AME Zion Church Revised HRSA COVID 19 Vaccine Outreach 3 month and 6 month proposed extension Scope of Work The African Methodist Episcopal Zion Church AME Zion Church is requesting additional funding for the continuation of its work to educate and vaccinate persons against the Corona Virus We will continue the work began in the initial phase of this grant and submit a proposal for an extension that encompasses 3 months and 6 months of continued service to undeserved communities The continuation of this project will intensify the AME Zion Church s planning and implementation of services to improve vaccine confidence Our outreach efforts will be primarily those families with children 12 and their extended families and communities with barriers to vaccine acquisition and education This will be accomplished by 1 Training and deploying a team of four 3 administrative team members seven 7 Area Coordinators and ten 10 Community Health Workers to provide culturally appropriate messaging outreach vaccine navigation and resources across Maryland Virginia Delaware and West Virginia 2 Work to diligently collaborate with other agencies programs faith communities schools etc as a resource for education and vaccination referrals Emphasis will be upon utilizing the A M E Zion Church as host sites for clinics and educational forums 3 Disseminating vaccine education and resource information through community programming events through collaboration with local schools worship centers and media outlets with the potential to reach an estimated goal for vaccine education of 2 000 persons with no less than 1 000 3 000 persons receiving their first or second vaccination 3

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach 3 month and 6 month proposed extension WORK PLAN FOR Team Ujima AME Zion Church MAED INFRASTRUCTURE Historical Perspective The A M E Zion Church The African Methodist Episcopal Zion Church is historically known as the Freedom Church It was officially formed in 1821 in New York City and was birthed out of overt discrimination and socioeconomic barriers in the White Methodist Church The first church founded by the AME Zion Church was built in 1800 and was named Zion The A M E Zion Church has been part of the Abolitionist movement and continue to be a voice for social justice and Freedom in the 21st Century Educational advancement education about health disparities and access social and economic uplift and missional outreach are pillars of this denomination The AME Zion Church is far reaching with churches in North and South America in the countries of Nigeria Liberia Malawi Mozambique Angola C te d Ivoire and Ghana in Africa England India Jamaica St Croix Virgin Islands Trinidad and Tobago GuyanaSuriname St Vincent and the Grenadines and Barbados in the Caribbean The MidAtlantic Episcopal District of the AME Zion Church of which this team is a part is comprised of the State of Pennsylvania Ohio Tennessee Virginia Maryland Washington DC Barbados Guyana Suriname and St Vincent and the Grenadines in the Caribbean There are approximately 161 African Methodist Episcopal Zion Church that comprise this region These churches range in size building structure employing both full time and bivocational leadership The Mid Atlantic operates under the Direction of a Bishop W Darin Moore who oversees administratively and spiritually the work of the entire Episcopal Area Each Episcopal District is determined by the General Conference of the AME Zion Church which is the governing body of the Church which convenes every four years 4

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach 3 month proposed extension and budget WORK PLAN FOR Team Ujima AME Zion Church MAED MONTH 1 December Objective 1 Collaborate with Coordinators and Community Health Workers in the implementation of a team all hands on deck approach to reach underserved areas of Petersburg VA Bluefield WV Eastern Shore MD and Delaware where the hesitancy numbers remain high and vaccinations remain low in the first two weeks Objective 1a Host a Community Holiday gathering in the City of Petersburg VA by working with local faith leaders and community sponsors to educate and vaccinate persons in Central Virginia Objective 1b Host a Community Holiday gathering with the Eastern Shore of Maryland and Delaware team who border each other This effort will involve calling all of our partners and others who are subrecipients to join together to reach those in the community These holiday gathering will involves a Tree lighting children gift giveaways and the disbursement of Holiday staples for the first 50 persons vaccinated Objective 1c Each area Coordinator will be asked to intensify their efforts to vaccinate persons and present a brief plan as to how they will accomplish this goal MONTH 2 January Objective 2 Due to possible weather constraints our plans are to shift our interactions with the community to a social media PSA and advertisement campaign that involves local supermarkets pharmacies schools and local recreation centers The team will explore local YMCA Boys Clubs the Girl and Boy Scouts of America and Pediatricians who will allow us to education and share information with their clientele Objective 2a Reach out to untapped resources about collaborating and developing partnership to ensure the health of the communities where they serve Objective 2b Develop a presence on Facebook Instagram and Twitter providing information and how persons can access vaccinations in our target areas 5

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach 3 month proposed extension and budget WORK PLAN FOR Team Ujima AME Zion Church MAED MONTH 2 January con t Objective 2c The goal will be to target at least 200 pharmacies and supermarkets to disseminate educational materials with information that allows persons to contact the team for one on one transportation to vaccination sites Objective 2d Our team will seek to acquire 5 000 interactions through our efforts outlined in objectives 2a 2c An added bonus would be to help individuals navigate the CDC website for information as well as the local health departments in their targeted zip codes MONTH 3 February Objective 3 In the height of the winter Team Ujima will utilize 80 of its time utilizing Virtual Trainings and Forums in all seven 7 of our target demographic areas With flu season also a concern the need to retrain and provide basic health tips will be vital to prevent the spread of both the flu and Covid 19 These forums will provide vaccination sites following each session and information on how Team Ujima can provide transportation for those desiring to be vaccinated weather permitting The key to our virtual efforts is the engagement of children youth and young adult ambassadors to reach their demographic I ve Been Vaccinated Have You campaign Objective 3a Develop a presence on Facebook with emphasis or branding and marketing the Team Ujima brand Deploy vaccine hesitancy ambassadors who will help to post cause trends and allow the teams efforts to go viral in our attempt to reach several hundred persons with each posting Objective 3b As a secondary effort develop a presence on Instagram utilizing many of the pictures taken during the initial phase of the grant to incite interest and disseminate information With emphasis or branding and marketing the Team Ujima brand Deploy vaccine hesitancy ambassadors who will help to post cause trends and allow the teams efforts to go viral in our attempt to reach several hundred persons with each posting Objective 3c Develop a presence on Twitter utilizing many of the aforementioned strategies in 3a and 3b and the same followup with regards to how persons can access vaccinations in our target areas 6

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach 6 month proposed extension and budget WORK PLAN FOR Team Ujima AME Zion Church MAED In the event that it is advantageous for both parties that we engage in a 6 month extension of this life saving work we are proposing the same timeline presented for the 3 month extension with the following three months being utilized in the following manner MONTH 4 6 March April and May of 2022 Objective 1 Ensure that all teams members are working at least two weeks out of a month in the community revisiting those locations and sites that collaborated regularly with us in the Fall At this point we want to ensure that we are capturing all required data and other information relative to the scope for proper tracking and subsequently evaluation In the remaining 3 months of the project we want to make sure the team has opportunity for continued growth and assessment of best practices At this junction all facets of our scope of work will be utilized to include canvassing education vaccinations virtual forums social media blasts advertisements City Wide Community Events and the utilization of partnerships Objective 1a Coordinators and CHW s will conduct community outreach targeting locations in jurisdictions with high vaccine hesitancy utilizing the information provided in our weekly team meetings maps and other data from the CDC HRSA and local health departments Objective 1b Coordinators and CHW s will target Hispanic communities in Hyattsville MD and the surrounding areas working with CASA and other grass roots organizations to form partnerships and establish trust in these communities This will be in tandem with providing persons with resources for transportation childcare and the vaccine care packages Objective 1c Coordinators and CHWs will receive on going best practice training at monthly meetings utilizing the same and additional resources mentioned previously throughout the strategic timeline A resource manual for sustainability will be provided to all partners to ensure the continued access to information and vaccine site locations Objective 1d Ensure that the proper outreach is done to connect team members to those persons who received their second shot and provide those persons with care packages to applaud their collective responsibility which is the meaning of Ujima 9

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach APPENDIX A 2 Job Descriptions AME Zion Church Bishop W Darin Moore Program Administrator Will oversee all operations and work closely with the Program Director He will periodic meetings for necessary updates and facilitate the support of the Mid Atlantic Episcopal District He will attend and conduct trainings as his schedule permits Dr Evalina Huggins Program Director oversees the day to day activities regional schedules submits budgeting and invoicing attends weekly University of Maryland meetings interacts with governmental officials reviews all public announcements and flyers determines the training schedule based upon the weekly meetings with UMB and HRSA requirements Ensure the payroll distribution orders or secures supplies as outlined in the budget monitors Qualtrics data entry holds weekly internal administrative meetings with team members and provides periodic update to the Program Administrator While supervising Team Ujima the program director will engage in Canvassing Education Information dissemination and vaccine clinics referrals and transportation Ms Alisha Dawkins Assistant to the Program Director is responsible for receiving and responding to all correspondence attend the weekly UMB meetings handle all administrative paperwork and work directly with the community health workers to problems and provide resources She will be expected to travel across our catchment area and interact with community agencies governments and others who can assist in providing facilities and opportunities for education and vaccinations Data Management Information Technology Director no longer needed in the extension This will be done by the Assistant to the Program Director Dr Timothy Freeman Invoicing Financial Review Specialist He will handle all invoices for salary support work with the program director to develop and review the budget provide finances for supply requisitions monthly phone internet support grant acquisitions of supplies and other responsibilities that require invoicing among team members the University of Maryland Health System and the State of Maryland This individual will ensure that we operate within the approved budget suggest the reallocation of funds to address justifiable and approved resources In this phase of the grant he will also serve as a Community Health Worker in Prince George s County 12

AME Zion Church Revised HRSA COVID 19 Vaccine Outreach APPENDIX A 2 Job Descriptions AME Zion Church Rev Milton A Williams Jr State Wide Coordinator for Maryland He will work directly with Regional Coordinators and CHW s throughout the State of Maryland Given the distance and reach required to cover Baltimore City County Prince Georges County and the Eastern Shore of Maryland These areas require extensive travel and oversight to ensure the implementation of all phases of this grant are successfully met across several jurisdictions The State Wide Coordinator will have similar responsibilities as the Program Director although State specific and will report directly to the Program Director and Administrator Regional Coordinators specific to the target areas of Baltimore City County MD Prince George s County MD Eastern Shore Maryland Delaware Western Virginia West Virginia Central Virginia Eastern Virginia Community Health Partners CHW s Refer to A 1 for list These are individuals responsible for galvanizing persons groups and organizations to collaboratively to education and vaccinate persons desiring the Covid 19 vaccine Their responsibilities include completing the CHW Profile Events Summary Education Summary and Vaccine Summaries in Qualtrics Community canvasing developing partnerships scheduling opportunities for education events and vaccine is within their purview They engage in the planning and implementing of training sessions provide transportation and utilize teams of 10 persons to assist them in their responsibilities They will receive training and staff development from the Program Director State Coordinator Regional Coordinator and University of Maryland Health Team Where at all possible we will utilize CHP workers from across jurisdictions to accomplish our goal of educating and vaccinating as many potential candidates as possible for City Wide or Community events 13
