These will help you to get the best from the services we offer. We ask that you use our services responsibly and that you keep any appointment you make.  People involved in your care will give you their names and ensure that you know how to contact them - Please let us know if you change your address or name. Please let us have your telephone number. As a Practice we also have responsibilities to our staff, other patients and visitors. Mindful of there responsibilities, we have a ‘zero tolerance’ approach to aggression and violence. Patients who are aggressive or who behave in a violent way (including threats of violence) towards members of the Practice will be removed from the Practice’s list. Please note that following guidance from the Department of Health on eligibility to receive NHS care, if you leave the UK for a period of over three months you are no longer entitled to remain registered with the Practice and we are required to return your health records to the local area team. This also means that the Practice is not permitted to provide you with supplies of NHS medication to cover your period of residence outside the UK. 7

How to Register with the Practice Please contact reception to arrange to register with the surgery. You will be asked to complete a registration form and patient questionnaire. The registration form and patient questionnaire can also be completed online at www.kilmenysurgery.co.uk. You do not need to see a doctor in order to register with the practice unless you want or need to be seen. You will be given an appointment for a health check with the nurse. IT IS PRACTICE POLICY THAT Identification is required from all new patients (whether registering permanently or temporarily). A combination of the following can be accepted as identification:              birth certificate marriage certificate medical card driving licence passport local authority rent card paid utility bills bank/building society cards/statements National Insurance number card payslip letter from Benefits Agency/benefit book/signing on card papers from the home office P45. The following documents can not be accepted as proof of identity  library card  video rental card  health club card  private rent book. How to Arrange to See a Doctor Our receptionists offer advice to assist you in making your appointment. Please say if you would prefer to see a particular doctor. All our staff and receptionists are bound by the same rules of confidentiality as the doctors. Surgeries are by appointment only throughout the day. Some pre-bookable appointments are available up to a month in advance. Other appointments must be booked on the day. The Practice is open from 8.00 am to 6.00 pm Monday to Friday (excluding bank holidays). We also hold Commuter Clinics which offer late appointments for those who work or cannot attend during normal surgery hours because of caring commitments. Appointments are available on Monday mornings from 7.30 am and on Monday and Thursday evenings until 8.00 pm. Some problems can be dealt with easily over the telephone. If you feel that this is the case, please ask the receptionist to book a telephone appointment. At times when requesting an appointment you may be asked to speak to a doctor or nurse about your problem. The receptionist will ask you for a number where you can be contacted and the doctor or nurse will ring you back as soon as possible. Please arrive promptly for your appointment and if you are unable to keep your appointment please cancel with as much notice as possible so that the appointment can be offered to someone else. 10

IMPORTANT – CANCELLING APPOINTMENTS If you are unable to attend any appointment please contact us as soon as possible so that someone else can use the appointment. Appointments can now be cancelled by text message on 07846 009901 Home Visits If you are too ill to come to the surgery a doctor may visit you at home. This would normally mean that you are confined to bed or otherwise physically unable to come to the surgery. We do ask you to come to attend the surgery whenever possible. The receptionist may ask for further information to enable the doctor to determine the priority and nature of the visit. The doctor may telephone you before the visit Urgent Problems – Out of Hours Service At nights, bank holidays and weekends ring the surgery number 01535 606415 to be transferred to the out of hours service which is run by NHS 111 Disabled Access Both Kilmeny and Long Lee surgeries have been designed with disabled access. All rooms used by patients are accessible by a wheelchair. A hearing loop is available at both surgeries – please ask at reception. There is a disabled toilet at both surgeries. 11

MAKING APPOINTMENTS & REQUESTING REPEAT PRESCRIPTION ONLINE If you are aged 16 or over and would like to be able to make appointments on-line or request your repeat prescriptions on-line you will need to come to register in person at either surgery reception and bring the following documents with you:  a current form of photo ID eg driving licence or passport  one utility bill or mobile phone bill or a bank statement bearing your name / family name. Ask the receptionist to register you for online prescriptions and appointments. Parents can register children under 16, who live with them, for on-line services. Proof of the parent’s identity must be provided (as above). The receptionist will give you a print out showing your user name and password. This will not be valid until 4 hours later. Go to the surgery website to register www.kilmenysurgery.co.uk and follow the instructions. You will be asked to change your password when you log on for the first time. WE NOW OFFER ELECTRONIC PRESCRIBING YOUR PRESCRIPTION IS SENT ELECTRONICALLY TO THE PHARMACY OF YOUR CHOICE PLEASE SEE YOUR PHARMACIST TO SIGN UP FOR THIS SERVICE How to Get a Repeat Prescription Repeat prescriptions should be ordered at least 48 hours in advance. They can be ordered online at our website www.kilmenysurgery.co.uk (pre-registration is required – please see the back page of this leaflet) or by telephoning the surgery after 11.00 am Monday to Friday or by sending in the tear off slip from the side of your prescription. Please tick the items you require. If you would like your prescription posting to you please remember to enclose a stamped addressed envelope and allow extra time for posting. Alternatively you may come to an arrangement with your usual pharmacist to leave your request there and pick up your medication at a later date. This will usually take longer than 48 hours. Please do not request additional items on your slip unless this has been agreed with your doctor. Please note when your review is due. This is the date on the bottom of the tear off slip. When your medication is due near this date please make an appointment to see your usual doctor. If you have a condition where you have to have blood tests, please attend a phlebotomy clinic at least one week before your review, so the doctor has the test results. 13

How to get test results If your doctor has told you that you can ring up for the results of blood or urine tests please ring the surgery after 11.00 am. Results can only be given to the patient or a parent in the case of children under 16. The results of smear tests are posted to you. X-Ray and scan results may take up to three weeks to come back. Please make a telephone appointment with the doctor who ordered the x-ray or scan to get your results. Are you seeing the right person?  If you are feeling unwell and not sure if you need to see a doctor, you can get advice from a pharmacist.  Advice and information can also be obtained from NHS Direct on 0845 4647 or on their website www.nhsdirect.nhs.uk  Another useful website for information is www.patient.co.uk  If you have problems with your teeth you should see a dentist. If you do not have a dentist contact NHS 111 For family planning advice, make an appointment with the family planning nurse.   If you have children under 5 years of age the Health Visitor can give help and advice with many problems. Data Protection Record keeping in the practice is computerised. The practice is registered with the data protection agency. We take our responsibilities for keeping your records private, very seriously. Your medical records are kept on a strictly confidential basis. You have a right of access to your medical records. Access to your medical records by clinicians outside the team (for example hospital doctors) is only with your explicit consent. Access to other agencies, for example solicitors and insurance companies is only with your written consent. Staff Training The surgery closes every Thursday lunchtime from 12.30 to 1.30 for staff training. On every second Thursday of the month the surgery closes from 12.30 to 5.00 pm again for staff training. This event includes all surgeries in the area.
