OUR HOPES |OUR STAFF NEWSLETTER | Issue 5 1 Our HOPES |Our Staff Newsletter Hoboken & Plainfield, New Jersey January, 2016 WELCOME TO OUR 51ST YEAR OF HOPES! On the Road to Healthier Homes By: Marisa Gerke Congratulations to HOPES for receiving the “On the Road for Healthier Homes” (ORHH) grant from the Horizon Foundation three years in a row! This grant aims to proactively improve health literacy and access among economically disadvantaged communities in Hudson County and Union County, New Jersey. Since 2012, “ORHH” has promoted health literacy through CEED services, SHIP counseling, chronic disease management workshops, and benefits enrollment support. Over the years new mobile services have been included, such as ACA Navigation, shopping and cooking classes, the Wellness Initiative for Seniors (WISE), and HIV/AIDS awareness. This comprehensive program model bundles all of HOPES’ health services to holistically address the needs of vulnerable populations and connects health seekers with information and assistance in places where they live and work. The program addresses the varying health needs of individuals through on-site counseling and instruction on topics such as managing chronic disease, coping with stress and anxiety, accessing health insurance, improving nutrition, locating providers and staying active.
