President & CEO MessageDear Friends, I am so grateful for the contributions and dedication of our incredible Family Health Center (FHC) team. As President/CEO, one of the joys of my role is being able to lead a dynamic team of individuals who show each day how much they love our community and value each of our community mem-bers.Everyday, I witness our team go above and beyond to provide quality care to our patients regardless of their ability to pay for our services. I continue to admire the extent to which our providers and other team members dedicate their time and expertise to making sure patients feel heard, respected and valued. Not only are we providing the patient care our community members need, we are working dil-igently to develop trusting, long-term relationships that help to transform the patient care experience overall. This year, our dedicated team of more than 150 community volunteers and dentists ensured that 266 patients received free dental care valued at $93,000 during our annual Dr. Lisandra Soto Dental Day Of Caring. Over 2,100 children were immunized and prepared for school during our annual Back to School Bash, where we welcomed 40 community partners to help educate event attendees on critical local programs and resources within the community.As we kick off the start of this new year, I am excited to see how our team can continue to positively impact this community by combating diseases, saving lives and promoting preventative care to help individuals stay healthy. Dear Community Members, It has been a pleasure to serve as the Chair of FHC’s Board of Directors for 2023. As Chair, I have had the honor of working with some of the brightest minds and most compassionate team members to provide excellent care to our patients. Thanks to the daily contributions of our incredible staff and our dedicated community partners, we have been able to make great strides in offering our patients the care they need.In 2023, the board outlined a set of strategic goals aimed at enhancing overall organizational performance. These objectives include the commitment to providing an outstanding patient experience, striving for patient-centered clinical excellence, and achieving financial strength and sustainability. To achieve these goals, we focused on optimizing patient interactions and using clinical measures adopted by Federally Qualified Health Centers (FQHCs) all over Michigan. Under the leadership of our esteemed President/CEO, Denise Crawford, FHC was able to demonstrate improved health outcomes and obtain improved patient-experience metrics. We are excited to build upon this momentum in 2024 by working seamlessly alongside our incredible FHC staff and community partners. We cannot express enough our sincerest gratitude for the work of our FHC staff. Their invaluable investment of effort, time, passion and commitment is what ensures that we are able to accomplish our goals and further our mission of providing quality care to every resident in Kalamazoo. Thanks to the hard work of each of our team members, we are able to share these 2023 highlights with you. Quality ReportOur team at FHC takes pride in providing quality care to our patients by:165,356PATIENTCENTERED VISITS TOTALEnsuring our diabetic patients receive the care they need to maintain their vision via:• On-site appointments using a RetinaVue camera to screen for retinopathy.• Eye specialists referrals when potential retinal problems are detected.• Empowering patients to make lifestyle choices to better manage and understand their chronic conditions and protect their vision.Prioritizing health education and preventive care planning resulting in 1,160 patients completing a mammogram this year.Partnering with the American Heart Association to improve the accuracy of blood pressure readings during visits.Enrolling patients in remote monitoring for blood pressure using bluetooth enabled devices that allow FHC medical staff to review blood pressure values and make medication adjustments as needed.

Since 1971, Family Health Center’s mission has been to ensure that ALL members of the community have access to quality, comprehensive, patient-centered health care. Together, we provide this care to Kalamazoo residents regardless of age, gender, race, color, national origin, size, religion, culture, language, sexual identity, physical or mental ability, gender identity, insurance status, housing status, employment status, or ability to pay. FHC had the pleasure of hosting its 3rd Annual Dr. Lisandra Soto Dental Day of Caring in March this year. This event, which brings together a host of dedicated dentists, hygienists and dental assistants committed to providing quality care to community members, was a success this year. As part of the commitment FHC made when establishing the event in 2019, staff and volunteers were able to provide free dental care to hundreds of adults in our community. The 17th Annual Back to School Bash (B2SB) came and went with fun, games and memories we will never forget! FHC was pleased to host our largest and most highly anticipated event of the year, the B2SB. This exciting event provides our team an opportunity to ensure that all children in our community are prepared to start the school year with the supplies and resources they need to succeed. Each year, we offer immunizations, well-child exams, backpacks filled with school and hygiene supplies, and helpful informational tools to children and their parents. This year, attendees enjoyed carnival rides, games, food, a petting zoo and fun prizes. FHC provided immunizations to more than 2,100 children, nearly 1,500 well-child exams and gave away 1,500 backpacks. We could not have imagined that since the creation of the event in 2007, we would be able to serve more than 20,000 children.
