This Patient & Visitor Guide will assist you during your stay with us. It is the goal of Ness County Hospital to make sure you have a complete understanding of all policies and procedures. The staff is hopeful that this guide will be helpful in this process.

Visiting Hours & Guest Stay
Guests are welcome to visit your room between the hours of 8:00 a.m. and 8:00p.m. We ask that children under the age of 12 be accompanied by an adult. If a family member wishes to stay the night, please check with a staff member for accommodations and sleeping arrangements.

 

Telephone Calls
To make an outgoing local phone call from your room, first dial 9 and then enter the phone
number. For long-distance calls, you should use a calling card or call collect. To do so, dial 9 to get an outside line. If you need assistance, press your call button and a staff member will assist you.

Wi-Fi Internet Access
Free Wi-Fi is available to guests. Log onto “NCH_Guest”. A password is required, ask your nurse or at the front desk to get the current one.

Television & Activities
Located on your night stand you will find a list of television channels and activities available
during your stay with us. Activities can be requested by contacting the nurses station using your
call button. Guests are welcome to bring in activities as well.

 

 

Tobacco Use  

Ness County Hospital has one designated smoking area, located on the north side of the facility.  Any use of tobacco products and electronic smoking devices, other than in this area, is prohibited by patients, visitors, employees, physician, etc.  

 

General Information

Who you will meet
Your health and comfort are a top priority of the Ness County Hospital staff. During your stay you will encounter a number of staff members. You physician will direct your medical care and prescribe treatment. Registered nurses work closely with your physician to provide care for you.  Radiology technologists, physical therapists, nutritional services staff, housekeepers, and others may also visit your room during your stay.


Calling your nurse
If you need assistance or have questions at anytime during your stay, simply push the call button. When you push the call button, the hallway light above your room door turns on. A staff member at the nurses station they will come to your room.


Getting out of bed
For your safety, you should not get out of bed without help unless you have been told you may do so. Your bed is higher and probably narrower than the one you have at home. Please call the nurse if you need help getting out of the bed. See page 15 for more information on fall prevention.


Pain management

If you are in pain, please let a nurse know as soon as you feel discomfort. You can use this chart when talking with your nurse about the pain you are experiencing.

 

Your Stay with Us

As a patient at the NessCountyHospital, you have a right to:

 

  1. Be informed at the time of receiving care or before cessation of care of your rights as a patient at NessCountyHospital.  If in some instances the patient’s condition or age does not allow communication of the Patient Rights, these rights may be communicated to the patient’s representative.  As soon as the patient can be informed of his/her rights, the hospital will provide that information to the patient;
  1. Respectful care given by competent personnel;
  1.  Upon request, be given the name of your attending physician, the names of all other practitioners directly participating in your care and the names and functions of other health care persons having direct contact with you; 
  1. Be informed of your health status, the information necessary to make treatment decisions reflecting your wishes, being involved in care planning and to request a change in your physician or transfer to another health facility due to religious or other reasons; 
  1. Accept medical care, to refuse treatment to the extent permitted by state law and to be informed of the medical consequences of refusing treatment; 
  1. Formulate advance directives and appoint a surrogate to make health care decisions on the patient’s behalf to the extent permitted by law and to have hospital staff and practitioners who provide care in the hospital comply with these directives; 
  1. Assistance in obtaining consultation with another physician or practitioner at the patient’s request and own expense; 
  1. Hospital services without discrimination based upon race, color, religion, sex, national origin or source of payment; 
  1. (Or your legally designated representative) Review the records pertaining to your medical care and to have the information explained or interpreted as necessary, except when restricted by law; 
  1. Examine and receive a detailed explanation of your bill;

Your Rights as a Patient

 

  1. File a complaint.  If you have a problem with your care you may inform any staff member, address it on the Patient Satisfaction Survey given to you on admission or contact the Director of Nursing or the Hospital Administrator at 785-798-2291.  We will try to resolve the problem immediately, if possible and notify you of the results.  You may lodge a grievance with the State directly by calling 1-800-842-0078; 
  1. Have a family member or representative of your choice and your own physician notified promptly of your admission to the hospital;
  1. Personal privacy;
  1. Receive care in a safe setting and to be free from all forms of abuse or harassment;
  1. Expect confidential treatment of all records pertaining to your care, and may approve or refuse their release to any individual outside the facility, except in cases such as suspected abuse and public health hazards when reporting is permitted or required by law;
  1. Be free from restraints, of any form, that are not medically necessary or are used as a means of coercion, discipline, convenience or retaliation by staff;
  1. Be free from seclusion imposed as a means of coercion, discipline, convenience, or retaliation by staff;
  1. Associate and communicate privately with persons of your choice, and send and receive your personal mail unopened, unless contraindicated as documented in the medical record;
  1. Retain and use your personal clothing and possessions as space permits, unless to do so would infringe upon the right of other patients and unless medically contraindicated as documented in the medical record;
  1. If married, and both are patients in the facility, be permitted to share a room, unless medically contraindicated as documented in the medical record;

 

  1. Meet with, and participate in the activities of social, religious and community groups at your discretion unless medically contraindicated as documented in the medical record;

  1. Be fully informed upon request, an accounting of financial arrangements, facility damage responsibilities, self-retained money arrangements, and guest meal service; 
  1. Be fully informed of approval, maintenance, and upkeep of their personal electrical appliances brought into the facility; 
  1. Have visitors that are not limited, restricted or otherwise denied visitation based on race, color, national origin, sex, sexual orientation, gender identity, disability, age or religion; 
  1. Be informed of any specific alteration in visitation rights when specific clinical situations warrant; 
  1. Consent to have visitors of their choice and the right to restrict or withdraw consent for an individual to visit.

With rights, there are also responsibilities.  As a patient at the NessCountyHospital, you will be expected to:

 

  1. Be open and honest about everything that relates to your treatment here; 
  1. Make an effort to understand your health and ask your doctor or hospital staff members if you do not understand; 
  1. Be cooperative with your doctor and others in your treatment; 
  1. Follow hospital policies so that all patients and employees can be treated with respect; 
  1. Provide the necessary information for insurance claims and work with the hospital to make payment arrangements; 
  1. Be responsible for your own actions if you refuse treatment or do not follow instructions; 
  1. Respect property of other persons and of facility; 
  1. Be responsible for personal behavior on control of noise, smoke and visitors.  (Smoking is NOT allowed in the hospital facility); 
  1. Be aware that patient room doors must provide access to and from room at all times, therefore, locks, latches, bolts, chains or any other type of retainer on doors are prohibited; 
  1. Respect other patient’s rights to privacy by NOT repeating what you see or hear; 
  1. Assure that financial obligations for health care are fulfilled as promptly as possible; 
  1. Be aware that personal belongings may be laundered by patient’s family or person designated by the family, unless specific arrangements are made; 

Be aware that personal hygiene supplies (toothpaste, comb, brush, etc.) may be provided by the family or purchased at the hospital

Patient Responsibilities

Ness County Hospital Associates and Board of Directors are committed to ensuring that the highest quality of care and service is provided to patients/customers and their families. NCH has a formal Complaint/Grievance process.

 

• If you have a complaint or grievance, please notify staff present at the time to allow present staff the opportunity to resolve the complaint or grievance.

 

• If the complaint/grievance cannot be resolved to the patient (or patient representative’s) satisfaction, then ask to speak to the department’s supervisor.

 

• If the complaint/grievance is still not resolved to the patient (or patient representative’s) satisfaction, please ask to speak or write to the Risk Manager/Quality Coordinator.

 

• Grievance forms are available at the nurses' station, at the front desk, or from the Risk Manager.

 

• Written complaint or grievance forms can be mailed to the Risk Manager or CEO at 312 Custer Ness City, KS. 67560

 

• If you are a Medicare patient and the complaint/grievance is related to quality of care, a coverage decision, or you wish to appeal premature discharge, you may contact the KePro @ 5201 W. • Complaints/grievances regarding a patient’s care or issues of abuse or neglect may also be addressed to the Department of Health and Environment (regardless of age or insurance) by calling 1-800-842-0078.


You will be advised of the receipt of the grievance/complaint by the Risk Manager. Upon completion, you will be notified of the steps taken on your behalf to investigate the grievance,

Complaint/Grievance Resolution

The purpose of our Charity Care Policy is to provide guidelines for a fair and comprehensive system of distributing charity care to eligible patients who meet financial guidelines set forth by the current poverty level guidelines. All qualifying applicants will be eligible for a charitable write-off based on the current poverty guidelines. Eligibility will be determined after completing the following qualifications:

 


1) Complete a Financial Statement


2) Provide most recent tax return (if no tax return filed, will need previous three months bank statements)

 

3) Provide three months pay stubs from current employer

 

4) Provide non-eligibility letter from Medicaid

 

5) Provide loan denial for services. You must apply for a loan at the financial institution of your choice for payment of your hospital bill. If you do not qualify for the loan, we need a copy of the denial.

 

6) Any other documentation that supports financial need.

 

The above information needs to be returned to our office within 15 days from the date the information was given. All taxable and non-taxable income will be considered. Application for charity care should be completed when a patient does not have third-party payer coverage or will have a substantial personal payment responsibility and does not have the resources to make such payment. Charity care write-offs will be applicable only to amounts outstanding after payment has been received from all other sources.

 

Meals on Wheels, Intermediate Care services, medication and supply pick-ups, elective services and accounts that have been turned to collection are excluded from charity care consideration. The application process can take place at time of service or at a later date when it becomes evident that a patient is having difficulty making payment.

Charity Care Policy

Ness County Hospital is member of the Partnership for Patients, a nationwide effort to improve safety for patients in 3,700 hospitals across the country. Here are some ways you can work with our staff to improve your safety.

 

Preventable Readmissions, Your Risk:

People are sometimes admitted back to the hospital soon after their initial hospitalization. Our goal is to prevent these readmissions.

 

What You Can Do:

Make sure we have explained your medications and follow-up appointments so that you are sure you understand. Know the signs and symptoms of your condition and when you should consult your primary care physician or other provider. Do not hesitate to ask questions. This may prevent a
relapse or complication. 

 

Medication Safety, Your Risk:

Medications have proven to be very beneficial for treating illness and preventing disease. However, using multiple medications increases the risk for drug interaction or unwanted side effects. Our physicians, nurses, and pharmacists work together to review your medications and ensure that they are safe for you.


What You Can Do:

Keep an accurate and up-to-date list of your medications and dosages that you take. Be sure to include vitamins, herbal supplements and over-the-counter products. Make sure you understand why you are taking each medication and ask your doctor if you are unsure.

 


Preventing Infections, Your Risk:

Some patients have a medical reason to have a catheter inserted into the bladder. These catheters increase the risk of developing a urinary tract infection (UTI). For your safety, we have guidelines to use catheters only when necessary and to remove them as soon as possible. We follow proven methods to ensure we provide appropriate insertion and catheter care.

 

What You Can Do:

If you or a loved one need a urinary catheter, ask why and what the plan is to remove it.

 

Preventing Falls, Your Risk:

Falls in hospitals are the leading cause of injury for patients over the age of 65. Some illnesses and medications may make patients light-headed and dizzy, placing them at risk for fall. For your safety, we assess your risk of falling. Our nurses have special training in safety measures to prevent harm if a fall does occur.

 

What You Can Do:

Tell us if you have had a recent fall or if you have any unsteadiness or weakness when moving around or walking. Don’t be embarrassed to ask for help. Use your call button and always ask for help if you need to get out of bed to use the bathroom, especially at night. Ask what is being done to help prevent you or a loved one from a fall and injury.

 

Preventing Blood Clots, Your Risk:

Blood clots in the legs and lungs - called Venous Thromboembolism (VTE) or Deep Vein Thrombosis (DVT) - are a significant cause of harm to patients in hospitals. Some medical conditions and physical limitations may place patients at a higher risk of blood clots.

 

What You Can Do:

If you are hospitalized, ask your doctor if you are at risk for blood clots and

what will be done to help prevent this from occurring.

 

 

 

 

Staying Safe in the Hospital

Some patients need to be isolated from other patients, hospital staff, and visitors during their hospital stay. This is because they are carrying harmful micro-organisms that have the potential of infecting others. In hospitals, where there are many sick people, preventing the spread of these micro-organisms becomes very important. This is because an already ill person may have a harder time fighting this new infection.


Perhaps your doctor has ordered that you need isolation precautions. This is not intended to be a punishment. The doctor and staff are only trying to protect you and others. However, being isolated may bring about feelings of loneliness or discouragement. Learning more about contact isolation may help you cope better with your situation.

 

Preventing The Spread of Infection

It is important that you understand these precautions so you know what is required. Please talk to your nurse or doctor if you have questions or concerns about these precautions. There are varying reasons why isolation precautions are utilized in health care facilities, such as:

 

• Transfer from one facility to another (i.e. nursing home to hospital);

 

• Signs or symptoms of suspected contagious illness (i.e. flu-like or respiratory symptoms, rash);

 

• Diagnosis of contagious illness (i.e. flu, chicken pox, shingles, etc.);

 

• To protect those with weakened immune systems (i.e. someone receiving chemotherapy).

 

For your protection and the protection of others in the facility, there are certain procedures that need to be followed by both hospital staff and visitors. Speak with your health care team if you have questions. For further infection prevention information, ask to speak with the Health & Safety

Isolation Precautions for Visitors & Patients

Hand hygiene is very important to your health. During your stay, it is even more important as it prevents the spread of infections. Things to look for during your stay at Ness County Hospital:

 

• Is the medical staff washing or sanitizing their hands upon entering & exiting your room?

 

• Are they washing or sanitizing their hands when switching between duties in your care such as administering meds, emptying catheter bags, drawing blood, or otherwise touching you?

 

• When other staff members enter your room, are they also washing or sanitizing their hands
(i.e. physical therapy, occupational therapy, social workers, admissions staff, etc.)?

 

 

Please note that both soap & water or the foaming hand sanitizer are safe and acceptable forms of hand cleansing.

Hand Hygiene Observations

In order to ensure that your stay at Ness County Hospital is as safe as possible and to decrease your chance of experiencing an accidental fall during this hospitalization, please review the following tips:

 

Tips for patients

 

• If you have been instructed to, do not hesitate to call a nurse for assistance when you need to get out of bed, especially if you are feeling weak or dizzy (some medications may have side effects that make you feel weak or dizzy).

 

• Sit at the side of the bed for a few minutes before getting up.

 

• If you use a walker, cane, or wheelchair at home, let the nurse know or bring in yours to use while hospitalized. These must stay within reach and make sure you remember to use them.

 

• Wear properly fitting shoes or slippers when walking.

 

• Pay close attention to any tubing, such as oxygen, catheters, etc. that may interfere with walking.

 

• The side rail at the bottom of the bed towards the door will be kept down unless you request that it be kept up. Call the nurse for assistance if needing out of bed. Do not climb over or through the rails.

 

• The bedside tables are not for support as they have wheels and could roll away from you.

 

• If there are any spills or wet areas in your room, notify the nurse immediately so that it may be cleaned up.

 

• A bed alarm may be used to remind you to call for help when getting out of bed.

 

Tips for family members or visitors

 

• Please make sure the nurse call light and the bed stand is within reach of the patient before leaving the room (items such as the phone and Kleenex should also be within the patient’s reach).

 

• If you notice that some medications have caused weakness or dizziness, please notify the nurse.

 

• Consider staying with your family member if they are at a high risk of falling or are confused.

 

• If you need to leave and the patient is confused or disoriented, notify the nurse so appropriate safety measures can be taken.

 

• Reinforce to the patient the need to ask the nurse for help when getting up.

Tips for Preventing Falls

Services Not Covered by Medicare

 

Certain services provided by Ness County Hospital are not covered by the Medicare program and therefore are the responsibility of the patient. Medicare requires this hospital to present the patient with written notice that these items will not be covered:

 

• Private room requested by patient when not medically necessary.

 

• Take home medical supplies and drugs.

 

• Routine B12 vitamin shot, Mycifradin, Laetrile and L-Dopa.

 

• Personal items (toothbrush & paste, comb, shower cap, disposable razor, denture cup, guest trays, etc.)

 

• Some oral medications (such as Tylenol, Ibuprofen, vitamins, etc.)

 

Observation Status Admissions

 

If you were admitted to the hospital under “Observation Status”, this is considered an outpatient service and is only covered under Medicare Part B. If you have Medicare, but only Part A benefits, this stay will not be covered by Medicare or your supplemental co-insurance policy. Observation Status means that you have not qualified for a regular Acute Care Stay and that your doctor is observing your condition to see if you meet the need to be admitted to the hospital.

Planning Your Discharge


Your physician will determine when you are ready to leave the hospital. Prior to leaving, your medical team will give you instructions about post-hospital care. Topic areas may include medications, diet, activities, and personal care. If you have any questions about post hospital care, please feel free to ask. Your understanding is very important. The discharge planner can meet with you and/or your family to help plan your care following your hospital visit. After you leave the hospital, there are services available to assist you while you recover.

 

Below is a list of possible options:

 

        • Outpatient services

        • Meals On Wheels
        • Home Emergency Alert System
        • Medication assistance
        • Bathing & personal care assistance
        • Home Health

 

Some of these services may be paid for by insurance, however some are not. If you feel any of these services would help you or your loved one with recovery, please let your physician or discharge planner know. The nerses staff can also help you with admission to a rehabilitation center or skilled nursing facility, access to needed equipment, and/or protective services.

 

Your Hospital Bill


After your discharge from the hospital, the billing department will bill your insurance company. Any portion of your bill not covered by your insurance company will be your responsibility. If you need a copy of your charges or have any questions regarding your bill, please contact the Business Office by calling 785-798-2291

 


Charity Care (see page 10)

 

This is available to patients who meet financial guidelines set forth by the current poverty level guidelines. If a patient is eligible for Charity Care, a write-off for medical expenses may be given. For more information on Charity Care, please contact the Business Office.

When You Are Ready to Leave

You can access your medical information online through InteliChart Patient Portal. Simply visit www.nchospital.org and click on the Patient Portal logo. To sign up for Patient Portal, follow these instructions:


1) Ask the hospital registration staff about signing up.


2) You will need to provide your email address and a PIN will be assigned to you.

 

3) You will receive a welcome email. Your email will contain a link to click on.

 

4) Once you click on that link, enter your PIN and Last Name when prompted.

 

5) Answer the questions that follow.

 

6) You will be instructed to check for a confirmation email. Click on the link provided inside that email.

 

7) Once you have successfully logged into the Patient Portal, you will be able to view medical records from your recent hospital stay. Go to the “My Health Record” tab and drop down to “Documents”.
