectors and equipped delegates with the knowledge and inspiration to continue making a positive impact on their communities. As part of the conference, The Academy celebrated its 50th Golden Jubilee with a remarkable event, combining the Golden Jubilee Gala Dinner and World Family Doctor Day festivities under the theme "Healthy Planet, Healthy People” on the evening of 26th April 2024. The World Family Doctor Day was a joint celebration with our sister organization, Malaysian Family Medicine Specialists’ Association (FMSA). This event brought together esteemed professionals, stakeholders, and community members to honor the contributions of family doctors worldwide and highlight the vital connection between planetary and human health. Alongside other professional organizations, the Academy played a vital role in supporting the amendment of the Medical Act 1971 (Act 50). Our heartfelt gratitude to the Honorable Minister of Health and Honorable Minister of Higher Education for supporting the coexistence of parallel pathways and Master of Medicine programs for specialist training in Malaysia. This significant move marks a progressive step forward in addressing the country's need for more specialists in our healthcare system. On 12th October 2024, the Academy celebrated a significant achievement with the officiation of its new premises at the Medical Academies Malaysia Building. The event was graced by the esteemed presence of our Patron, His Royal Highness, Yang di-Pertuan Besar Negeri Sembilan Darul Khusus, Tuanku Muhriz Ibni Almarhum Tuanku Munawir, who officiated the ceremony. Also present was Her Royal Highness, Tunku Ampuan Besar Negeri Sembilan, Tuanku Aishah Rohani binti Almarhum Tengku Besar Mahmud. This state-of-the-art building features advanced amenities, including the TJ Danaraj Auditorium with a seating capacity of up to 350, versatile Flexi Halls for various events, and the Academies Simulation Education Training Sites dedicated to simulation-based medical training. Recognising the important role of Family Physician, Dr Zainal Fitri Bin Zakaria, our member was appointed as the Royal Family Physician for His Royal Highness, The Yang di-Pertuan Besar Negeri Sembilan Darul Khusus, Tuanku Muhriz Ibni Almarhum Tuanku Munawir and His Royal Family.
