FOOD ADDICTION AND OBESITY BY JEREMY CLORFENE PH D Provided by Robard Corporation Starting in the 1970s the United States population has consistently gained weight and that trend does not appear to be stopping Two thirds of our nation is considered overweight and more than one third of adults are considered obese 1 2 The impact on health productivity and overall well being has been enormous all of these have suffered due to obesity and poor health behaviors Mounting evidence supports the claim that food addiction exists Obesity expert Kelly D Brownell Dean of the Sanford School of Public Policy at Duke University has described our culture as a toxic food environment which is responsible for the rapid increase in obesity rates 3 These toxic foods are very tasty cheap and convenient Specifically highly processed carbohydrate sugar designed foods are the chosen source which is the direct cause of our nation s obesity epidemic 4 Specifically foods like pizza chocolate soda French fries chips and cookies are all foods not in their original food states and patients with obesity report eating these in excessive quantity and lack control in stopping What becomes apparent in treating patients with obesity is that the pattern of weight loss and weight regain and the inability to control their eating habits clinically presents like an addiction The I can t control it and I can t stop thinking about my favorite food reality gives enormous validity to the notion that what people are experiencing is in fact an addiction One researcher psychologist Ashley Gearhardt along with her colleagues William R Corbin and Kelly D Brownell developed a robust psychometric instrument The Yale Food Addiction Scale YAFS to assist in identifying those who are in fact struggling with food addiction 5 In fact the clinical presentation and symptom profile between substance abuse and food addiction is also well documented thus contributing even further to the legitimacy of food addiction as being a real phenomenon 6 7 The challenge clinicians have in treating obesity is that if a patient is diagnosed as having a food addiction or self identifies themselves as a food addict treatment strategies that don t specifically address their addiction run the risk of falling considerably short in their efficacy and success It would be equivalent to focusing on and treating depression in a person who is also diagnosed with alcoholism or focusing on treating heart disease while the patient continues to smoke In many cases the alcoholism is the cause of the depression and the smoking is the cause of the heart disease Concomitantly the food addiction is often the cause of the obesity In effect clinicians are treating both obesity and addiction Even though we all know treating addiction is difficult it would be clinically limiting to ignore avoid or tepidly address food addiction By definition addiction implies loss of control over a substance with significant negative life consequences The most accepted and proven treatment strategy for addiction requires abstinence 8 Moderation is not a treatment option That is learning how to partake or use the addicted substance reasonably and in moderation is contraindicated and self evident that it can t be done Treatment strategies that support patients with obesity in following a healthy meal plan while implementing focused interventions is critical for immediate and long term success Patients need help to refrain from eating their selected addicted food choices Patients who have been successful sustaining long term weight loss will tell us that they simply don t eat their addicted foods anymore No doubt this is not an easy task but an important goal to be discussed and focused on early in treatment Along with building an abstinence plan arming patients with time tested evidence based skills to manage their impulses such as recording their food and regularly exercise are necessary components of a comprehensive treatment strategy 9 Let s embrace this real complex food addiction phenomena by remaining compassionate persistent and patient so we can continue to serve our patients
