Editor s Message Journal IDA Good Help is Hard to Find Dr Jack Drone We are finishing our basement We have made the transition through concrete walls and floors to painted drywall and ample light The carpenter who is doing the work was having a hard time getting the flooring completed with his help I had assisted on laying carpet squares in my office so I volunteered my Friday off to install the carpet squares with one of his hired men I believed that this would be a piece of cake I had 1 500 square feet of clean concrete to work with I would apply the carpet adhesive ahead of me let it dry and then lay the carpet squares Mike was my assistant in the process The carpet tiles were 20 inch squares On the down side was an arrow The arrows were to be rotated from 12 o clock to 3 o clock to 6 o clock then to 9 o clock and then repeat The directions stated for the next tile to be rotated 90 degrees to vary the pattern and make the fit as good as it could be Simple right I troweled the first quarter of the basement with adhesive and I was ready to lay the carpet squares I placed the first facing 12 o clock then 3 o clock and then 6 o clock Mike was starting his own row right behind mine but they did not look the same as the ones I was placing I stopped and asked him if he was laying them at 12 3 6 and 9 o clock He said he was unsure of what I meant As my irritability was growing he told me that he usually looked on his phone for the time He really did not know what I meant by a traditional clock and rotating each tile 90 degrees I really felt for him He said he quit school as soon as he could and he liked rough carpentry framing and insulating far more than finish carpentry We agreed to redefine the clock approach to up right down and left He laid a row of tile and then said he needed a cigarette break Eight hours a lunch break and five cigarette breaks later the crew left I finished the floor four hours later with no lunch or smoke breaks I am truly not complaining about that day at all I had fun getting every tile to look exactly like I wanted it to and when my sons got home from basketball practice they were absolutely excited that their gaming space could be reclaimed My point lies in that in the middle of any dental procedures that I am completing the assistants in our office would never get up to have a cigarette and leave me alone to work We have worked very hard to educate all personnel in the office to know the procedure well enough to do it in the dark Working that day with someone less committed to my goals made me appreciate the ladies that I work with daily We have a great Annual Session in French Lick this year It is a great chance to educate and team build I hope that the membership takes the opportunity to attend and educate those we work closely with daily I hope to see you all there Journal Editor Dr Jack Drone is a general dentist practicing in Rensselaer IN He can be reached at jack hillcrestdental net Treat Your Staff to a Trip to French Lick See Page 30 for Annual Session CE Opportunities Perfect for Your Entire Team Spring 2017 Volume 96 Issue 2 Journal of the Indiana Dental Association 5

Journal IDA President s Message President s Message French Lick is not what it s supposed to be Dr Jay Asdell The 159th meeting of the Indiana Dental Association will be held June 7 10 2017 in French Lick IN This will be the third time that the IDA has ventured southward to the rolling hills of southern Indiana If you ve never visited this part of our beautiful state you are definitely missing something The name French Lick is not just a catchy name The history can be traced to the area s salt licks and mineral springs which were Indian hunting grounds for hundreds of years before George Rogers Clark discovered the salt licks in 1778 on his way to Vincennes Before there was a stock law in Indiana farmers let their livestock run wild and many would wander for miles to drink from the mineral springs and spend time at the salt lick This land was sold to Dr William A Bowles in 1829 Dr Bowles was a physician and established the first health resort some time before 1840 Then in 1841 he built the first hotel in French Lick at the site of the current French Lick Hotel This hotel remained unchanged until 1881 Dr Bowles entered into a lease with Dr John Lane for the land just north of the French Lick Hotel In 1845 Dr Lane was responsible for building the Mile Lick Hotel which eventually became the West Baden Springs Hotel After five years Lane was able to purchase the West Baden Springs Hotel from Dr Bowles There became two resorts in the area about a mile apart Dr Bowles went on to become a Captain with an Indiana regiment in the Mexican War He became friends with the Union soldier Jefferson Davis who became the president of the Confederacy Bowles became a leader in the Knights of the Golden Circle which was a pro South organization He was arrested for treason but was eventually pardoned by President Lincoln and died in French Lick The French Lick Hotel changed hands several times in the years following the Civil War until 1880 when the hotel was purchased at a sheriff s sale by Hiram Wells and James Andrews Wells and Andrews made many improvements to the hotel and changed it from a summer resort to a year round resort after adding a general power plant A disastrous fire in 1897 burned the main building and employee quarters The hotel was then sold to a syndicate from Louisville headed by Capt John Howard and his son Dr John Howard In 1901 the property was sold to Thomas Taggart a 6 businessman from Indianapolis Taggart expanded the facility over the next 20 years adding three six story wings and facing it with the trademark buff colored French Lick brick He ordered construction of the first area golf course in 1907 He added a second course in 1920 designed by the famed Donald Ross The Ross Course hosted the 1924 PGA Championship which was won by Walter Hagen Taggart became the mayor of Indianapolis a US Senator and the national chairman of the Democratic Party Taggart s marketing savvy was instrumental in bringing celebrities such as Joe and Rose Kennedy FDR Harry Truman Richard Nixon Ronald Reagan Bing Crosby Bob Hope Howard Hughes and Lana Turner Hoagy Carmichael Duke Ellington Louie Armstrong and many more to French Lick In the elegant Pluto Bar guests of the hotel could order a dose of Pluto Water the hotel s special brand of mineral water which was bottled on site and sold around the world My grandfather who was a dentist in World War I was walking down a street in Paris when he noticed a bottle of Pluto Water from French Lick in the shopfront window of a French store He said that it made him homesick Production of Pluto Water stopped in 1971 after lithium one of the elements in Pluto Water was classified as a controlled substance In 1884 ownership of the West Baden Springs Hotel was passed to several men who passed control to Lee W Sinclair of Salem IN Sinclair metamorphosed West Baden into a sophisticated resort adding a casino a two deck pony and bicycle track an opera house and a full size baseball field However in 1901 a fire destroyed the original West Baden Springs Hotel which allowed Sinclair to build the hotel of his dreams It was a circular building topped by an immense dome and decorated like the grandest hotels in Europe The new West Baden Springs Hotel opened in June of 1902 and was billed as The Eighth Wonder of the World It featured an atrium 200 feet across by 100 feet tall which Journal of the Indiana Dental Association Spring 2017 Volume 96 Issue 2

President s Message remained the world s largest free span dome until the Houston Astrodome was built in the 1960s As in French Lick the wealthy and famous including many from the gangster world came to take in the southern Indiana scenery and gamble often booking lengthy stays West Baden had its own mineral water called Sprudel Water which reportedly cured or improved all diseases and afflictions Sinclair died in 1916 and his daughter Lillian took over the operation of the hotel She completed a massive renovation of the hotel and then sold it to Ed Ballard who also owned the Grand Hotel on Mackinac Island for a million dollars in 1923 When the stock market crashed in 1929 the hotel emptied of guests almost overnight Ballard closed the hotel in 1932 and sold it to the Jesuits in 1934 The Jesuits dismantled the four Moorish towers and kept it as a seminary until 1964 It was purchased in 1966 and became the Northwood Institute a private college until 1983 Even though the hotel was listed as a National Historic Landmark in 1974 it was closed to the public in 1989 for safety reasons In 1991 there was a collapse of an exterior wall due to a buildup of ice and water on the roof Indiana Landmarks the country s largest state wide historic preservation organization worked toward improving the hotel to make it attractive for redevelopment In 1992 they spent 140 000 to stabilize the Journal IDA property In 1994 the Cook Group a global medical device company from Bloomington stepped in to preserve both the French Lick Hotel and the West Baden Springs Hotel Each hotel and the associated grounds were the subject of a multimillion dollar renovation with the focus of returning them to the splendor of the days of Sinclair and uniting them as one resort In 2006 the French Lick Springs Hotel reopened with a new addition the French Lick Casino The original 1917 Hill Course was restored and reopened as the Donald Ross Course The West Baden Springs Hotel restoration was completed in 2007 and the French Lick Resort was born If you ve never visited this Oasis of Fun in southern Indiana as stated previously you are really missing something If you have been there there is nothing better than a return visit The IDA meeting is packed with lots of fun and knowledge There will be a live band on each night and the historic North Central Hospitality Suite will be available for all participants Make it a point to get here IDA President Dr Jay Asdell is an oral surgeon practicing in South Bend IN He can be reached at hoosieroms gmail com You re Invited Dr Jay Asdell s Annual Session Hospitality Suite Meet Mingle with Fellow IDA Members Leadership Enjoy Light Appetizers Adult Beverages Wednesday June 7 2017 Thursday June 8 2017 Friday June 9 2017 7 00 11 00 p m 2 00 4 00 p m 3 00 5 00 p m Look for The Presidential Hospitality Suite Spring 2017 Volume 96 Issue 2 Journal of the Indiana Dental Association 7

Journal IDA Executive Director s Message Executive Director s Message How I Lost My Prostate Mr Douglas M Bush The story actually begins over three years ago I was seeing my physician for my physical and the associated blood work I got my regular lecture on the need to watch my cholesterol He also noted that my PSA prostate specific antigen a protein that can signal the presence of cancer was borderline high and that it merited watching Another year goes by Same tests Same lecture This time my doctor commented that the PSA was high and I should see a urologist He hastily added that everything felt normal during the infamous digital exam so most likely there was nothing to worry about I chose to latch onto the nothing to worry about comment and ignored the recommended urologist visit Another 18 months pass I applied for IDA Long Term Care insurance and the underwriters noticed the medical record reference to the urologist referral that never happened They refused to issue a policy until I got another PSA test More annoyed than concerned I scheduled another test My PSA level had tripled This time no hem hawing I set the appointment with a urologist Lesson 1 It s fine to be an optimist but don t ignore unfavorable medical test results If the physician recommends you see a specialist see a specialist He was a nice young man who looked fairly fresh out of residency I ll call him Dr Young Guy Dr Young Guy reviewed my numbers and offered reassurance There was a good chance all was fine but he recommended a needle biopsy to find out for sure By this time I had done a bit of reading on the subject and I knew a biopsy was not a pleasant experience I asked Exactly how are you going to do this test His explanation began Well God didn t put the prostate where it is easy to get to With that he described the procedure You are dentists and you are accustomed to poking needles where they don t belong But even taking that into account I m going to spare you the details of a prostate biopsy I will say that if you were ever given a voodoo doll of your worst enemy then given a 8 needle and told you could poke your adversary anywhere you want you would never consider poking where this doctor intended to go Common decency would not allow it The morning of the biopsy I was escorted into the treatment room by a nurse who seemed friendly enough but first impressions can be deceiving I was told to get undressed I then climbed onto the exam table and covered myself with a paper sheet I m told that in dental school you are taught how to hide the syringe from your patients Why create unnecessary anxiety if by sleight of hand you can keep the needle out of sight My nurse must have skipped needle concealment class She stood in front of me while she drew up the injection I honestly didn t know they made syringes that big and I futilely hoped that it was for a horse or elephant in an adjoining exam room But no such luck She explained it was an antibiotic to reduce the chance of infection from the biopsy Lesson 2 Continue to hide syringes from your patients It s the right thing to do After the attack I was told to lie on my side and wait for the doctor Dr Young Guy came in and cheerfully greeted me He then explained that the front office had overlooked some paperwork I needed to sign While I continued to whimper from the injection he held the clipboard and I consented to something Lesson 3 Don t ask your patients to sign consent forms while they are naked Much has been written about prostate biopsies including a fair number of horror stories I can honestly say it was not as bad as I feared Certainly unpleasant but in terms of pain not nearly as bad as Nurse Ratched s antibiotic injection After Journal of the Indiana Dental Association Spring 2017 Volume 96 Issue 2

Executive Director s Message 20 minutes of probing prodding and poking I got dressed and hobbled down the hallway and out to my car I briefly considered returning to the office but after finding out how uncomfortable the car seat was I decided I had earned an afternoon off The results came back about a week later The lady who called said There are some signs of cancer cells and you need to come back in for a consultation with the doctor As I suspected that was code for You have cancer Journal IDA In addition to reassurance Jim gave me another gift for which I will forever be grateful A referral to his surgeon When I met with Jim s doctor Dr Old Guy and asked how many times he had performed the surgery he replied I have no idea Well over a thousand I ve assisted with that many more Are you good at it I asked I m very good he replied Outside of family one of first people I spoke to about my diagnosis was Dr Jim Shupe a pediatric dentist from Fort Wayne I knew that Jim had had prostate surgery the previous year and when I ran into him at an IDA social event I pulled him into a corner to give him my news The first words out of Jim s mouth Don t be afraid of the surgery That s significant because I was indeed afraid of the surgery There is no absolute medical protocol for prostate cancer treatment The three most common alternatives are Dr Old Guy was my man Lesson 4 When faced with tough decisions seek counsel from friends you know and trust The surgery was on Election Day November 8 Alice and I arrived at the hospital before they opened security had to let us in the main entrance When the registration personnel showed up I got additional encouragement 1 Watch and wait In most cases prostate cancer is slow growing which means some patients can wait and observe the rate at which their cancer progresses before receiving treatment The downside you can wait too long If it is the rare fast growing cancer or if it metastasizes beyond the prostate the condition can quickly become much more serious Oh I see your surgery is being done by Dr Old Guy his associate and his anesthesiologist No wonder you are here early they like to start early They are very good We call the three of them the dream team 2 Radiation All forms of prostate cancer treatment have unpleasant inconvenient and embarrassing side effects Radiation is often successful at killing the cancer but it also damages surrounding muscle and nerves That produces negative side effects that can manifest months or years after the treatment ends I was receiving laparoscopic robotic surgery While I ve found that most folks in the medical dental field are familiar with Da Vinci Robotic Surgery it sounded like science fiction to me While the robot does the cutting and snipping the surgeon is on the other side of the room manipulating controls from inside what appears to be an NFL instant replay booth The precision of the robot is amazing There are online photos of it removing the peel from a grape then suturing 3 Surgery It s called a radical prostatectomy The prostate and surrounding tissue is removed Sensitive muscles and nerves are almost always damaged producing side effects that begin immediately but that can improve over time Maybe she says that about all of the surgical teams but I didn t care It was still good to hear I decided that if my choices were immediate side effects that improved over time vs no immediate side effects but problems in the future I d opt for the former So I met again with Dr Young Guy to discuss surgery When I asked how many times he had performed the procedure he told me around 50 No offense I said But I want someone with more experience To his credit Dr Young guy replied None taken I understand A Da Vinci Surgical Robot making a ham and cheese sandwich photo courtesy of Gomerblog com Spring 2017 Volume 96 Issue 2 Journal of the Indiana Dental Association 9

Journal IDA Executive Director s Message it back into place Of course the night before the surgery I made the mistake of watching the Terminator movie sequel where robots become self aware and take over the world I had nightmares of a robot going rogue during the middle of my procedure As I was rolled into the operating room I asked the nurse if I could see my robot It s right there she replied as she pointed The robot sat in the corner waiting on me It appeared safe It resembled an oversized Star Wars R2 unit only with extra arms The anesthesiologist put a mask over my face and told me to breathe normally The next thing I remember they were wheeling me into my room where Alice was waiting me the day I got home from the hospital Glad you are home Now get your ass out of bed and start walking That s the encouraging chair side disposition that endears Neal to his patients At the time of this writing I m three months post surgery and feeling great Though I will continue to be monitored with regular PSA tests Dr Old Guy has declared me cancer free In the grand scheme of things the dreaded side effects have been little more than insignificant annoyances I am grateful for a good God wonderful doctors supportive IDA members terrific staff and loving family Lesson 6 The first wealth is health Ralph Waldo Emerson Lesson 5 While not perfect we are still blessed with the best healthcare in the world If I m going to be sick I want to be sick in the good ole USA After a two day hospital stay I was recouping at home Dr Neal Richter an IDA past president and prostate cancer survivor called me the day he heard about my diagnosis and has been my coach and cheerleader through the entire process I still have the empathetic voicemail message he left 10 Mr Doug Bush is serving his 20th year as IDA Executive Director He can be reached at doug indental org Journal of the Indiana Dental Association Spring 2017 Volume 96 Issue 2

MEMBER ZONE All Politics is Local Dr William B Risk In January the Indiana Dental Association had its annual Dental Day at the Capitol event Thirty IDA members and their guests went to the State House to represent their profession and discuss dentistry s concerns about Donated Dental Services funding student loan forgiveness and uncovered dental benefits and how addressing these issues would benefit the citizens of Indiana It is important for dentistry to be at the legislature while it is session to emphasize how important these issues are to the dental profession The problem is that this important event occurs only one day out of the year Even though the IDA has a very capable lobbyist Ed Popcheff who is working at the State House every day the legislature is in session this is not enough It is the personal individual contact with the lawmakers throughout the year that is really the most effective way to deliver dentistry s message Let s face it Dental Day at the Capitol is inconvenient I have patients scheduled Indianapolis is a long way away little Suzy s birthday is that day I don t know any legislators these are just a few of the excuses we can use to stay at home In Indiana we are privileged to have a part time citizen legislature This means our lawmakers live among us throughout the year During the legislative sessions our lawmakers generally spend Monday through Thursday in Indianapolis doing the business of the State and looking after the interests of the citizens of Indiana Friday Saturday and Sunday they are at home looking after their personal affairs explaining why they voted the way they did and running for reelection and this is only in the spring During the rest of the year they are available locally They can be seen in restaurants on the golf course at ribbon cuttings and other events They also spend this time learning of the concerns of their constituents and finding ways to address these concerns 12 This makes it possible for each of us to become acquainted with our senators and representatives and be an advocate for dentistry all year long with very little disruption of our daily life While Dental Day at the Capitol is quite important we can still be an effective spokesperson for our profession at home How can we do this On the internet go Indiana General Assembly and determine who your legislators are Next click on the legislators names and go to their web pages under either the republican or democratic caucus Here Dr Bill Risk speaks with a legislative aid as he prepares to present dentistry s concerns during IDA s Dental Day at the Capitol on January 25 2017 Journal of the Indiana Dental Association Spring 2017 Volume 96 Issue 2

Special Section Annual Session Journal IDA Wall of Wine Back by Demand Our first ever Wall of Wine was such a huge success last year in fact every bottle of wine was pulled that we re bringing it back again this year If you didn t attend last year s Annual Session you might not be familiar with our Wall of Wine Picture a huge vintner rack nestled into each cubby is a bottle of wine But the bottles are wrapped in paper bags so there s no way of knowing what s inside each individual bottle IDA members IDPAC members and their families will have the opportunity to purchase a ticket entitling them to one mystery bottle from the vintner rack The excitement comes as they unwrap their bottles from the paper bags did they win something valuable something unique something exotic something rare But the real excitement comes in knowing they ve become part of the political process because each ticket sold benefits IDPAC Last year all 14 IDA components and many IDA members either donated cases of wine or donated funds to purchase cases of wine Several members donated bottles of wine that were valued at 50 or more Members had a 20 chance of pulling a premium bottle of wine And if you purchased your tickets after I did your odds were greatly improved In 2016 we had a total of 240 bottles of wine and raised 5 975 We are hoping to double the number of bottles in the wall this year Thank you to everyone who donated wine last year and those who have already submitted donations for 2017 If you re looking for an easy and fun way to get involved with IDPAC why not donate a bottle of wine Silent Auction Bidding is Underway Did you know you can start bidding on Silent Auction items right now Once again we are utilizing the Give Smart online auction system and bidding is currently underway at indental org givesmart You ll need to register online in order to bid If you registered last year you will need to register again this year Fortunately you already know how quick and easy it is We re still collecting items for the Silent Auction which can be delivered to the IDA Central Office by 4 00 p m on Wednesday May 31 or contact Laurie Campbell to make other arrangements In the meantime if you have an item you d like to donate let us know about it by submitting the information online at indental org luau That will allow us to get your item listed at our Give Smart site and start collecting bids View Bid On Silent Auction Items at indental org GiveSmart Spring 2017 Volume 96 Issue 2 Journal of the Indiana Dental Association 39

Last Word Journal IDA OMG PMS Oh my God Pardon my syntax Dr Michael D Rader Please excuse my rambling wordy sentences and overwrought paragraphs I can t help it I m hopelessly stuck in the 20th century and just don t possess the essential tools to communicate in the digital age I m embarrassed to admit I m text and emoji illiterate I m blaming the French It all began when one of those Frog kings wrote R pondez S il Vous Plait at the bottom of a royal invitation Somehow that pretentious tradition crossed the Atlantic and has continued today although now we just write RSVP when we want to know how many Salisbury Steaks with seasonal vegetables to order for Buddy and Tiffany s wedding reception The acronym craze really picked up during WWII when disaffected solders sailors and Marines added SNAFU and FUBAR to the lexicon Common decency and Doug Bush PITA prevents me from defining these terms to those of a more delicate nature The additions have been for the most part gradual and used mostly as shorthand by specialized groups Examples such as SCUBA self contained underwater breathing apparatus and laser light amplification by stimulated emission of radiation are now in common use The use of acronyms initially a simplification replacing long technical or profane terms was never meant to replace language Today we text LOL laughing out loud or PML pee myself laughing to signal when we are amused Marsha just isn t Jan s friend but her BFF best friend forever Got a good idea for the weekend SLAP Sounds like a plan That great idea judged to be a lame No problem prevent teenage angst and ridicule by texting JK just kidding The critical component for the weekend is the parents Everything s good they re SITD still in the dark Can t talk now P911 parent alert PAL parents are listening or PSOS parents standing over shoulder Finding it s too much work to text that Mr Smith the chemistry teacher is a boring lecturer Simplify your critique of Mr Smith with B 3 Blah Blah Blah Let your BFF know you re worried about the midterm by texting FC fingers crossed and when the grade is worse than expected KMN kill me now We haven t even begun to mine the emoji smiley faces frowns tears hearts flowers cats astrologic signs sport symbols primed to replace words thoughts and emotions When a cultural trend jumps from teen usage to the business world we need to take note JP Morgan eliminated voicemail for thousands of their employees when two thirds no longer used the phone preferring text messaging instant messaging or social media to reach others Coca Cola has followed suit while Citibank and Bank of America are considering the idea History has shown us that as the workforce becomes filled with younger workers old technologies disappear Studies consistently show Millennials text at much higher rates than their older coworkers Text messages are replacing phone calls as the preferred mode of communication Business consultants have found Millennials are more task oriented They avoid small talk and the phone which many find overly intrusive This getting down to business mentality while efficient doesn t come without peril An abbreviated text message is more likely to miss the intended meaning and tone while a phone call is less likely to be misinterpreted And we haven t even considered the confusion caused by those pesky autocorrect mistakes Texting also allows the lazy cowardly and guilty to skirt face to face conversations and confrontations Here the baby boomers and X ers fare no better In a survey 59 of those surveyed thought it acceptable to break up with someone via text message The use of texting will always be a problem for the sales department A good salesman needs to build rapport ply his sales pitch and counter objections in real time My college roommate a great salesman avoided using the phone for sales He always favored a face to face sales call even telling potential customers when asked why he was flying across country to make his presentation You re less likely to say no to my face Society and the workplace are changing as new technology enters common use The advantages are often overstated as frequently as the dangers are minimized Some day it will be SOP standard operating procedure in every office to appoint and confirm patients without using a phone Whether via a text instant message or some not yet imagined technology we risk losing the human contact that defines the unique qualities of every practice The kindly and reassuring phone call the evening following a difficult procedure might also become obsolete as technology marches inexorably on If this leads to diminished or lost human touch in the practice of dentistry that would be TBSS too bad so sad About Dr Rader Dr Michael D Rader is a general dentist practicing in South Bend IN He can be reached at drrader sbcglobal net Spring 2017 Volume 96 Issue 2 Journal of the Indiana Dental Association 45
