OUR PROGRAMS COMMUNITY HEALTH Only 23 of the entire population of Haiti including just 5 of the rural population has access to high quality primary care despite over 90 living within 5km of a health facility Our Community Health Workers visit households to follow up with patients convene weekly health screenings and promote healthy behaviors nutrition and vaccination coverage through education 24 123 PATIENTS REACHED THROUGH COMMUNITY HEALTH WORKERS MATERNAL HEALTH According to the National Institutes of Health Haiti has the highest maternal and neonatal mortality rates in the Western Hemisphere Our Maternal Health Program offers mothers consultations by an OB GYN midwife nurse two ultrasounds all necessary lab tests medications vitamins birth planning and counseling 2 952 PRE NATAL VISITS Mothers

SEXUAL REPRODUCTIVE HEALTH EDUCATION FOR ADOLESCENTS We offer peer cohort based education to girls and boys about anatomy contraception gender based violence sexually transmitted diseases and consent In 2022 we welcomed over 438 adolescents into the 20 week long program FAMILY PLANNING We provide unbiased family planning counseling empowering patients to make the best decisions for their reproductive health We provide short term and long term family planning methods PEDIATRIC CARE In Haiti 6 in 10 children are not fully vaccinated Pneumonia accounts for 40 of child mortality in children ages 1 5 The need for high quality affordable health services for children in Haiti is urgent C2C clinics care for over 15 000 children annually providing vaccinations and screenings for pneumonia anemia and other illnesses CARE2COMMUNITIES ORG 8 502 CHILDREN UNDER FIVE SEEN FOR THE FIRST TIME 2022 ANNUAL REPORT

LOOKING AHEAD We are committed to increasing the reach of our healthcare services through operational and programmatic expansion while continuing to evaluate the impact of our core primary care service model on the health outcomes of the patients we serve Over the next year we plan to tackle the pediatric immunization gap through health education programs clinician patient counseling and utilizing community health workers to reach children in their homes expand our maternal health program by increasing the number of days we offer antenatal consultation implementing the program in more clinics and leveraging our community health nurses adopt new interventions to meet people in their homes and eliminate barriers to care like high transportation costs by expanding our community health program We believe that public private partnership in healthcare is the most effective sustainable way to strengthen healthcare in developing countries Our model harnesses the assets of the public sector leveraging medical expertise and systemic capabilities and integrates best practices and innovation from the private sector to deliver best in class sustainable healthcare services in one of the world s most challenging health markets As we look globally at healthcare needs and opportunities C2C is dedicated to sharing information about the success of our operations and replicable business model
