Getreuer, DDS President, Board of Directors Dear Friends of SilverSource, As I reflect on another transformative year at SilverSource, I am thrilled to share this Community Impact Report and highlight our success stories and initiatives to improve the lives of seniors and their families. Strategic Investments & Milestones The big news this year is our new space! We simply ran out of offices for our professional social work team. Our new location offers more room for our team, expanded programming, a family counseling space, an expanded food pantry, and a Lifelong Learning Lab. Plus, it has better parking for visitors! We are grateful for funding for another wheel-chair van, allowing us to meet the 23% increase in requests for wheelchair rides to medical appointments. Transportation remains the second biggest challenge facing seniors, after affordable housing. We are committed to helping to remove the cost of transportation as a barrier to healthcare. • Our educational programming includes a partnership with UCONN Extension services, who provide nutrition education for seniors in our classroom. • The "Wine Women and Dementia" program at the Fergusson Library brought the community together with caregiver support professionals as well as those interested in ongoing dementia care for their loved ones. • Our partnership with Protective Services for the Elderly division of the State Department of Social Services taps our nimble professional team on cases of elder abuse and neglect to provide critical resources quickly across towns in Fairfield County. All these advancements are made possible by the unwavering support of you-our donors, partners, and volunteers. Meet Patricia icia after es. AT A GLANCE Patricia is a recently widowed 79-year-old woman with numerous medical issues. She has only one son, Chris, who lives in California. CHALLENGES In addition to cardiovascular issues, she has diabetes, thyroid disease, and high blood pressure. The loss of her husband has not only left Patricia with financial challenges due to the loss of his Social Security income but has led to increased isolation and depression. Patricia needs a knee replacement and has mobility issues that require her to use a walker. SOLUTIONS SilverSource helped Patricia navigate her transition to live with her son in California. While her son made arrangements for mom on the west coast, SilverSource supported Patricia with assistance for medical prescriptions, heating oil and food. Patricia also received weekly Well-Check calls from our Volunteers to reduce isolation and connect her to our support team. Meet Grey Benefits Check Up Our social work team worked with Grey to ensure that he was getting all his veterans benefits as well as other support. AT A GLANCE Grey is a U.S. Army Veteran who lives alone. He has been struggling with medical issues and has a history of losing his balance and falling. He was in and out of the hospital frequently over several months. CHALLENGES Released from a post hospitalization rehabilitation center, Grey fell almost immediately after returning home. After another hospitalization and further therapeutic rehabilitation, he gained strength and learned how to transfer from his wheelchair to a bed. We assessed his apartment and determined what safeguards were needed for him before he returned home. SOLUTIONS SilverSource purchased and installed a wheelchair ramp, as well as grab bars in his bathroom. Additionally, SilverSource helped with rent, energy assistance, SNAP benefits, and food. These steps helped ensure a positive outcome for him allowing him to live at home safely. SilverSource partners with volunteers at the Veterans Resource Center to support his ongoing home care and safety needs. AT A GLANCE Pedro is a 65-year-old man born in the Dominican Republic who came to the U.S. in 1990 to play professional baseball. A kind and cheerful man, Pedro can laugh at the little things in life and inspires others while he sings and plays the piano at his church. CHALLENGES Pedro suffers from loss of sight and has been diagnosed with retinitis pigmentosa, an inherited eye disease. While it has been a huge challenge in his life, it does not prevent him from living a full life. SOLUTIONS SilverSource helped Pedro by paying for his prescriptions and dental work. He also receives weekly food deliveries and transportation to medical appointments. Additionally, SilverSource provided Pedro with a tablet computer, which has become his guide and his eyes, allowing him to grocery shop and cross the street safely. Food & Nutrition Staff and volunteers prepare and deliver weekly groceries that meet the need of a client's dietary restrictions, such as diabetes and kidney disease. 354 Seniors received healthy food support 297 Seniors received support for healthcare* 5,533 Rides to medical appointments *Healthcare needs include hearing, vison and dental SilverSource saves clients an average of $3,000 in home care costs. Meet Didi AT A GLANCE Didi is a 71-year-old woman who was referred to us by Protective Services for the Elderly due to elder abuse, fraud and neglect. She is disabled, suffers from memory loss and was found abandoned in her home with no food. CHALLENGES Didi has a brain injury resulting from a stroke. She was deprived of food and care by a family member and was left alone for extended periods of time. In addition to being neglected, the family member took advantage and depleted her life savings. SOLUTIONS SilverSource worked with Connecticut Protective Services for the Elderly (PSE) to help Didi remain in her home with the support of a qualified caregiver. SilverSource also helped Didi by providing meals and food for herself and her cat while collaborating with Stamford Health and the court system on her behalf. Navigating Uncertainty SilverSource is committed to serving residents of lower Fairfield County by delivering a wide range of programs and support services to improve the health and well-being of seniors. We provide more than 50 individualized services along with emergency financial assistance to help cover essential expenses, enabling them to keep their independence and quality of life despite financial challenges. Housing and Health-A Measure of Wellbeing The rising cost of housing and the lack of affordable senior housing continues to pressure seniors who hope to remain in the community. Seniors who come to us for assistance are paying an average of 50% of their limited income on rent. This forces seniors to make challenging decisions about paying for food, healthcare or shelter. SilverSource helped 124 older people stay in their homes over the past year. Access to Healthcare SilverSource filled 1,407 prescriptions and provided a total of 5,533 rides to and from medical appointments. Additionally, we helped 297 seniors with financial assistance for healthcare, including hearing, vision, and dental. Our Ride to Wellness program transports older adults to and from medical appointments free of charge, removing the cost of transportation as a barrier to healthcare. Combating Food Insecurity SilverSource ensures our most vulnerable seniors who are economically challenged and home-bound due to medical conditions or disabilities receive essential financial aid and care management. Our volunteers shop and deliver food weekly, providing needed supplies and a welcome visit. In addition, our expanded food pantry offers food, adult diapers, soap, and other personal care and hygiene products. Staying Connected in Person and Online Our Outreach program provided more than 8,557 visits. Our new space includes a Lifelong Learning Lab so we can continue to bridge the digital divide for seniors. Programs will help seniors navigate the internet safely, preparing them to connect with friends and loved ones online, access community and state resources, as well as medical records and appointments. Wine, Women & Dementia Film Event Focus on Caregiver Support Partnering with the Ferguson Library in Stamford, SilverSource hosted a free screening of the award-winning documentary Wine, Women & Dementia. We brought filmmaker Kitty Norton to Stamford to share her personal story of being a caregiver to her mom who suffered from dementia. After the screening, the audience was able to connect with Kitty, other caregivers, our partner caregiving professionals and SilverSource social workers to share information and resources. Healthy Eating 101 SilverSource kicked off a new partnership program with UCONN's Extension Program to provide an interactive series of healthy eating and cooking sessions for seniors. The first one, Savory Soups, was held in January. The UCONN Extension instructors, who taught in both in English and Spanish, shared how to prepare healthy soups and make smart ingredient choices. Seniors learned new ways to add flavor without salt or cream and many have been willing to try foods that are totally unfamiliar to them ... even better, these sessions serve to break down barriers - offering a shared, hands-on experience across cultural backgrounds! As we age, our nutrition needs change and many older adults don't eat as well as they should. Caring Connections Reducing isolation while keeping older residents connected to their community is a collaborative effort among our social work staff and caring volunteers. Through our weekly grocery program, home visits, Well-Check call program, holiday and corporate volunteer programs we are making more than 10,000 caring connections each year.
