Our plan is a Blue Options PPO, which allows you to seek services from in-network providers of your choice without a referral. HEALTH SAVINGS ACCOUNT (HSA)This booklet provides only a summary of your benefits. The main purpose of this account is to offset the cost of a qualifying high deductible health plan (HDHP) and provide savings for your out-of-pocket eligible health care expenses - those you and your tax dependents may have now, in the future, and during your retirement.Why Is It A Good Idea To Have An HSA?HSAs benefit everyone who is eligible to have this account, including single individuals, families and soon-to-be retirees. You save money on taxes in three ways:• Tax-Free Deposits: The money you contribute to your HSA isn't taxed (up to the IRS annual limit)• Tax-Free Earnings: Your interest and any investment earnings grow tax-free• Tax-Free Withdrawals: The money used toward eligible health care expenses isn't taxed now or in the futureSetting aside pre-tax dollars into your HSA means you pay fewer taxes and increase your take-home pay by your tax savings. You save money on eligible expenses that you are paying for out of your pocket. The amount you save depends on your tax bracket. For example, if you are in the 30% tax bracket, you can save $30 on every $100 spent on eligible health care expenses.HSA funds roll over from year to year and accumulate in your account. There is no "use-it-or-lose-it" rule with HSAs, and you decide how and when to use your HSA funds, which can be used for eligible expenses you have now, in the future, or during retirement. Also, when you have a certain balance in your HSA, investment opportunities are available.Refer to your HSA documentation for more information.HSA Contribution LimitsFor the 2024-2025 plan year, Sentinel will contribute $65 (twice per month) to your Optum Bank HSA up to an annual amount of $1,560. This amount counts to the IRS annual maximum listed below. To receive the employer contribution, you must enroll in the HDHP and HSA.You can contribute to your HSA on a pre-tax basis through payroll deductions up to the IRS statutory maximums. The IRS has established the following maximum HSA contributions:FOR THE 2024 TAX YEAR: $4,150 Individual / $8,300 FamilyFOR THE 2025 TAX YEAR: $4,300 Individual / $8,550 FamilyIf you are age 55 and over, you may contribute an extra $1,000 catch-up contribution. This is available in 2024 and 2025.

Telehealth provides acute and behavioral care 24 hours a day via phone or video by board-certified doctors and behavioral health specialist. Telehealth is a good option for minor health problems when you can’t see your regular doctor. It is also a convenient choice when you want to speak to a counselor or therapist. Some providers will also offer telehealth appointments. These accounts are administered by Flores.Flexible Spending Accounts allow you to set aside money from your paycheck before taxes are taken out. You then use your pre-tax FSA funds throughout the plan year to pay for eligible health care or dependent care expenses. You save money on expenses you're already paying for. These programs are designed to fill the gaps in coverage such as your deductibles and co-insurance under your major medical. These benefits are elected on a voluntary basis and paid for by the employee.WORKSITE BENEFITSAccidentPays a lump sum benefit if you or a covered family member experience an accident. Coverage can be purchased for employee and dependents. Wellness benefit is offered to all covered participants. Coverage is offered through Sun Life.Critical IllnessPays a lump sum benefit should a covered family member experience a heart attack, stroke or other covered serious illnesses. Wellness benefit is offered to all covered participants. Coverage is offered through Sun Life.Hospital IndemnityPays a lump sum benefit for hospital-related costs related to an illness or injury. Wellness benefit is offered to all covered participants. Coverage is offered through Sun Life.LegalFree and discounted legal care for divorce, traffic ticket defense, document review, etc. This benefit includes free tax return preparation and identity theft solutions. Any PTO requested in the first 90 days of employment is subject to prior approval from the supervisor. Consecutive time off should be limited to 5 business days, unless otherwise approved by the Managing Partner. Whenever possible, PTO should be requested to your supervisor in writing at least 14 days prior to the first day of leave. If an employee’s time off usage is deemed to be excessive causing undue burden on team members, missed deadlines, or a decrease in productivity or service to Clients, that employee may lose the ability to participate in the unlimited paid time off policy.Community ServiceFull-time Employees are awarded forty (40) hours of Community Service Leave annually which may be used for volunteer participation in the programs, services, and organizations during regular business hours. Employees must receive approval from their supervisor to use this leave. The supervisor may require that the leave be taken at a time other than the one requested, based on the needs of the firm and its clients. Approved community service leave hours must be tracked in the PTO timekeeping system under “Community Service.” Leave not taken is forfeited; it shall not be carried into the next calendar year and employees shall not be paid for this leave upon separation.Parental LeaveSentinel provides up to four weeks of paid parental leave following the birth of an employee’s child or the placement of a child with an employee in connection with adoption or foster care. The purpose of paid parental leave is to enable the employee to care for and bond with a newborn or a newly adopted or newly placed child. This policy will run concurrently with Family and Medical leave, as applicable.For more information, please see Employee Handbook.

Holiday pay for non-exempt employees will be paid at the employee’s regular rate of compensation for the hours the employee regularly would have worked during the day the holiday is observed. Holiday pay for an exempt employee is already included in his or her salary for the week. If a holiday falls during a period in which you are on an approved paid leave, you shall receive holiday pay for that recognized holiday and any accumulated vacation days will not be charged against you for that holiday. Unused holidays will not be reimbursed in cash or additional days off and are forfeited upon separation from employment for any reason. To be eligible, you must be an active employee past 90 days of employment, in good standing, and caught up on all outstanding tasks. Eligible associate will choose one set day every week to work from home- either a Tuesday or Thursday. The second day will rotate weekly between the remaining days, excluding Wednesday. All associates should be in the office Wednesdays for Working Together Wednesdays each week barring any previously approved time off.FLEXIBILITY IN THE WORKPLACE
