www.fivetownslink.com Individuals in our Community Rachel Zelmanovich This summer’s Ice Bucket Challenge raised awareness for Amyotrophic Lateral Sclerosis, also known as ALS. What most people didn’t realize, however, was that ALS is the adult version of a debilitating disease found in children known as Spinal Muscular Atrophy or SMA. Rachel Zelmanovich, of North Woodmere, has a 7 year old daughter, Emma, battling the genetic disorder. SMA deprives Emma of physical strength by affecting her spinal cord’s motor nerve cells as well as her ability to walk, stand, eat and breathe. Here is what Zelmanovich has to say about SMA and the effects it has on her family. Elana Dure: How did you ind out your daughter had SMA? Rachel Zelmanovich: It was interesting because she was doing everything typical until the age of six months. Then we noticed that she stopped crawling and that she started having some muscle weakness in her arms that caused tremors and shakiness. We took her to neurologists and they did a genetic testing on her. She was about 13 months old when we got a diagnosis. ED: What are some of the challenges you and your family encountered? RZ: Our home is a huge challenge because our home is not modi ied to her needs. She is in a motorized wheelchair so she is very limited to how she can get around the house. Another challenge is that I also have two other children. The amount of attention that is given to my daughter with special needs is extremely high. She can never be left alone because she is very fragile and weak and has no re lexes to protect herself if she falls. ED: Do you think your children have the same sibling dynamic that is found in other families? RZ: It’s like the typical sibling rivalry. It’s the day-to-day sisterly love. My other daughters are de initely helpful. I’m sure they don’t fully understand what’s involved, but they do know that they need to be gentle with her and they can’t be forceful. They are very careful with the way that they are around her. ED: Is your daughter able to go to school and be in regular classes? RZ: Yeah. The thing about SMA is that it doesn’t affect the brain. We thank God for that because it might be a physical challenge, but we get to hear her tell us every day that she loves us and we can tell her every day that we love her. Emma goes to HAFTR and is currently in 2nd grade. HAFTR welcomed Emma with open arms and has gone far and beyond to make Emma’s life as normal as possible. HAFTR’s faculty has made all the necessary accommodations for Emma, and in some cases have changed day to day activities of the entire school to make Emma it in. HAFTR is incredible. We are also so lucky to have Lauren as her shadow. ED: Wow. That is really special. I’m curious to know, what kinds of support have been helpful to you and your family? RZ: Chai Lifeline has gone far and beyond. I have no words for them. They would send young girls to come and play with my other kids. They would send us on family getaway trips just to give us some normalcy in our lives. My daughter just underwent surgery back in July down at John Hopkins Hospital [in Baltimore]. They knew we were coming from New York so they hooked us up with meals and housing. It was nonstop help and support. They were absolutely amazing. I tell her shadow all the time, if there are kids in school who are clearly not feeling well, don’t keep her away from them because I don’t want to make the kids feel bad, but clean her hands and wipe her desk. We can’t prevent if she gets sick—we’ll deal with it when it happens—but we try really hard for that not to happen. ED: Do you have any advice for parents who may be going through a similar situation? RZ: Where we are now as a family is a totally different place from ive years ago. One thing we learned is that the best thing you can do is talk about it. Share your feelings and realize that you are not alone. You are not the only family on Long Island that has a child with special needs. You can talk to other families and ind out what their experiences are. Five years ago, I would never have done that. It takes time. ED: If you were able to tell the world one thing about SMA, what would you want them to know? RZ: Children with SMA are very special. They are bright, loving kids who were robbed of the very basic aspect of life that we all take for granted. We should all be mindful that every dollar we contribute really helps make a difference in the lives of SMA children. There are three amazing organizations that work very hard to help so many families. They are Chai Lifeline, Muscular Dystrophy Assoc. (MDA), and Families of SMA (FSMA). They do amazing things for so many people. Hopefully we’ll be able to ind a cure with the money we raised for research, because there is currently no cure for SMA. CLEAN EATING Shallot Brisket Ingredients: 1 3-lb flat-cut beef brisket 2 tsp Chinese five-spice powder 1 tsp each sea salt and fresh ground black pepper 8 shallots, cut into 1/4-inch slices (about 1 1/2 cups) 1 3/4 cups low-sodium beef broth 3 Tbsp balsamic vinegar 2 Tbsp arrowroot powder Instructions: Preheat oven to 375°F. Sprinkle brisket on both sides with five-spice, salt and pepper. Arrange brisket in a 9 by 13-inch baking pan. (Note: The side of brisket with more fat should be facing up.) Arrange shallots on and around brisket. Bake, uncovered, for 30 minutes. Meanwhile, in a measuring cup, combine broth, vinegar and arrowroot, whisking to dissolve arrowroot. Remove brisket from oven and pour broth mixture into pan. Cover tightly with foil and continue baking until meat is very tender, about 3 hours. Remove brisket from oven and uncover. Let cool in pan juices for 30 minutes. Transfer brisket to a cutting board and slice meat against the grain. Serve meat with shallot sauce mixture spooned on top. Approximately 225 calories for a serving of 1/2 oz brisket and 1/4 cup sauce. 1/

PREVENTING STREP THROAT INFECTIONS I was familiar with probiotics, and frequently recommended probiotics to treat or prevent diarrhea. But for Strep? David Elazar Simai, M.D. Strep Throat is one of the most common pediatric illnesses. The bacteria responsible for this infection are scienti ically called Group A Streptococcus, but the disease became colloquially known as “strep” throat. The biggest fear of all is the one of neglected strep. In some patients, untreated strep infections may elicit the body to produce antibodies that target the heart valves and cause a heart condition known as Rheumatic Fever. Strep cannot be diagnosed over the phone. While doctors may point out that classic strep infections are not accompanied by diarrhea, clear runny nose & a chesty cough – there are always exceptions. No matter how experienced the doctor was, the study showed, he was only correct in predicting the strep results in 50% of the times after examining the patients. As I like to mention to parents while we chat and wait for the rapid strep results to complete, “some of the worst looking throat are not caused by strep, and surprisingly, some of the best looking throats are infected with strep”. You should also know that the rapid strep test does have a margin of error on both sides. The rapid strep test can be falsely positive and falsely negative. Because strep could be elusive, every child with a sore throat should be tested with a rapid and an overnight strep test in order to properly diagnose the disease. Prevention Until a few years ago, the only advice I shared with parents concerned about recurrent strep infection was to make sure that their children are well rested. I still feel that this is a valid point. It is well known that our immune system “cleanses” the body at nights. This is precisely the reason that when one is ill, s/he spikes high fevers mostly at nights. About 4 years ago, my oldest sibling Daniella Weill, was visiting in New York. Since she was studying at the time at Maayanot Institute in Jerusalem to become a “Naturopathic Doctor”, I encouraged her to shadow me at the of ice and get some exposure to pediatric illnesses. At one point, we saw a patient with strep throat. Daniella almost cried when I wrote the prescription for Amoxil. “What then, would you suggest for Strep?” I asked. “Probiotics.” she answered. I was familiar with probiotics, and frequently recommended probiotics to treat or prevent diarrhea. But for Strep? I politely suggested that she may have misclassi ied this piece of information, but Daniella insisted that she was right. At home, my second oldest son had just recently inished a course of antibiotics for his 6th strep infections in the span of 3-4 months. We tried Amoxil as well as stronger antibiotics but to no avail. I decided to test Daniella’s idea. From that day, my son would have a yogurt with probiotic once a day. After a few weeks, my mother taught us how to make homemade yogurt which tastes much less sour and has no sugar added. My son remained strep free for about 12 months. It was during a major snowstorm that we ran out of yogurt and a few short days later, he was on Amoxil again. After this “mini trial” at home, I decided to share this news with my patients. While I could not guarantee that the kids will remain strep free, I felt that it could do no harm for kids to have yogurt with probiotics. As expected, the kids who started eating yogurts daily did not get sick with strep as often, and my mother’s yogurt recipe was now used in many households. On occasion, I did see some “yogurt failures”, but for the most part, the hypothesis held true. The explanation I came with is that the billions of good bacteria found in the yogurt may compete with the harmful bacteria in our throats. Giving probiotics would correct the imbalance of bacteria in the stomach and make the throat less attractive for strep. I have searched to ind a scienti ic study that proves this hypothesis but did not ind any. Thus, I cannot claim that this idea is scienti ically proven nor medically accepted, but I would like to coin this as the “SimaiWeill Hypothesis”. Having said that, I do feel that there is no harm in eating yogurt. So, if your kids have been infected with strep often, you should try and give them a yogurt daily for 6 months. Any yogurt with active culture may help, but I suggest that you start with plain yogurts. If you child is allergic to milk, you may start supplementing with probiotic powder. These can be added to any luid or food as long as they are not brought to a boiling point. One of the most helpful mothers from my practiced just shared with me today that she perfected my mother’s yogurt recipe by visiting the website makeyourownyogurt.com. (I must state that I have no vested interest in any yogurt producer or website, my only concern is your children’s health). I would love to hear feedback from my readers who tried this “yogurt hypothesis”. I feel that this may be a very helpful advice, especially in crowded neighborhoods, where the contagious strains of strep spread so rapidly. Even the most disciplined individuals struggle with healthy eating during the holidays. Here are some trustworthy tips to navigate through the holiday season pitfalls and avoid gaining the extra pounds.
