Employee Benefits Guide March 2024– February 2025EligibilityAll full-time employees are eligible to enroll in the employee benefits outlined in this guide. If you are a newly hired employee, you become eligible for benefits on the 1stof the month following 60 days from your date of hire. Employees may also enroll their spouse and any dependent children up to the age of 26 in the benefits they elect. If a dependent child turns 26 during the plan year, he or she will automatically be removed from the benefits at the end of their birth month as they are no longer eligible. For questions on dependent children Eligibility, please visit https://www.healthcare.gov/young-adults/children-under-26/. Open EnrollmentWe are moving with United Healthcare (Medical), Equitable (Dental & Vision) and Colonial (Supplemental) for the 2024-2025 plan year. Open Enrollment is from February 15th-19th. To get enrolled this year, all eligible employees will need to speak to a Benefits Counselor. A Benefit Counselor will meet with each employee individually and review all benefit options. There will be a Benefit Counselor onsite February 15th. If you are unable to meet with a Benefits Counselor in person, please schedule a call with a Benefits Counselor at the following link or scanning the QR code:Elections must be made no later than Friday, February 19th. This is an active enrollment which means all employees must speak to a Benefits Counselor and either elect or decline all coverages as we will also be offering NEW voluntary supplemental coverage. You must speak to a counselor and elect/decline coverage even if you are not making any changes. Emergency Room visits are for life-threatening emergencies only (ex: seizures, major blood loss, compound fractures, head injury)2. Urgent care is for urgent but not life-threatening concerns (ex: a few stitches)3. Primary Care Doctor is for sickness that cannot be diagnosed via telemedicine (ex: strep throat, sprain)3. Telemedicine allows you to speak with a doctor over the phone within minutes and can be used to treat several conditions, such as the flu, earache, sinus infections, allergies, etc. If you aren’t sure, start here!4. When filling a prescription, ask for generic or over-the-counter equivalent. Note: if you’ve seen a commercial for that drug, it is most likely a specialty and will cost you.

Virtual care services include general medical, dermatology, mental health, back and neck care.How do I access HealthiestYou?Plan participants can download the HealthiestYou app, visit the website at member.healthiestyou.com or call 1-866-703-1259 to set up their account. Once their account is set up, they can access all of the HealthiestYou services through the HealthiestYou app or website, and visits with a doctor by calling the number above.What is Expert Medical Services?In addition to the virtual care services that HealthiestYou provides, plan participants also have access to Expert Medical Services through HealthiestYou. This includes procedures or hospital confinement for complications arising from elective or cosmetic surgery. This does not include congenital birth defects or anomalies of a child, or reconstructive surgery related to a covered sickness or injuries received in a covered accident.  Committing or attempting to commit a felony, or engaging in an illegal occupation.  Having a disorder including but not limited to aective disorders, neurosis, anxiety, stress and adjustment reactions.
